If a college student seems depressed or mentions suicide, respond calmly, ask directly about suicide, listen without judgment, and connect them with immediate or ongoing help. Asking whether someone is thinking about suicide does not increase suicidal thoughts or behavior, according to the National Institute of Mental Health (NIMH).
If the student has a plan, has started harming themselves, says they intend to die, or faces an immediate medical danger, stay with them, involve another trusted person, and get emergency help. In the United States, call 911 for imminent danger or a life-threatening emergency; call or text 988 for a suicide, mental-health, or substance-use crisis. Campus procedures and after-hours services vary, so use the college’s current official contacts as well.
First decide whether there is immediate danger
You do not need to diagnose depression or calculate a risk score. Your first decision is whether the student may be in immediate danger. Treat the situation as urgent if they say they are going to kill themselves, describe a current plan or intent, have access to a means and appear ready to use it, have already injured themselves or taken an overdose, or have a life-threatening medical problem.
| What you observe or hear | What to do |
|---|---|
| Immediate danger, a suicide attempt, serious self-harm, overdose, or a life-threatening injury | Call 911 in the United States or go to an emergency department. Stay with the student and follow the dispatcher’s instructions. |
| The student says they are thinking about suicide, but no immediate attempt or emergency is apparent | Stay present, ask calmly about safety, and call or text 988 together. Involve another trusted person and contact campus crisis or counseling services. |
| Concerning changes such as withdrawal, hopelessness, or increased substance use without a stated crisis | Arrange a prompt conversation, ask directly about suicide, and offer a concrete connection to professional or campus support. |
Do not leave someone alone after they say they are going to kill themselves. NIMH advises bringing in a trusted person, seeking crisis or emergency help, and not promising secrecy. If you are unsure whether danger is imminent, explain what you are seeing to 988, a campus crisis service, or 911 and let the responder help determine the next step.

Start the conversation with an observation and care
Choose a reasonably private setting and a time when neither of you has to rush. You can speak in person, by phone, or by video if distance makes that necessary. Begin with what you have noticed rather than a label or accusation.
A simple opening
“I’ve noticed you seem overwhelmed and have been keeping to yourself. I care about you. Do you want to talk?”
Specific observations are easier to answer than “What’s wrong?” For example, you might mention missed classes, a sudden change in sleep, not replying to messages, giving away belongings, or seeming unusually agitated. Keep the focus on concern, not on proving that the student has a disorder.
Use an open invitation
Give the student room to choose how much to say. Try “What has this week been like?” or “What feels hardest right now?” Avoid turning the first conversation into an interrogation. A quiet pause can give them time to answer.
Ask directly about suicide
If suicide is on your mind, ask plainly: “Are you thinking about suicide?” You can also say, “Have you been thinking about killing yourself?” NIMH states that studies show asking whether someone is suicidal does not increase suicidal behavior or thoughts. SAMHSA likewise says asking will not put the idea into someone’s head or make an attempt more likely.

Use ordinary language and then wait for the answer. Do not soften the question so much that its meaning is unclear. Asking directly does not force a confession, and a “no” does not prove that everything is safe. You can follow with, “When did those thoughts start?” and “Are you in immediate danger right now?” If the student says yes, stay calm and move to the safety steps below.
Listen without judgment or a rushed fix
Your job in the first conversation is to understand and help the student reach appropriate care, not to win an argument or solve every cause of distress. SAMHSA recommends listening without judgment. You can acknowledge pain without agreeing that hopeless conclusions are true.

- “Thank you for telling me. I’m glad you told someone.”
- “That sounds painful and exhausting.”
- “I’m here with you while we find more help.”
- “What kind of help feels possible right now?”
- “Would you like me to sit with you while you call counseling or 988?”
Responses that can shut the conversation down
- “You have so much to be grateful for.”
- “Everyone feels this way in college.”
- “You must promise me you will never do that.”
- “Think about how this would affect your family.”
- “I know exactly how you feel.”
- Rapidly offering solutions before hearing what the student is saying.
Do not promise to keep suicidal thoughts secret. Explain that you will respect their privacy as far as you can, but safety may require involving a trusted person or emergency professional. Tell them what you are going to do before you make a call whenever circumstances allow.
What to do when the student may be in immediate danger
- Stay with them. Keep the student with you, or make sure they are in a private, secure place with another caring person. Do not leave them alone to “calm down” if they have expressed intent or have harmed themselves.
- Call emergency help for life-threatening danger. In the United States, call 911 or go to an emergency department for an attempt, serious injury, overdose, or imminent danger. State the location, what the student said or did, whether weapons or medications are involved, and any urgent medical symptoms.
- Use 988 for a crisis conversation. Call or text 988 in the United States for suicide, mental-health, or substance-use crisis support, including concern about another person. SAMHSA describes the service as available 24/7 and also lists online chat at 988lifeline.org.
- Bring in another trusted person. This might be a parent or other family member, roommate, resident-life staff member, instructor, adviser, or campus safety professional. Choose someone who can stay calm and help with transportation or communication.
- Reduce immediate access to obvious hazards only when it is safe. Do not put yourself at risk or start a physical struggle. Tell emergency or crisis responders what dangerous items, medications, or substances may be present so they can give appropriate instructions.
If the student has already taken medication, used drugs, or injured themselves, treat it as a medical emergency even if they now say they feel fine. Do not wait for symptoms to worsen.
Use the right support for the level of need
911, 988, campus services, and ongoing treatment are not interchangeable. They address different situations.

911 or an emergency department
Use 911 in the United States when danger is imminent or there is a life-threatening emergency. A medical emergency, a suicide attempt, severe intoxication, or serious injury takes priority over arranging a routine counseling appointment.
988
Call or text 988 when the student or supporter needs immediate crisis support but the situation is not clearly a life-threatening emergency. You can contact 988 about a loved one. Keep the student involved in the conversation when possible, and follow the counselor’s directions.
Campus and community services
For non-emergency support, help the student contact the college counseling center, student-support office, residence-life staff, or the institution’s published after-hours crisis service. The correct procedure differs by college and can change, so find the current information on the institution’s official website, student portal, orientation materials, or residence-life communications. A community mental-health professional or primary-care clinician may also help with assessment and treatment referral.
Recognize warning signs without treating them as a diagnosis
NIMH’s warning-sign guidance says to get help as soon as possible when signs are new or increasing. One sign alone does not establish that a student is suicidal, and a friend or roommate cannot diagnose depression from a checklist. Several changes together, a sudden escalation, or any direct talk about death deserves prompt attention.
- Talking about wanting to die, killing themselves, or having no reason to live.
- Expressing hopelessness, unbearable emotional or physical pain, feeling trapped, guilty, ashamed, or like a burden.
- Making a suicide plan, researching ways to die, obtaining materials, or discussing how and when it could happen.
- Withdrawing from friends, family, classes, activities, or ordinary communication; saying goodbye; or giving away important possessions.
- Extreme sadness, agitation, anxiety, anger, or sudden mood swings.
- New or increased dangerous risk-taking.
- Major changes in eating or sleeping.
- Increased use of alcohol or drugs.
Approach these signs as reasons to check in and connect the student with help, not as evidence that you know their diagnosis or level of risk. A student can be in danger without displaying a visible warning sign, and a student can show one of these behaviors for reasons unrelated to suicide.

Help the student take one concrete next step
Depression, fear, shame, exhaustion, financial stress, academic pressure, or substance use can make even a simple phone call feel impossible. Offer practical help rather than a vague instruction to “get help.”
- Ask which option feels least difficult: campus counseling, a doctor, a trusted family member, 988, or another qualified professional.
- Offer to sit beside them while they call, text, or complete an online appointment request.
- Help them write down what to say: “I’m a college student and I’m having thoughts about suicide. I need an urgent appointment.”
- Offer transportation or help identifying the college’s after-hours process, without taking control away from the student when safety permits.
- Ask what barrier is stopping them. They may be worried about cost, privacy, academic consequences, stigma, scheduling, or being misunderstood; a counselor or 988 can explain available options.
SAMHSA notes that it can take several conversations before someone accepts help. If the student declines a non-emergency referral, remain available, keep checking in, and return to the direct safety question if new warning signs appear. A refusal does not cancel your responsibility to act if danger becomes imminent.
Plan follow-up after the first conversation
Support should not end when the student says they feel somewhat better or completes one call. NIMH recommends follow-up after a crisis. Agree on a specific time to check in rather than saying only “call me if you need anything.”
A practical check-in plan
- Set the next contact: for example, a call later today and another check-in tomorrow.
- Ask what support they accepted and whether they reached a counselor, 988, a doctor, or another trusted person.
- Ask again about safety if their mood worsens, they stop responding, or new warning signs appear.
- Help with ordinary stabilizing tasks they request, such as food, a ride, class communication, or contacting a support person.
- Keep a short list of the college’s counseling and after-hours numbers, 988, and 911 where you can find it quickly.
Do not make yourself the student’s only support. With the student’s participation when possible, widen the circle to include qualified professionals and trusted people who can share responsibility.

You cannot force someone to accept routine care. SAMHSA advises that the best thing a supporter can do is be present and listen without judgment. Say, “I respect that you are not ready for that appointment. I am still concerned, and I want to stay connected. Can we decide what you will do if the thoughts become harder to control?”

Ask whether they will contact 988, a campus crisis service, or another trusted person if safety changes. Continue specific, non-judgmental check-ins. If they disclose a plan, intent, an attempt, or another imminent danger, do not treat refusal as the final decision: call 911 or seek emergency help.
Protect the student’s privacy while acting on safety
College students may be adults who control their own health information. You can still give a counselor, 988, campus crisis staff, or emergency responders the information you have, even if the student will not sign a release. Tell the professional what you observed, what the student said, when it happened, and where the student is. Ask what you should do next.
A privacy concern should not stop you from seeking emergency assistance. Explain to the student that you are sharing only what is needed to keep them safe, and avoid posting details in group chats or on social media.
Support yourself and set sustainable boundaries
Being worried about a student can be frightening and exhausting. You can care deeply without becoming their therapist or staying available every minute. Tell another trusted adult or professional what is happening, protect your own sleep and safety, and ask 988 or campus staff for guidance about your role.
Do not agree to be the sole person who monitors the student, stores dangerous items, or guarantees that nothing will happen. If you are threatened, feel unsafe, or cannot remain with the student during an emergency, move to a safe location and call emergency services. A clear boundary such as “I care about you, and I need to involve someone who is trained for this” is an act of protection, not abandonment.
Geography and contact limits
The 988 and 911 instructions above are for the United States. Readers elsewhere should use their country’s local suicide-crisis and emergency numbers. A campus counseling center may not provide 24-hour response, and its referral or notification rules vary. Verify the college’s current contacts and after-hours instructions rather than relying on an old handbook or an unverified directory.
When in doubt, prioritize immediate physical safety, stay with the student when it is safe to do so, ask directly about suicide, and involve a trained crisis or emergency professional.
