You can generally request, review, and ask to amend the medical record your U.S. health care provider or health plan maintains, even when AI helped prepare the visit note. Under the federal HIPAA baseline, there is no separate AI-specific request process in the reviewed HHS guidance: start with the provider or plan that holds your record, then request a correction if you find information that is inaccurate or incomplete.
How do I get a copy of my medical records?
Ask the provider or health plan that maintains the record how to submit an access request. Depending on the organization, that may mean using a patient portal, sending a written request, or contacting its medical-records department. You generally do not have to explain why you want access.
HIPAA’s access right generally covers protected health information in a covered entity’s designated record set, whether it is electronic or on paper. That can include medical and billing records, claims and payment information, and clinical notes. HHS notes that a portal may let you view and download information; in most cases, you may ask for a preferred format, such as email, if the provider or plan can provide it that way. HHS: Your Medical Records and HHS OCR: Individuals’ Right under HIPAA to Access their Health Information explain the federal baseline.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesThe request is for records the covered provider or plan maintains. The available HHS guidance does not establish that an AI software vendor necessarily keeps a separate patient-facing record you can request directly. Some limited exceptions also apply. For example, the general access right described in HHS’s patient guidance does not cover a provider’s psychotherapy notes, which are kept separate from the medical and billing record.

A provider cannot refuse to give you a copy solely because you have not paid your medical bill. A reasonable fee may apply for copying and mailing, but not for searching for or retrieving your records. HHS also says electronic records may not be charged on a per-page basis. See HHS’s patient guide to medical records for details.
How long does an access request take?
Under the federal HIPAA access baseline, a covered entity must act on your request no later than 30 calendar days after receiving it. If it cannot meet that deadline, it may take up to 30 additional calendar days, but it must send you written notice within the first 30 days explaining the delay and giving the expected completion date. HHS describes 30 days as an outer limit and encourages faster responses when possible. HHS OCR’s access guidance provides the timing rules.

Can I see the notes from my AI-assisted appointment?
Request the final visit record maintained by your provider or plan, including the clinical note if it is part of the designated record set. Do not assume that a draft generated during the visit, a raw recording, an AI transcript, or an AI vendor’s internal data is automatically part of the accessible record. HHS’s general access and correction guidance does not settle the status of every artifact used in an AI documentation workflow.
What should I check when I review the record?
Compare the note with your understanding of what happened at the appointment. Not every record contains every item below, but these are useful areas to check for errors or omissions that could affect care or administration:

Focus on specific information that appears wrong or is missing. HHS says patients may request amendment of information they believe is inaccurate or incomplete; whether the request is accepted is decided through the provider’s or plan’s amendment process.
How do I fix a mistake in my medical record?
- Contact the organization that maintains the record. Ask the provider or health plan how to submit an amendment request; use its designated process if it has one.
- Identify the disputed information. Specify the note, date, or entry and describe what is inaccurate or incomplete.
- State the correction you believe is right. Explain what should be corrected or added, keeping the request focused on the relevant facts.
- Keep a copy. Save your request and the organization’s response so you can refer to them if you need to follow up.
HIPAA gives individuals a right to request amendment; it does not mean a patient can directly overwrite or erase a signed clinical note. The provider or plan handles the amendment. If the request is accepted, the covered entity must make reasonable efforts to provide the amendment to people you identify as needing it and to others it knows might rely on the information to your detriment. HHS OCR says electronic correction processes may use linked documentation and can be handled electronically when the parties agree. See HHS OCR’s access and amendment guidance and HHS’s health IT guidance on HIPAA rights.

How long does an amendment request take?
Amendment requests have a different general response period from access requests. HHS OCR’s health IT guidance says a covered entity has up to 60 days to act on an amendment request, with an additional 30-day extension in certain circumstances. Check any written notice for the reason for an extension and the expected completion date. HHS’s health IT guidance describes this amendment timeline.

What if my doctor won’t correct my record?
If the provider or plan denies the request, it must give you a written denial and an opportunity to submit a statement of disagreement for inclusion in the record. The disputed information and your statement must be linked, and the disagreement must accompany later disclosures of that information. Keep the denial and your statement. You are asking for your disagreement to be attached to the record; a denial does not mean the original entry is erased. HHS explains this process in Your Medical Records and its health IT guidance.

Does the same process apply everywhere?
This article describes the U.S. federal HIPAA baseline, not individualized legal advice or a state-by-state or international survey. State law may add details or provide different rights. If you need a jurisdiction-specific deadline or procedure, check the current rules where you live and ask the provider or plan which process applies.
