Start with connection, not a diagnosis: choose a calm moment, describe what you have noticed without labeling your child, listen without interrupting, and ask what kind of support they want. Stay on their team while you watch for symptoms that persist for weeks, disrupt daily life, or create safety risks. A pediatrician or primary-care clinician can help decide what happens next, and urgent danger requires immediate crisis help.

What supportive, stigma-free help looks like

Mental health is part of overall health. Children function and develop best when they have steady, supportive relationships and environments. A mental-health struggle is not a moral failing, bad character, or something a child caused by not trying hard enough. Your job is to make it safer to talk, notice changes, involve appropriate adults, and escalate care when needed—not to diagnose your child at home.

Use partnership as the organizing principle. UNICEF pediatrician and adolescent-medicine specialist Dr. Hina Talib recommends beginning from a positive place, choosing a calm moment, listening more than talking, noticing your own first reaction, and asking how the child wants you to respond. As she puts it: The most important thing is to remind them that you’re on their team no matter what. (UNICEF Parenting).

That approach protects dignity while still taking symptoms seriously. You can be clear that help is available without implying that your child is broken or permanently defined by a diagnosis.

How to start the conversation

Choose conditions that lower pressure

  • Pick a relatively calm time, not the middle of an argument, rushed school morning, or public setting.
  • Try a side-by-side activity such as walking, driving, cooking, or sitting with a pet. Eye contact can feel intense for some children.
  • Allow enough time that your child does not have to deliver a complete explanation immediately.
  • If speaking feels too difficult, offer texting, a note, or a conversation with another trusted adult as a first step.

Open with observations, not labels

Describe specific changes you have seen and leave room for correction. For example: “You have seemed exhausted, stopped seeing friends, and missed several assignments. I care about you and want to understand what is going on.” Avoid opening with “You are depressed,” “You are anxious,” or “You are acting crazy.” Those statements claim certainty, invite an argument about a label, and can make a child hide information.

Use an open invitation

UNICEF’s suggested closing prompt is simple: What’s on your mind? You can also ask, “What has been the hardest part of this week?” or “Would you rather I listen, help you think through options, or help arrange support?” Ask one question at a time and tolerate pauses. A short answer does not mean the conversation failed; it may be the first evidence that talking with you is safe.

Watch your first reaction

Surprise, fear, anger, or rapid problem-solving can shut down disclosure. Before responding, take a breath and lower your voice. Thank your child for telling you, even if the information is upsetting: “I’m glad you told me. We can work out the next step together.” If you react poorly, repair it directly: “My face and tone looked alarmed. That was about my worry, not blame toward you. I’m listening.”

Make listening visible

Active listening is a behavior, not just an intention. The Centers for Disease Control and Prevention defines it this way: Active listening is letting your child share their thoughts and feelings—without interruption. Its conversation guidance, reviewed November 29, 2024, recommends creating regular opportunities to talk and listening without interrupting.

  1. Pause your agenda. Put down the phone, stop typing, and give your child your attention.
  2. Reflect what you heard. Say, “It sounds like lunch has become stressful because you feel watched,” rather than immediately offering a solution.
  3. Check your understanding. Ask, “Did I get that right?” A child can correct you without having to defend a diagnosis.
  4. Validate the feeling. “That sounds lonely” acknowledges the experience. It does not mean you agree with every conclusion or plan.
  5. Ask permission before advice. “Do you want ideas, or should I just stay with you for a minute?” If there is an immediate safety risk, you do not need permission to seek emergency help.

Do not promise secrecy you cannot keep. Explain the boundary calmly: “I will protect your privacy, but if you may be hurt or someone else may be in danger, I need to involve adults who can keep you safe. I’ll tell you what I’m doing whenever I can.”

Language that reduces shame

Prefer behavior-based descriptions

Instead of Try Why it helps
“You are lazy.” “Getting out of bed and starting work have been difficult lately.” Names a change that can be understood and addressed.
“You are an anxious kid.” “You have been worried about school and avoiding the bus.” Avoids turning a symptom into an identity.
“Stop being dramatic.” “I can see this feels overwhelming. Tell me what makes it feel unsafe.” Invites detail instead of punishment.
“Everyone feels sad; get over it.” “Sadness happens, and this seems to be lasting and affecting your days.” Normalizes emotion while recognizing impairment.

UNICEF explains that uncomfortable emotions are part of being human and that talking with loved ones supports mental health. You can normalize feelings without minimizing suffering: “Many people feel overwhelmed sometimes, and you deserve help with how intense this has become.” Do not use sarcasm, threats, public disclosures, comparisons with siblings, or a diagnosis as an insult.

Separate the child from the problem

Use phrases such as “the panic,” “these sleep problems,” or “what you are carrying” when that wording feels natural to your child. Ask what language they prefer. Some children find a diagnosis useful; others prefer “mental-health support” or a description of symptoms. The choice can change over time, and respectful language is more important than finding one perfect term.

How to tell a difficult phase from a problem needing professional advice

A single bad day, argument, or night of poor sleep does not establish a disorder. Persistence and interference matter more than one event. The National Institute of Mental Health (NIMH) advises contacting a professional when emotions or behavior last weeks or longer, cause significant distress, or interfere with life at home, school, or with friends. UNICEF similarly highlights symptoms lasting a few weeks and disrupting everyday functioning.

Changes that justify arranging an evaluation

  • Persistent sadness, irritability, fear, or hopelessness.
  • Marked withdrawal from family, friends, sports, hobbies, or activities previously enjoyed.
  • Major sleep or appetite changes, ongoing exhaustion, or unexplained physical complaints.
  • A drop in school attendance, concentration, grades, or ability to complete ordinary tasks.
  • Substance use, dangerous risk-taking, aggression, or sudden behavior changes.
  • Self-harm, talk about death, suicidal thoughts, or statements that life is not worth living.

Keep a brief factual record: dates, what you observed, what your child reported, changes at school or home, sleep and appetite patterns, medications or substances, and what makes things better or worse. Do not turn it into surveillance or interrogations. The purpose is to give a clinician a clearer starting picture.

What to do if your child refuses help

Refusal can mean fear of being judged, a previous bad experience, worry about confidentiality, uncertainty about what therapy involves, cost concerns, or simply not being ready to talk. Arguing about whether help is needed often increases resistance.

  1. Ask about the objection. “What worries you most about talking with someone?” Listen for a practical barrier and an emotional one.
  2. Offer choices. Your child might choose a pediatrician, school counselor, therapist, telehealth appointment, clinician of a particular gender, or a first appointment with you present. Choice restores control.
  3. Describe the first visit accurately. Explain that an evaluation may include a parent interview, information from school, and a private conversation or behavioral observation with the child. It is a chance to understand needs, not a test the child can fail.
  4. Start with a health visit if that feels less stigmatizing. A pediatrician or primary-care clinician can check physical contributors, discuss symptoms, and recommend next steps.
  5. Keep the door open. Say, “You do not have to decide everything tonight. I will check in tomorrow, and we can revisit which option feels least uncomfortable.” Continue ordinary connection and routines.

If safety is in question, do not wait for agreement. NIMH and UNICEF guidance support discussing concerns with a health provider even when a child has not given permission, especially when red flags need assessment. Share only information needed for safety and explain to your child what you disclosed and why whenever possible.

Where to begin and how support routes differ

There is no single best entry point for every family. Match the route to urgency, developmental fit, access, and your child’s comfort. CDC guidance identifies schools, primary care, insurance resources, trusted referrals, and professional locator services as ways to find a therapist.

Route Best starting use Parent involvement and coordination Questions to check
Pediatrician or primary-care clinician New or unclear symptoms, physical complaints, medication questions, or uncertainty about urgency. Usually includes caregiver observations; can coordinate referrals and medical evaluation. What evaluation is needed? Which services accept our insurance? What should we do while waiting?
School counselor, psychologist, nurse, or trusted staff member School avoidance, learning impact, peer problems, or a need for support during the school day. Can connect home and school plans, subject to local consent and privacy rules. What can remain confidential? Who will be informed? What accommodations or check-ins are available?
Licensed child or adolescent mental-health professional Persistent distress, impairment, self-harm concerns, trauma, anxiety, mood, behavior, or family conflict requiring focused care. Effective child psychotherapy often includes parent involvement, home or school skills practice, and progress tracking. What training and license does the clinician have? How are caregivers included? How is progress reviewed?
Family counseling or parent coaching Communication breakdown, recurring conflict, behavior-management needs, or support for caregivers. Caregivers are active participants; coaching can strengthen nurturing relationships and help manage behavior. Is the approach suitable for the child’s age, culture, language, and safety needs?
Emergency or crisis service Immediate danger, a suicide attempt, active self-harm with serious injury, threats to others, or inability to stay safe. Caregivers stay with the child when safe and follow emergency instructions. What is the fastest local route? What information and medications should accompany the child?

Before booking, check licensing, confidentiality rules for the child’s age, language and cultural fit, accessibility, geography, cost, insurance coverage, wait time, and whether the clinician has experience with the presenting concern. A child’s comfort matters, but it cannot override immediate safety.

What an evaluation and treatment may involve

NIMH notes that an evaluation can combine a parent interview, information from school, and a child interview or behavioral observation. Bring your observation record, relevant school information, medical history, current medicines, and questions. Tell the clinician about strengths as well as difficulties; treatment planning should build on what already works.

Possible supports include psychotherapy, medication when clinically indicated, family counseling, and parent support. The appropriate combination depends on the child’s symptoms, age, health, preferences, risks, and professional assessment. Effective child psychotherapy commonly includes caregivers, practice of skills at home or school, and tracking progress rather than relying on one reassuring conversation.

Make a treatment plan collaborative

  • Ask what the first goals are and how progress will be measured.
  • Clarify which sessions are private, what information will be shared with caregivers, and the safety exceptions.
  • Agree on one or two home practices that are realistic, such as a predictable bedtime routine, a daily check-in, or reducing a specific conflict trigger.
  • Schedule a review point. If symptoms worsen, side effects appear, or the fit feels wrong, contact the clinician rather than quietly stopping.

Middle-school children

Offer privacy and concrete choices. Ask about friendships, online spaces, bullying, school workload, body changes, and sleep without assuming the cause. A brief daily check-in can work better than a single intense conversation.

Teenagers

Respect growing independence and explain confidentiality before appointments. Ask whether they want you in the room, nearby, or involved only for scheduling and safety information, subject to local law and clinical policy. Do not search devices or disclose personal information as a first response unless there is a safety reason; discuss what you are worried about and what boundary is necessary.

Coordinate home and school carefully

With appropriate consent, share a concise plan with the school: the trusted adult, warning signs, practical accommodations, attendance expectations, and how urgent concerns will be communicated. Avoid broadcasting a diagnosis or private details to staff who do not need them. Review whether the plan is helping rather than assuming more monitoring is always better.

When the situation is an emergency

Act immediately if your child is unsafe, says they want to hurt themselves or someone else, has attempted suicide, has serious self-harm injuries, or may be in immediate danger. In the United States, call or text 988 for the Suicide & Crisis Lifeline. Use emergency services for life-threatening situations. Outside the United States, use your country’s emergency number or crisis service; availability and procedures vary by jurisdiction.

  1. Stay with the child, or keep a responsible adult with them, if doing so is safe.
  2. Use a calm, direct question: “Are you thinking about killing yourself or hurting someone right now?” Asking does not plant the idea; it clarifies risk.
  3. Move away firearms, medications, weapons, and other immediately dangerous items when you can do so safely.
  4. Call the appropriate crisis or emergency service and follow its instructions. Do not drive an acutely unsafe person alone if emergency responders advise another plan.
  5. Tell responders what was said, what was taken or used, injuries, medications, substances, access to lethal means, and any prior attempts.

After the immediate crisis, arrange follow-up and ask for a written safety plan. Keep communication open without turning every interaction into a risk interrogation.

Common mistakes that increase stigma

  • Turning symptoms into character judgments: replace “lazy” or “attention-seeking” with observable behavior and concern.
  • Demanding a complete explanation: children may disclose in fragments; keep future conversations available.
  • Making help a punishment: therapy should not be threatened as a consequence for grades, defiance, or embarrassment.
  • Promising absolute secrecy: explain safety limits before a crisis.
  • Over-sharing with relatives or school staff: disclose only what supports care and safety.
  • Trying to be the only helper: involve qualified professionals when persistence, impairment, or risk warrants it.
  • Abandoning the process after one poor fit: tell the clinician what felt wrong and seek a better developmental, cultural, or communication match.

A practical first-week plan

  1. Today: choose a calm moment, make a behavior-based observation, and ask what is on your child’s mind.
  2. Within 24 hours: note symptoms, duration, impairment, safety concerns, and your child’s preferred kind of support.
  3. Within a few days: contact the pediatrician or primary-care practice, and ask the school’s designated support person about available services and privacy rules.
  4. Before an appointment: gather school and medical information, insurance details, medications, and questions about confidentiality and parent involvement.
  5. Throughout: maintain ordinary routines, schedule low-pressure connection, praise help-seeking, and reassess safety whenever the child reports self-harm or suicidal thoughts.

Support is not a label. It is a series of respectful actions: listen, describe what you see, involve your child in choices when safety allows, coordinate with qualified professionals, and respond urgently to danger.
