Canine cognitive dysfunction syndrome (CDS), often called dementia, can gradually change how a dog navigates familiar places, interacts with people, sleeps, toilets, and behaves. Those changes are not proof of dementia: pain, sensory loss, and other illnesses can look similar. Arrange a veterinary assessment for persistent or worsening changes, and seek prompt advice for sudden symptoms—especially balance problems.
What dementia symptoms can look like in dogs
Veterinarians often use the DISHAA pattern to organize observations: disorientation, changes in interactions, sleep-wake disruption, house-soiling, and altered activity or anxiety. Signs may appear singly or in combination and often develop gradually.
Disorientation
- Getting lost in a familiar room or route.
- Staring or becoming stuck in a corner.
Changes in interactions
- Becoming unusually clingy, withdrawn, avoidant, or irritable.
- Seeming not to recognize familiar people or animals.
Sleep-wake changes
- Wandering or restlessness at night.
- Sleeping more during the day.
House-soiling
Urinating or defecating indoors after being reliably house-trained can be a sign to discuss with a veterinarian, though it can also have other causes.

Activity and anxiety
- Showing less interest in play or walks.
- Pacing or repeating activities.
- Developing new anxiety, fears, irritability, or aggression.
CDS may begin around age nine or older, according to Cornell University College of Veterinary Medicine. Age is context, not a diagnosis; a younger dog with behavior changes still needs assessment when those changes are persistent or concerning.

When to contact a veterinarian
Book an evaluation if an older dog develops persistent or progressive behavior changes, disrupted sleep, house-soiling, anxiety, or altered interactions. Contact a veterinarian promptly about sudden or marked changes rather than assuming they are normal aging.
Sudden balance-related signs are different from the gradual pattern often associated with CDS. Cornell notes that sudden circling, inability to stand, a head tilt, nausea, or rapid repetitive eye movements can occur with vestibular syndrome. These signs need veterinary assessment; do not casually label them dementia.

How to prepare for the appointment
A dated log can help the veterinarian see the pattern and timing of changes. Record:
- What changed and when it began.
- How often each behavior occurs and when it happens.
- Changes in appetite, mobility, toileting, or sleep.
- Current medications and any recent changes.
DISHAA is a way to organize observations, not a home diagnostic test. A single behavior—or a checklist of behaviors—cannot establish that a dog has dementia.

What a veterinary assessment may involve
CDS is assessed from clinical signs after other possible causes have been considered. Cornell describes a thorough physical examination and possible blood and urine tests to investigate look-alikes such as pain, arthritis, seizures, systemic illness, and vision or hearing loss. In some cases, a veterinarian may use MRI to help rule out a brain tumor or another condition.
This evaluation matters because similar behavior changes can arise from problems that need different care. The veterinarian can interpret the changes alongside the dog’s overall health and decide whether further testing is appropriate.

Care and support if a dog has CDS
There is no single cure, and care is individualized. Cornell University College of Veterinary Medicine states: “There is no one treatment or cure for CDS, and research is ongoing in this area.” Management may address concurrent health problems while supporting safety, routine, and engagement.

Offer enrichment and activity
Mental and physical enrichment may be part of a care plan. Cornell identifies interactive toys as one option for brain stimulation; a puzzle toy is an activity, not a treatment that prevents or cures dementia.
Discuss nutrition and medication with the veterinarian
Nutrition and medication choices should take the individual dog’s health into account. Cornell identifies selegiline as a medication approved for CDS treatment in North America; ask a veterinarian whether it is appropriate. Discuss supplements as well rather than starting them on your own.
VCA’s guidance for therapeutic diets says behavioral improvements are typically seen within 6–12 weeks after starting a diet with sufficiently high antioxidant levels. VCA also says that if there is no improvement within 12 weeks, nutrition alone is unlikely to make a difference. This is guidance, not a guaranteed response, and trying a diet should not delay veterinary follow-up.

What the available estimates do—and do not—show
The Merck Veterinary Manual reports two estimates from different studies and designs:
| Estimate | What it describes | Important qualification |
|---|---|---|
| 28% of dogs aged 11–12 years and 68% of dogs aged 15–16 years showed signs consistent with CDS. | Age-group findings reported by Merck from a cited study. | The search result did not provide the study year. Signs consistent with CDS do not mean every dog had a clinical diagnosis. |
| 14.2% of dogs older than 10 years were estimated to have CDS; more than 85% of these estimated cases had not been diagnosed. | An estimate reported by Merck from an internet survey. | The search result did not provide the survey year. This estimate is not directly interchangeable with the age-group findings above. |
These figures describe different evidence, not one combined prevalence rate. They do not tell an owner whether an individual dog’s symptoms are caused by CDS.
