For mild diarrhea or stomach upset while taking antibiotics, keep replacing lost fluids and contact your prescriber or pharmacist before using an antidiarrheal or changing your medicine. Get medical advice if diarrhea lasts more than two days, or if you have fever, significant abdominal pain, signs of dehydration, or black or bloody stool. These symptoms can need assessment, including for Clostridioides difficile (C. difficile), a cause of serious antibiotic-associated diarrhea.
Why antibiotics can cause diarrhea or stomach upset
Antibiotic-associated diarrhea is commonly described as three or more loose, watery stools a day after antibiotic use. Mild symptoms may begin within hours or days and often settle within a few days after treatment starts or ends. Mayo Clinic estimates that about one in five people who take antibiotics gets antibiotic-associated diarrhea; that is a general estimate, not a rate that applies to every antibiotic or population. Not every case is mild. Persistent or severe diarrhea can have causes that require medical assessment, including C. difficile. Symptoms alone cannot establish the cause.
What to do for mild symptoms
Replace fluids
Drink regularly to replace fluid loss. Water, broth, and fluids containing electrolytes are options. An oral rehydration solution, available without a prescription, contains water, electrolytes, and sugar to help replace losses; it supports hydration but does not treat the cause of diarrhea. Ask before taking an antidiarrheal
Do not assume an over-the-counter antidiarrheal is suitable. Ask a healthcare professional first: some medicines may be inappropriate when C. difficile or another bacterial infection is possible. A pharmacist or prescriber can advise based on your symptoms and medication.
Contact the prescriber before changing the antibiotic
Do not stop, switch, or alter a prescribed antibiotic on your own. Contact the clinician who prescribed it and describe the symptoms; whether the medicine should be changed depends on the situation. The American Gastroenterological Association’s patient resource on antibiotics advises contacting the healthcare provider before stopping treatment.

When to seek medical advice
Contact a healthcare professional if diarrhea lasts longer than two days, or if you develop abdominal pain, fever, signs of dehydration, or black, tarry, or bloody stool. Seek prompt advice for severe symptoms rather than trying to manage them with fluids or an antidiarrheal alone. C. difficile is one important possible cause of serious antibiotic-associated diarrhea and needs clinical assessment. Mayo Clinic lists warning signs and causes.

The evidence is mixed, so probiotics are optional rather than a reliable treatment. A Cochrane review, based on a literature search through March 3, 2025, found that probiotics may reduce antibiotic-associated diarrhea and C. difficile-associated diarrhea. It describes a possible small, short-term preventive benefit for people taking antibiotics who do not have weakened immune systems; more large studies are needed in low-risk groups. The review does not establish that every strain or product works, or that probiotics treat active severe diarrhea. Read the Cochrane review summary.

- Use antibiotics only when they are needed and prescribed.
- Wash your hands.
- Tell your clinician if you have previously had antibiotic-associated diarrhea or C. difficile, so that history can inform care.
