Require the same engineering evidence for AI-generated pull requests as for any other change, then add explicit controls for accountability, test independence, and risk. Every AI-assisted pull request should name a human owner, pass the repository’s build, functional-test, static-analysis, dependency, and secret checks, and receive approval from a person who understands the change. Treat AI-authored tests as production code: inspect their assertions, look for deleted or weakened coverage, and add independent negative and boundary cases. Route security-sensitive or high-impact changes to qualified reviewers with stronger approval rules. A green pipeline or an AI-generated review is evidence that checks passed—not proof that the implementation is correct.

What the standard must accomplish

An effective policy answers five questions for every AI-assisted pull request (PR): who is accountable, what evidence is required, how tests are evaluated, when review is escalated, and what blocks a merge. The policy should apply whether a developer pasted a suggestion from an assistant, asked an agent to edit several files, or generated an entire feature. The amount of scrutiny can change with risk, but the responsibility cannot be delegated to the model.

OWASP’s DevSecOps Guideline says generated code needs at least the same scrutiny as code from an unknown contributor. GitHub’s guidance describes running automated tests and static analysis before human review. OWASP’s Secure Coding with AI Cheat Sheet warns that a passing test suite generated by the same agent that produced the implementation provides no independent assurance. Use those principles as the baseline, then express them as repository rules that your tooling can enforce.

Define accountability before code is merged

Name a responsible developer

Each AI-generated or AI-modified PR must identify a human owner. That person is accountable for functional correctness, security, operability, and future maintenance, even when an agent wrote most of the diff. The owner should be able to explain the design, important control flow, assumptions, and failure modes without asking the model to interpret the code during the review.

Record the owner in the PR’s normal assignee field or in a required metadata field. If your organization tracks AI assistance, add a label such as ai-assisted; the label is useful for routing and auditing, but it must not replace ordinary review. Do not count the generating agent, an automated code-review bot, or a status check as the required human approval.

Require a human approval that is meaningful

A reviewer should inspect the intent and context of the change, not merely click through green checks. Approval means the reviewer has read enough code and tests to judge whether the stated requirement is met, the architecture and conventions are respected, and the owner’s explanation is credible. A reviewer who cannot understand the generated code should request simplification, documentation, or a different implementation rather than approving on trust.

Declare the change’s risk

Ask the author to describe affected data, privileges, trust boundaries, production systems, dependencies, and reversibility in the PR description. A short declaration makes escalation predictable. It also prevents a small-looking AI diff from hiding a consequential behavior change in configuration, generated files, or a dependency update.

Baseline workflow for every AI-assisted PR

Use the following sequence as the default merge path. Repositories can add checks, but should not silently skip one because the code was generated.

  1. State the requirement and scope. The PR description should link the issue or requirement, summarize intended behavior, list changed components, and identify the responsible developer. Note where AI assistance was used if your policy requires that disclosure.
  2. Inspect the complete diff. Review source, tests, configuration, migrations, generated artifacts, dependency manifests, and deleted files. Ask the author to split unrelated refactors from the functional change; smaller diffs make generated mistakes easier to detect.
  3. Run deterministic build or compile checks. A successful build catches type, syntax, packaging, and generated-code errors, but it does not establish behavioral correctness.
  4. Run the project’s functional and integration tests. Use the normal test commands and environments for the repository. Preserve failures as merge-blocking evidence unless an authorized exception records why a failure is unrelated and how it will be resolved.
  5. Run static analysis and security automation. Use the linters, type checkers, analyzers, dependency checks, secret scanners, and other checks already required for ordinary PRs. Configure critical findings to block merging.
  6. Review tests as code. Examine whether the tests actually exercise the requirement and failure behavior. Check for deleted tests, weaker assertions, excessive mocking, and assertions that merely restate the generated implementation.
  7. Complete human review and risk escalation. Route ownership-sensitive files to qualified reviewers and apply elevated approval rules when the change affects security, infrastructure, or a high-impact production path.
  8. Record the decision. The approval, unresolved findings, exception authority, and any follow-up work should remain visible in the PR or linked change record.

Set a minimum evidence package

Write the baseline as a repository policy so it is enforceable rather than advisory. A practical requirement for every AI-assisted PR is:

  • A named human owner and at least one human approval from someone other than the generating agent.
  • Successful build or compile checks for the affected components.
  • Successful project tests, including relevant integration or end-to-end checks already used by the repository.
  • Passing static analysis and the repository’s normal dependency and secret checks.
  • A review of test-file changes, including additions, deletions, assertion changes, fixtures, and mocks.
  • A written explanation for any intentionally changed behavior, removed test, suppressed finding, or temporary workaround.

Do not invent a universal percentage for test coverage or a universal number of reviewers. The cited guidance does not establish either. Define local targets through your existing engineering and vulnerability policies, and raise them where the risk assessment demands it.

Review AI-authored tests for independence

Look for coverage that disappeared

Compare the test diff with the previous version, not just the new file in isolation. Flag deleted cases, reduced parameter sets, assertions replaced by broad “does not throw” checks, and fixtures that no longer represent realistic data. An agent may remove a failing test to make a generated implementation pass; the deletion is a product and reliability decision that requires an explicit human explanation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reject implementation-shaped assertions

A test is weak when it verifies the exact internal steps the model happened to write instead of the externally required behavior. For example, asserting that a particular helper was called can pass while the user-visible result is wrong. Prefer assertions about outputs, state transitions, authorization decisions, emitted events, and error contracts that remain valid if the implementation is refactored.

Question mocks that bypass the risk

Ask why each new mock, stub, fake service, or snapshot is necessary. A mock that returns the expected value unconditionally can make a broken integration appear healthy. Keep unit isolation where it is useful, but add integration coverage for parsing, persistence, authentication, authorization, external-service contracts, and other boundaries where the defect would occur.

Add independent negative and boundary cases

For meaningful risks, write or request cases the generating agent did not use to produce the implementation. Include invalid types and formats, missing fields, empty collections, maximum and minimum values, duplicate requests, timeouts, partial failures, expired credentials, unauthorized identities, and malformed or hostile input as appropriate to the feature. The objective is not to maximize test count; it is to challenge assumptions that a happy-path generator is likely to share with its own tests.

Use a separate reviewer or generation path when practical

Tests written by the same agent as the implementation are not independent evidence. Independence can come from a human-authored test, a separate reviewer who derives cases from the requirement, a different tool or model, or a higher-level integration test. None of these removes the need for human judgment, but each reduces the chance that the implementation and its tests share the same blind spot.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Classify risk and scale review depth

Use a small set of change classes and document the decision in the PR. The categories below are a starting framework; map them to your existing risk and vulnerability policies rather than treating them as a mandated industry scale.

Change characteristics Minimum review posture Typical additional evidence
Routine, reversible behavior with no sensitive data or privilege change Baseline build, tests, analysis, and one accountable human approval Normal unit and integration coverage; clear requirement-to-test mapping
Shared libraries, data migrations, dependency updates, production configuration, or changes with a broad blast radius Elevated reviewer expertise and a second review where local policy requires it Migration rollback or recovery plan, compatibility checks, dependency rationale, and staging evidence
Authentication, authorization, cryptography, input parsing, deserialization, file uploads, or security configuration Code-owner routing to qualified security or subsystem reviewers; stronger approval requirements Abuse cases, negative authorization tests, boundary and malformed-input tests, and threat-oriented review
CI/CD, infrastructure, deployment controls, new trust boundaries, or changes handling highly sensitive data Elevated or two-person approval according to local risk policy; do not bypass required owners Least-privilege analysis, rollback plan, environment separation checks, and evidence from a controlled test environment

Compare risk using five axes: security impact and data sensitivity; changes to trust boundaries or privilege; production blast radius and reversibility; dependency or infrastructure effects; and confidence that tests independently cover both expected and failure behavior. Increase reviewer expertise and test depth as any of these risks rises.

Route sensitive files with ownership rules

Repository permissions should make the safe path the easy path. Define owners for security, infrastructure, deployment, and other high-consequence areas, then require those owners to approve matching files. A CODEOWNERS-style file can express the intent:

# Security-sensitive application paths
/security/ @security-reviewers
/src/auth/ @identity-reviewers
/src/crypto/ @security-reviewers

# Delivery and infrastructure
/.github/workflows/ @platform-reviewers
/infrastructure/ @platform-reviewers

Keep the ownership list current, ensure listed teams have qualified members, and configure branch protection or the equivalent repository control so an owner approval is required rather than merely suggested. Reviewers should be able to reject a change that is technically green but violates a security invariant or operational constraint.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Make CI gates explicit

Block on required failures

Required checks should include the build or compile job, relevant unit and integration tests, static analysis, dependency checks, and secret scanning. A critical security finding should block the merge. Define “critical” in the organization’s existing vulnerability policy; OWASP AISVS describes critical as CVSS 9.0 or higher, or an organization’s equivalent threshold. The threshold is a policy choice, not a universal requirement for every repository.

Separate advisory signals from merge blockers

Not every warning deserves an immediate block, but every exception needs an owner, rationale, expiration or remediation date, and authorization. Do not let a developer dismiss a finding simply because an AI tool produced the code. Preserve the original result and the human decision in the change record.

Protect the workflow itself

Review changes to CI definitions, build scripts, deployment manifests, and security tooling as code with elevated ownership. An agent can weaken a gate by editing the workflow that runs it. Require workflow changes to pass the same protected-branch rules as application code and verify that the resulting jobs still execute the intended checks.

Human reviewer checklist

Reviewers can use this checklist as a comment template or required form:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Requirement: What user, business, or operational requirement does this change satisfy? Is the behavior demonstrated by tests that would fail for a meaningful regression?
  • Scope: Does the diff include unrelated refactoring, generated files, configuration, or dependency changes that need separate review?
  • Architecture: Does the implementation fit established interfaces, error handling, logging, data ownership, and performance expectations?
  • Security: Are authentication, authorization, input validation, output encoding, secrets, and trust-boundary assumptions correct?
  • Dependencies: Are new packages real, maintained enough for the project, correctly licensed under local policy, and used for a justified purpose?
  • Tests: Were tests removed or weakened? Do mocks hide the behavior under test? Are invalid, boundary, and failure cases covered independently?
  • Operations: Can the change be observed, rolled back, migrated, and supported in production? Does it alter CI/CD or infrastructure controls?
  • Understanding: Can the responsible developer explain the code and its trade-offs without relying on an agent to answer basic questions?

Handle exceptions without normalizing bypasses

Failures and urgent fixes sometimes require an exception, but the exception process should be more demanding than an ordinary merge. Require an authorized human to document which check is being bypassed, why the risk is accepted, what compensating control exists, who approved the decision, and when remediation will occur. For a security finding, include the affected component, severity under the organization’s policy, exposure window, and rollback or containment plan.

Do not create a blanket “AI-generated” exemption. If a generated change is urgent, classify the urgency and impact, then use the same emergency-change process applied to human-written code. Re-run skipped checks and obtain the normal ownership approvals as soon as the incident permits.

Implement the policy in stages

Stage 1: Make ownership visible

Add the responsible-owner field, an AI-assistance label if useful, and a concise PR template that asks for scope, risk, tests, and known limitations. Train reviewers that disclosure is for routing and accountability, not a reason to assume the code is defective.

Stage 2: Enforce the baseline pipeline

Mark build, test, analysis, dependency, and secret checks as required for protected branches. Identify repositories that lack one of these checks and either add it or document the compensating control.

Free tools Windows power users keep installed

Stage 3: Protect high-risk paths

Create and test ownership rules for authentication, authorization, cryptography, parsing, uploads, security configuration, CI/CD, infrastructure, and new trust boundaries. Verify that a change touching one of these paths cannot merge without the qualified approval.

Stage 4: Audit test quality and exceptions

Sample merged AI-assisted PRs for deleted tests, assertion weakening, excessive mocks, and undocumented exceptions. Use findings to improve prompts, templates, reviewer training, and automated detection. Do not turn the sample into an unsupported universal defect rate or coverage target.

Keep the standard current

OWASP AISVS is a living repository artifact, so verify its current version and wording before copying a requirement into a formal policy. GitHub’s documentation describes suggested workflow practices rather than a universal standard. NIST’s Secure Software Development Practices for Generative AI and Dual-Use Foundation Models: An SSDF Community Profile was published on July 26, 2024; it supplies broader secure-development context, not a prescriptive pull-request checklist. Assign an owner to review these references and your internal vulnerability policy periodically, especially when repository tooling or AI capabilities change.

Policy language you can adapt

The following compact rule can be placed in an engineering standard and expanded with local tool names:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Every AI-generated or AI-modified pull request must name a responsible human developer, pass required build, test, static-analysis, dependency, and secret checks, and receive approval from a qualified human reviewer. The generating agent cannot satisfy the approval requirement. Changes to test files require review for deleted or weakened coverage, implementation-shaped assertions, and unjustified mocks; meaningful risks require independent negative and boundary tests. Changes affecting security-sensitive paths, dependencies, infrastructure, CI/CD, privileges, trust boundaries, or high-impact production systems must follow code-owner routing and any elevated approval rules defined by the organization. Critical findings block merge unless an authorized human records an exception under the vulnerability policy.

Frequently Asked Questions

Should every AI-assisted pull request require two human reviewers?

Not necessarily. The available guidance does not set a universal reviewer count. Require at least one accountable human approval for every change, then use risk, blast radius, privilege, trust-boundary, dependency, and infrastructure factors to decide when a second qualified reviewer is necessary.

Is a high test-coverage percentage an adequate standard for AI-generated code?

No single coverage percentage is established by the cited guidance, and coverage can rise while assertions remain weak. Require meaningful behavior, failure, boundary, and security tests, and define any numeric target locally through your existing engineering policy.

Can an AI code-review bot approve a pull request?

A bot can provide useful findings and run checks, but it cannot replace the human approval or accountability requirement. The responsible developer and qualified reviewer must understand the change and its risks.

When should the OWASP AISVS wording be copied into an internal policy?

Verify the current AISVS version and wording first because it is a living repository artifact. Treat it as a source for controls and thresholds to adapt, not as proof that every organization must use identical rules.

The Bottom Line

Set one enforceable baseline for every AI-assisted PR—human ownership, normal build and security gates, and a meaningful approval—then escalate test independence and reviewer expertise as privilege, trust-boundary, dependency, infrastructure, data sensitivity, or blast radius increases. The merge decision belongs to people who can explain and support the code.
