The most reliable way to reduce ketamine-related risk is to stop using it or reduce use with help from a health professional. Frequent, high-dose use can lead to tolerance, escalating use, dependence and withdrawal, as well as potentially lasting injury to the bladder and other organs. If use continues, risk-reduction steps can lower some dangers, but no recreational dose or schedule is known to be safe.

This UK-grounded guide reflects the Advisory Council on the Misuse of Drugs (ACMD) review published 28 January 2026. Prescribed clinical ketamine use is different from non-medical high-dose use: the ACMD says chronic therapeutic use at recommended doses is unlikely to cause significant addiction, while recognising addiction potential, especially among people regularly taking high doses. Read the ACMD review.

Can ketamine cause dependence?

Yes. Repeated high-dose use can produce tolerance, meaning a person may feel they need more to get the same effect. Use can then escalate, and stopping after heavy use may bring withdrawal symptoms. Dependence is not inevitable, and prescribed treatment at recommended doses should not be equated with non-medical high-dose use; however, neither a recreational dose nor a particular pattern of use can be guaranteed safe.

The ACMD describes a 5–8% estimate of more problematic use involving high doses taken daily among responses to its 2025 call for evidence. That is not a population prevalence estimate. A separate survey reported that 218 of 1,285 ketamine users (17%) met DSM-IV dependence criteria; dependence was strongly associated with the amount used per session and how often sessions occurred. This was a survey of ketamine users, not the general population. The ACMD review and evidence annex also cite a Crime Survey for England and Wales estimate that 299,000 people aged 16–59 (0.9%) reported ketamine use in the previous year in 2022–23; that figure applies to England and Wales, not other countries.

What reduces risk most?

Reduce or stop use

Reducing use or stopping is the safer direction, particularly if frequency or dose is increasing, stopping feels difficult, or physical symptoms have started. A GP, local drug and alcohol service, or specialist clinician can help plan a change and assess related health needs. Seek advice rather than relying on a self-directed taper or trying to manage withdrawal with other drugs.

If use continues, reduce immediate hazards

The ACMD advises people who continue to use ketamine to avoid using alone, avoid risky settings, and avoid combining it with alcohol, opioids or benzodiazepines. These depressants can increase adverse effects and overdose risk. Avoid situations where intoxication could expose someone to falls, drowning, violence or sexual exploitation. Do not drive or undertake other activities where impairment could cause an accident.

The report recommends using small test doses and pre-measured amounts, spacing doses and sessions, and leaving longer intervals between episodes—ideally at least four weeks. This is a harm-reduction recommendation, not evidence that a four-week interval or any dose is safe. Ketamine may be of uncertain purity or adulterated, so a familiar source or past experience cannot guarantee what a dose contains. The ACMD notes that evidence for ketamine-specific harm-reduction interventions is limited. See the ACMD’s risk-reduction recommendations.

How can people who inject or share paraphernalia reduce infection risk?

Injection carries additional risks. The ACMD advises people who inject to access sterile equipment and advice through a needle and syringe programme, use new sterile equipment each time, and never share injecting equipment. Shared paraphernalia—including straws used for snorting—can also transmit infections. Discuss relevant exposure with a healthcare professional and ask about blood-borne virus testing. These steps reduce some infection risks; they do not make ketamine use safe.

  • 5-pack convenience: Includes 5 individually wrapped multi-drug cup tests for occasional personal screening. A compact amount takes little storage space; sealed pouches protect contents until needed. At least 11 months of shelf life is available upon receipt, leaving time to plan a future check.

What are the signs of ketamine bladder damage?

Frequent, high-dose use is associated with injury to the bladder and upper urinary tract, as well as kidney, abdominal, liver and bile-duct problems. Urinary symptoms should not be dismissed or treated as an expected temporary effect: seek prompt medical assessment, especially if symptoms persist or worsen.

Persistent abdominal pain, sometimes with vomiting, is often called “K-cramps” in relation to ketamine use. The ACMD review says it affects more than a quarter of regular users; it also reports cholestatic liver injury and chronic bile-duct dilatation in about 10% of regular users. These are review-reported estimates for regular users, not risks that can be applied to every person or every pattern of use. Some long-term effects may not resolve, or may only partly resolve, after stopping. The ACMD review discusses these harms.

If ketamine-related urinary or abdominal symptoms are present, tell the clinician honestly about ketamine use and ask for assessment. For ketamine-induced uropathy, discontinuing ketamine is the most important treatment component identified by the ACMD; medical or surgical care may also be needed. Care may need to address both urinary injury and the ketamine use disorder. Do not try to treat ketamine-related pain with opioids or more ketamine without medical direction.

Where can someone get help for ketamine use?

Start with a GP or local drug and alcohol service and explain what is being used, how often, and any withdrawal, mental-health or physical symptoms. The appropriate route depends on where the person lives in the UK. A useful service should be accessible and able to provide or coordinate substance-use support, mental-health care and physical assessment—including urinary or urological care when indicated. If a first contact cannot meet all needs, ask for a referral or coordinated support rather than treating each problem in isolation.

  • CREDIBLE: This test is CLIA Waived for home or over the counter (OTC) use, utilizes the same technology as lab screening tests, and its results are accepted in most professional and clinical settings.
  • COMPREHENSIVE: This lab-grade test can detect 14 drugs simultaneously: Buprenorphine (BUP), Tetrahydrocannabinol (THC), Opiates (OPI - 2000 ng/mL cut-off level), Amphetamine (AMP), Barbiturates (BAR), Benzodiazepines (BZO), Cocaine (COC), Methamphetamine (MET), Methylenedioxymethamphetamine (MDMA), Methadone (MTD), Oxycodone (OXY), Phencyclidine (PCP), Propoxyphene (PPX), and Tricyclic Antidepressants (TCA).
  • ACCURATE: More than 99% accurate, it uses lab-recommended cutoff levels for each drug and includes a temperature strip to verify the temperature of the fresh sample for Specimen Validity Testing.
  • EASY TO USE: To use, simply remove the cap, collect urine in the test cup, wait a FULL 5 minutes, and then read the result; disposable plastic gloves are included, a negative result shows two lines, a positive result shows only the control line, a faint test line still means negative, and results should not be read after 5 minutes.

Treatment for ketamine use disorder centres on psychosocial support to help a person move toward abstinence or reduced use, alongside care for psychological, social and physical problems. The ACMD says there are no specific medicines established for ketamine dependence; medication approaches in the evidence base have not been evaluated in high-quality studies, and reported benefits are anecdotal. Do not self-medicate or treat these approaches as proven alternatives to professional care. Evidence about withdrawal management is also limited and low quality, so an individual plan should be made with a clinician or specialist service.

The ACMD recommends accessible community support and outreach, alongside education, professional training, drug-checking access and safer-use advice. Its Recommendation 12 says: “Integrated harm reduction approaches should be developed and delivered, combining education, professional training, access to drug checking and safer use practices.” ACMD report, published 28 January 2026.

Does UK legal status affect where to seek help?

The ACMD’s 2026 review recommended that ketamine remain a Class B controlled substance in the UK. Legal status and related policy can change; check current official legislation for the position where you live. Whatever the legal context, medical assessment for symptoms and support for reducing or stopping use are appropriate reasons to contact a health professional.
