Prevent double-bookings by enforcing availability when the appointment is written to the database—not by trusting an earlier availability check. Two requests can see the same open time before either one books it. A database constraint is usually the strongest final guard; transactions, locks, or serializable isolation can protect more complex booking rules when implemented with explicit conflict and retry handling.
Why an availability check alone fails
A typical race looks like this:
- Request A checks 10:00 and sees it is available.
- Request B checks 10:00 and also sees it is available.
- Both requests try to create an appointment.
If the system only checks availability before inserting, both writes may succeed. The read is useful for displaying choices, but it does not reserve the time. Treat the committed database write as authoritative, and do not tell the customer a booking succeeded until its transaction commits.
Define exactly what must not happen
Choose the invariant before choosing a concurrency technique. For example: one appointment per provider and fixed slot; no overlapping active appointments for a provider; one booking per customer; or no more than a stated capacity at a time. Define which appointment states consume availability. A temporary hold and a confirmed appointment may both block time, while a cancelled appointment may release it.

The interval convention and the states covered by the rule must match the product’s actual scheduling policy.

Choose enforcement that matches the schedule
Fixed grid of slots
If every appointment uses a predefined slot and one database row represents that slot, enforce uniqueness on the resource and slot identifier—for example, (provider_id, slot_start). A unique constraint or index lets the database accept only one conflicting insert, even when requests reach different application instances. Include any other key that defines exclusivity in your model.
Variable-duration appointments
When appointments can have different start and end times, the rule is about overlapping intervals, not identical start times. PostgreSQL supports exclusion constraints that can prevent overlapping ranges for the same resource among specified active states. Range exclusion is database-specific, not a universal SQL feature. Check whether the database you deploy offers an equivalent constraint. Where it does not, a transactional protocol may be needed, but every writer must follow that protocol for it to protect the invariant.

Use transactions and locks for multi-step rules
A database constraint is preferable when it can express the invariant. A row lock can help when booking involves a known resource or inventory row that must be updated consistently. PostgreSQL’s documentation explains row-level locks, lock modes, and deadlocks in its explicit locking guide. Locks remain until the transaction ends, so keep the transaction short and never hold it open while a customer fills out a form.

Serializable isolation can protect correctness when the rule depends on a broader set of reads and writes. It does not mean every transaction will succeed: applications must handle serialization failures, deadlocks, and constraint conflicts as expected outcomes. See PostgreSQL’s application-level consistency guidance and MVCC and concurrency overview. Confirm details against the version actually deployed.
- Use consistent lock ordering when a transaction locks multiple rows, reducing deadlock risk.
- On a deadlock or serialization failure, roll back and retry the whole transaction where appropriate.
- Keep retries bounded and distinguish transient transaction failures from a genuine booking conflict.
- Retain a database constraint as a final integrity guard wherever possible; a locking protocol only protects writes that obey it.
Model holds, cancellations, and capacity
If a customer needs time to complete a form or payment, create a persisted hold with an expiration. A countdown shown in the interface is not a reservation unless the hold is represented in the data and counted by the same enforcement rule as a confirmed appointment. Define what happens when a hold expires, a payment fails, or a customer cancels. Cleanup and release operations should be safe to repeat.

For group appointments or shared resources, decide whether the resource is exclusive or has a bounded capacity. A simple uniqueness rule is not enough when several people may book the same session. Capacity updates must be protected from concurrent requests so that two transactions cannot both consume the final available place. The Universal Scheduling Protocol describes holds as temporary reservations and emphasizes matching booking rules to the capacity model; check its current specification before relying on details.
Respond safely to conflicts and retries
When a constraint rejects a conflicting write, return a clear result such as “That time was just taken,” refresh the displayed availability, and offer alternatives. Do not turn a failed insert into a success merely because an earlier read showed the slot as open.

Network timeouts create a separate ambiguity: the first request might have committed even though the client never received the response. Give each booking attempt a stable idempotency key or equivalent request identity, so replaying the same attempt cannot create a second appointment. Keep email, calendar, and payment side effects outside the critical reservation transaction where practical. A durable event or outbox and idempotent consumers can help ensure notifications follow a committed booking without duplicating it; no single architecture is mandatory for every system.
