Declare a security incident immediately, start a UTC timeline, and constrain the agent before investigating further. Revoke the agent’s identity, sessions, tokens, OAuth grants and cloud trust relationships; rotate every secret that touched the run; block unnecessary tool access and network egress; and preserve prompts, responses, tool calls, approvals, logs and affected data locations in tamper-evident storage. Then determine both what the agent could reach and what it actually read, changed or transmitted. Recovery should be staged, rebuilt from trusted artifacts, tested with restricted autonomy and approved by the system owner before normal access returns.
First response: declare, contain and preserve
Treat an AI agent’s access to sensitive production data as a security incident even when you do not yet know whether data left the environment. The agent’s identity, delegated permissions, prompts, memory, tools and generated artifacts are part of the incident boundary. A final answer from the model is not a reliable record of everything that happened.
- Declare the incident and assign owners. Name an incident commander, security lead, system owner, legal or privacy contact and communications lead. Record UTC timestamps, the original alert and every major decision in a protected incident record.
- Stop further access with the least disruptive effective control. Disable the agent or place it in a deny-by-default mode, revoke active sessions and tokens, remove tool permissions, and block external egress where feasible. Keep an approved fallback for essential business functions instead of making an unplanned change that causes a second outage.
- Rotate and quarantine. Rotate every credential exposed to the workflow and isolate code, packages, infrastructure changes, memory or vector stores, queues, scheduled jobs and other artifacts the agent created or modified.
- Preserve evidence before cleanup. Export the complete action chain and protect copies against alteration. Do not delete prompts, logs or generated files simply because they look suspicious.
OWASP’s current Agentic Incident Response guidance classifies active production exploitation or confirmed data exfiltration as critical and uses a one-hour response target for that playbook. That is an illustrative target, not a universal legal or operational deadline; set an organization-specific service level and escalation path.
How to contain the agent without causing more production damage
Containment must stop additional exposure while preserving availability for critical work. A blanket shutdown can interrupt safety processes, customer transactions or dependent automation, while leaving the agent connected can expand the incident. Use a preapproved decision tree for each AI component, documenting the business cost, cascading effects, containment options and approval authority. AWS guidance (2026) recommends “pre-built decision trees per AI component anchored to business functions” and testing them with ML engineers and business owners.

Choose the narrowest control that stops the risk
- Disable the agent runtime when it is actively making unauthorized calls or the scope cannot be bounded quickly.
- Revoke tool permissions and leave the model available only for non-sensitive, read-only analysis when that safely preserves an essential function.
- Block egress at the agent’s network boundary when external transmission is suspected; preserve internal logs and approved service dependencies.
- Freeze writes to production systems, repositories, deployment pipelines, ticketing systems and data stores while allowing a separately approved read-only fallback.
- Isolate memory and vector stores from new writes and downstream retrieval until their contents and access logs are reviewed. Isolation can affect other workflows, so identify dependencies before cutting connectivity.
- Use a documented fallback operated by a human or a constrained service account for essential transactions. Record who approved it and which controls differ from normal operation.
Do not rely on a prompt telling the model to stop. Enforcement must occur at identity, authorization, network and tool layers that the agent cannot override.
Revoke every path to the environment
Disable the agent’s service account and invalidate short-lived sessions. Revoke API keys, OAuth grants, workload identities, role assumptions, repository tokens, signing credentials, database passwords, cloud access keys and any trust relationship that allowed the workflow to reach another account or tenant. Check both the agent platform and the underlying cloud, SaaS and on-premises systems; revoking a model-provider token alone may leave a valid cloud role active.
Rotate credentials and quarantine what the run touched
Rotate secrets in an order that prevents the agent from using an old credential while you are still investigating. Start with credentials that can reach the most sensitive systems or create persistence, then work through every secret presented to tools, prompts, environment variables, memory, build jobs or downstream services. - Cloud keys, role-assumption credentials and workload-identity bindings.
- Model-provider and external API keys.
- Source-control, package-registry and CI/CD tokens.
- Database users, replication credentials and administrative passwords.
- Code-signing, artifact-signing and deployment credentials.
- Service-to-service certificates, webhook secrets and encryption-key access grants.
After rotation, verify that old credentials fail and that legitimate services received the new values through an approved secret-management path. Record the time, owner and validation result for each change.
Quarantine generated and modified artifacts
Preserve a forensic copy, then prevent execution or distribution of anything produced or changed during the suspect window. Quarantine generated source code, packages, container images, infrastructure-as-code, deployment manifests, database migrations, scripts, notebooks, prompts, system instructions, memory records, vector indexes, scheduled jobs, queues and configuration changes. Mark each item with its origin, hash, timestamp and storage location. Do not “clean up” an artifact before its original state is captured.
Inspect for persistence: new accounts, SSH keys, OAuth applications, scheduled tasks, serverless functions, modified startup scripts, altered policies, webhooks, hidden workflow steps and changes to retention or logging settings. Notify downstream consumers if a compromised package, image, dataset or generated configuration was already shared.

Preserve evidence that can reconstruct the full action chain
Evidence should show what the agent was told, what it could do, what it attempted and what actually happened. Preserve originals in tamper-evident storage, hash exported files or use equivalent integrity protection, restrict access, and maintain a chain-of-custody record showing who collected, copied, viewed or transferred each item.
| Evidence category | Capture | Why it matters |
|---|---|---|
| Prompts and model context | System prompts, user prompts, retrieved documents, memory entries, tool instructions and policy messages, with model and prompt versions | Shows the instructions and hidden context that shaped decisions and may reveal injected or unauthorized content |
| Responses and tool calls | Every model response, function call, arguments, return value, approval and denial, including failed calls | Final output can omit attempted reads, writes, privilege changes or exfiltration paths |
| Identity and authorization | Agent identity, service account, token and OAuth-grant metadata, role bindings, policy evaluations and session times | Defines the agent’s capability boundary and identifies credentials that must be revoked |
| Data-access records | Database, object-store, file, API and tenant-level reads, writes, exports, deletes and queries | Allows actual access to be separated from theoretical reach |
| Network and egress telemetry | DNS, proxy, firewall, load-balancer, endpoint and flow records, destinations, volumes and timestamps | Helps determine whether sensitive content or credentials left the environment |
| Artifacts and provenance | Hashes, repository history, package metadata, image digests, deployment records, memory/vector-store snapshots and job definitions | Reveals unauthorized changes, persistence and downstream exposure |
| Human and platform events | Approvals, operator actions, model/provider version, configuration changes, alerts and incident decisions | Establishes sequence, accountability and whether a control was bypassed |
OWASP’s AI Agent Security guidance warns that sensitive data can be exposed in agent context or logs. Scope logging so it captures the action chain without creating a new uncontrolled repository of secrets; apply access controls, retention rules and redaction that preserve forensic value. Redaction should be reversible only under authorized investigative control, and the unredacted source should remain protected.
Determine what the agent could access versus what it did
Start with capability, then prove activity. A role or network route shows potential access; it does not prove that records were read or exported. Conversely, absence of a successful final response does not prove that no sensitive data was exposed.

Build the capability map
- List every identity used by the agent, including delegated and assumed roles.
- Enumerate tools, plugins, APIs, databases, buckets, files, queues, repositories, tenants and administrative consoles reachable during the run.
- Record read, write, delete, export, privilege-management and network permissions, plus any policy exceptions.
- Map trust relationships and downstream services that would accept the agent’s credentials or artifacts.
- Identify data classes in each reachable location: credentials, personal data, health or financial information, intellectual property, regulated records and customer or employee data.
Verify actual activity
- Correlate tool-call timestamps with database, object-store, file, API and identity logs.
- Check query results, object downloads, export jobs, archive creation, screenshots, clipboard or browser automation and bulk-read patterns.
- Review writes, deletes, policy changes, new accounts, role grants, altered retention settings and deployment activity.
- Inspect network destinations, DNS lookups, upload sizes, encrypted tunnels and unusual service-to-service traffic.
- Search memory stores, vector indexes, caches, temporary files, model context windows and generated artifacts for copied sensitive content.
- Trace packages, images, configurations, reports or datasets delivered to other teams, tenants, vendors or customers.
Document uncertainty explicitly. A useful scope record distinguishes reachable, confirmed read, confirmed changed, confirmed transmitted and not yet determinable. Do not label data “safe” merely because exfiltration has not been proven; preserve the evidence and state the confidence level.
Assess notification and communications duties
Bring legal, privacy, compliance, communications, the system owner and relevant contract owners into the incident early. Notification decisions depend on the data involved, affected people and organizations, jurisdictions, contractual terms, regulator rules and the quality of available evidence. Access is not automatically exfiltration, but lack of proof is not proof of no exposure.
The Tool Desk
Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →- Classify the affected records and identify the data subjects, customers, employees, tenants or partners involved.
- Map where the systems and people are located and which privacy or sector rules apply.
- Review customer, supplier, cloud and processor contracts for notice deadlines, evidence-sharing requirements and approved wording.
- Ask counsel whether regulators, law enforcement, insurers, affected individuals or business partners must be notified.
- Coordinate a single factual timeline and avoid speculative claims about the volume or destination of data.
- Preserve privilege and restrict sensitive investigative details to the people who need them.
Communications should say what is known, what remains under investigation, what protective steps are being taken and where affected parties can obtain updates. Do not promise that no data was exposed until the scope analysis supports that conclusion.

Eradicate the access path and recover in stages
Recovery begins only after containment and evidence preservation are sufficient to make a safe decision. Follow NIST SP 800-61 Revision 3’s lifecycle of preparation, response, recovery and improvement, aligned with the Cybersecurity Framework 2.0.
- Remove unauthorized changes. Delete persistence, disable rogue accounts and applications, revert policy and configuration changes, and invalidate compromised sessions.
- Rebuild from trusted provenance. Roll back or rebuild code, images, infrastructure and data-processing jobs from verified repositories and known-good artifacts. Do not restore a convenient snapshot whose provenance is uncertain.
- Close the exploited path. Correct excessive permissions, unsafe tool wrappers, prompt-injection routes, unvalidated content ingestion, unrestricted network access and missing approval gates.
- Rotate again where necessary. If credentials may have been present in logs, memory, artifacts or backups, include those locations in a second rotation and verify revocation.
- Validate controls. Test least-privilege policies, egress filtering, monitoring, alerting, logging integrity, data-loss controls and rollback procedures using benign test data.
- Run a constrained fallback. Restore a read-only or human-approved mode with limited tools, narrow data scope, short-lived credentials and explicit rate and volume limits.
- Expand gradually. Add one permission or tool at a time, monitor outcomes, and stop if behavior deviates from the approved test plan.
When is it safe to turn the agent back on?
Do not restore broad autonomy because the agent appears quiet or because a deadline has arrived. The business owner and security lead should approve a written restoration gate covering all of the following:
- All agent identities, sessions, tokens, OAuth grants and trust relationships were revoked or reissued, and old credentials were tested as invalid.
- Generated code, packages, infrastructure, prompts, memory stores, vector indexes and jobs were reviewed, rebuilt or explicitly approved from trusted provenance.
- Reachable data and tools are limited to the minimum required function; sensitive production data is excluded unless a documented owner accepts the risk.
- Outbound network paths, destinations, upload volume and content controls are enforced and monitored.
- Prompts, responses, tool calls, approvals, policy decisions, identity events and data-access logs are complete, protected and retained for the required period.
- Human approval is required for writes, exports, privilege changes, deployments and other irreversible actions.
- A tested fallback can keep essential business functions running if the agent is disabled again.
- Alert thresholds, on-call ownership, rollback steps and stop conditions are written and exercised.
Start with a canary workload or synthetic data, then move to a small production slice. Require explicit approval before each increase in autonomy, data scope or tool access.
Common mistakes and safer alternatives
| Mistake | Why it fails | Safer alternative |
|---|---|---|
| Deleting the agent session, logs or memory immediately | Destroys evidence and can hide the actual scope | Snapshot and hash evidence first, then quarantine access |
| Revoking only the model-provider key | Cloud roles, database users or OAuth grants may remain valid | Inventory and revoke every identity, token and trust relationship |
| Assuming capability equals exfiltration | Overstates impact and can trigger inaccurate notifications | Separate reachable resources from verified reads and transmissions |
| Assuming no final answer means no exposure | Tool calls and failed responses can still reveal or export data | Review the complete call, result, network and storage chain |
| Using an untested emergency shutdown | Can interrupt critical operations or create cascading outages | Use a business-aware decision tree and approved fallback |
| Restoring the old permissions after cleanup | Reintroduces the original path and may preserve hidden persistence | Rebuild least privilege, validate controls and expand autonomy gradually |
| Logging everything without protection | Agent context and logs may become a second sensitive-data store | Scope collection, protect access, apply retention and controlled redaction |
Turn the incident into durable controls
After recovery, run a blameless review with security, platform engineering, ML or AI owners, legal and privacy, operations and the affected business team. Focus on system conditions rather than individual fault.
- Document the original access path, decisions, timing, impact and unresolved uncertainty.
- Assign owners and deadlines for least-privilege changes, credential-rotation automation, logging retention, egress policy and artifact provenance.
- Define which actions always require human approval and which can run autonomously under volume, destination and data-class limits.
- Keep an inventory of agents, identities, tools, models, prompts, memory stores, data locations and downstream consumers.
- Create severity criteria and organization-specific response-time objectives; treat OWASP’s one-hour critical target as a reference point, not a universal rule.
- Tabletop the scenario with the same people who would authorize containment and business fallback. Test disabled credentials, evidence export, notification decisions, rollback and staged restoration.
For future agent deployments, evaluate identity isolation, least-privilege granularity, token lifetime and revocation speed, completeness and tamper resistance of prompt and audit logs, sensitive-data redaction, egress controls, rollback and provenance, SIEM or SOAR integration, continuity options and tabletop support. These controls determine how quickly an incident can be contained without sacrificing essential service.
