Monitor an AI agent by recording each tool action and its outcome in a structured, correlated audit trail, then use those events to detect problems and support response. Enforce authorization in the systems that execute actions—not in the model—and require explicit approval for high-impact or irreversible operations.

What should an AI agent audit record capture?

A model-call log alone cannot show what an agent actually changed, accessed, or sent. Record the observable steps in each run, including tool invocations, authorization decisions, approvals, and results. Use a stable run or trace ID and timestamps so investigators can order events and connect actions across multi-step or multi-agent workflows. Canadian Centre for Cyber Security guidance calls for unified audit logs for inter-agent interactions and human-readable records of tool use and results (guidance on careful adoption of agentic AI).

For each action, capture:

  • Identity: the initiating human or service principal, the agent identity, and—where applicable—the identities of other participating agents.
  • Action and target: the tool or function invoked and the resource it acted on.
  • Context: timestamp, run or trace ID, and links to preceding and following steps.
  • Authorization: the policy decision and the approval state that applied to this specific action.
  • Input and outcome: parameters in a safe, redacted or hashed form where needed, plus the result or error.

Do not treat chain-of-thought as an audit record or a guaranteed account of internal computation. Observable actions, policy decisions, and outcomes are more useful for reconstructing what happened. Define record retention and access according to your investigation, governance, and data-protection needs. OWASP’s AI Agent Security Cheat Sheet and the Canadian guidance support action trails and protection against sensitive-data exposure.

How should authorization and approval work?

Make permission checks part of the execution path. The orchestration layer can record and correlate events, but the downstream system that performs a sensitive operation should independently verify that the identity has authority to perform that action on that target. OWASP’s 2025 Excessive Agency guidance recommends downstream authorization, least privilege, and logging and monitoring extension and downstream-system activity. An agent’s request, or its own statement that an action is permitted, is not authorization.

Decide how much oversight an action needs based on its possible consequences. This classification is an organizational design choice, not a universal legal threshold.

Action type Practical control
Read-only retrieval Log routine access and monitor for unexpected tools, targets, or rates.
Writes or outbound messages Scope the tool and target narrowly; require approval when the impact warrants it.
Code execution, financial operations, destructive changes, or privilege changes Use tighter scopes and explicit approval where appropriate; provide interruption or rollback mechanisms where feasible.
Unknown or unrecognized tools Handle conservatively: independently check scope, privilege, and approval state before execution.

For an approval to be meaningful, bind it to the exact action and target rather than a broad request that could authorize later, different operations. Prevent replay for irreversible actions. Define safe behavior for authorization-service or audit-pipeline failures; for sensitive actions, do not proceed when required authorization or audit recording is unavailable. OWASP advises: “Require explicit approval for high-impact or irreversible actions.” It also says: “Provide clear audit trails of agent decisions and actions.” These are statements from the OWASP AI Agent Security Cheat Sheet.

How do you protect audit evidence and sensitive data?

Keep audit records outside the agent’s ability to alter or delete them. Restrict who can view or export records, and avoid storing credentials or personal data in plain text. Redact or summarize sensitive parameters—or retain a suitable hash—while preserving enough context to investigate the action. These controls must balance investigative value with data-protection needs; indiscriminately logging every prompt and parameter can expose information without improving the action trail.

Which events should trigger monitoring or an alert?

Send agent activity into the operational monitoring and security processes used for other production systems. Possible alert conditions include:

  • Policy denials or attempted approval bypasses.
  • Use of an unexpected tool or access to an unexpected target.
  • Unusual action rates, repeated failures, or a sudden change in activity.
  • Failure of the audit or logging pipeline.

These are practical alert examples, not a universal prescribed list. Tune thresholds to the agent’s normal workload and the consequences of its actions so that responders can distinguish suspicious behavior from expected variation. The UK National Cyber Security Centre’s guidance on managing the cyber risk of agentic AI treats observability as part of security operations and incident response.

How should teams investigate and contain suspicious activity?

Make the audit trail useful in an incident, not just visible on a dashboard. Responders should be able to identify the agent’s owner and permissions, inspect the correlated trace, stop or restrict further actions, and preserve evidence. Connect those capabilities to your existing incident-response procedures, including the people responsible for the agent and the systems it can access.

NIST’s AI security control-overlay use cases include single-agent and multi-agent scenarios and describe selecting, adapting, or supplementing SP 800-53 controls for specific systems. Treat the overlays as a control-mapping resource, not a complete agent audit schema.

Can observability software provide an audit trail?

Observability platforms can help reconstruct runs, inspect tool calls, and organize production monitoring. They do not, by themselves, establish that an action was authorized: that check belongs in the application’s execution path and the downstream system. Compare tools against your actual needs for framework and tool integrations, trace depth, hosting and data boundaries, redaction, access control, retention, export, evaluation and alerting workflows, and expected usage cost.
