Before connecting an AI agent to real systems, define what it may do, log its actions and effects, and give an operator a reliable way to interrupt it. A chat transcript alone cannot show everything an agent did through tools or in its surrounding environment. Least privilege, independent checks for high-impact actions, and a system-specific recovery plan limit the damage monitoring alone cannot prevent.

Define what the agent is allowed to do

Set the agent’s permitted tools, data, targets, and actions before deployment. Scope its identity to the task, avoid broad standing permissions, and use credentials with the shortest practical lifetime. Where possible, keep credentials out of the model’s direct reach. Restrict connections to external systems rather than assuming the agent will use broad access safely.

Monitoring helps you notice unsafe behavior; it does not stop an over-privileged agent from acting. OWASP’s 2025 excessive-agency guidance and the UK National Cyber Security Centre (NCSC) both emphasize limiting an agent’s permissions and authority.

Match the approval gate to the action’s risk

Classify actions by their impact and reversibility, not just by the tool name. A read-only lookup may need a lighter gate than a write to a production record using the same system. Use a policy check or explicit human approval before destructive, financial, administrative, externally visible, or difficult-to-reverse actions. Unknown actions should be denied if their classification or authorization cannot be verified.

Action category Practical gate
Low-impact, reversible reads within the task’s permitted scope May run without individual review when the identity, resource, and purpose are authorized.
Writes or other changes that are bounded and readily reversible Apply a policy check; require review when the target, scope, or consequence makes the change consequential.
Destructive, financial, administrative, externally visible, or hard-to-reverse actions Require independent authorization or explicit approval before execution.

OWASP recommends previews and approval for high-impact or irreversible operations, plus independent validation by the component that executes them. Bind an approval to the exact actor, tool, target, parameters, time, and expiry; an approval for one action should not authorize a changed request. Its example in which only two mapped low-risk tools skip human review is illustrative, not a universal threshold.

Assume inputs from tools and other agents may be untrusted

Retrieved pages, emails, tool outputs, and inter-agent messages can contain misleading or malicious instructions. Validate those inputs, but enforce permissions in the system that performs the action—not by asking the model to decide whether its own request is allowed. In a multi-agent workflow, check authorization again at every trust boundary.

Instrument the full action path

Capture events while a run is active and retain them for investigation afterward. For each tool invocation, connect the request to the agent and human or service identity involved, the target resource, the action and parameters, the policy decision, any required approval, the result, and a timestamp. Preserve enough context to distinguish what the agent requested from what the tool actually changed.

What to record or observe Why it matters
Agent and human or service identity Shows which principal initiated or authorized the action.
Tool, target resource, requested action, and parameters Establishes what the agent attempted and where.
Authorization decision, policy rationale, and approval details Shows whether the action was permitted and under what conditions.
Tool result and resulting state change Distinguishes a request from a completed action and helps identify what must be recovered.
Timestamp and surrounding environment telemetry Helps correlate activity across services, sandboxes, proxies, and network traffic.

Microsoft recommends logging tool inputs and outputs, identity, and decision rationale. NCSC advises collecting telemetry from both the agent and its surrounding sandbox, including access logs, proxies, and network traffic. Protect records against modification or deletion; NCSC says immutable logs are preferable where possible.

Alert on actions and state changes

Build alerts around what the agent attempts and what changes, rather than relying only on unusual model text. Candidate conditions include:

  • A call to a tool or resource outside the task’s permitted scope.
  • A write or deletion when the task authorized reads only.
  • A burst of actions, repeated calls, or a loop that continues without useful progress.
  • An unexpected identity, destination, or target resource.
  • A denied or expired approval, or an attempt to change parameters after approval.
  • An action attempted after an operator has issued a stop signal.

These are practical alert examples, not a universal taxonomy prescribed by the cited guidance. Tune them to the deployment’s policies and expected behavior, and route high-severity alerts to someone able to intervene.

Assess observability for each tool and environment

Do not assume all agent capabilities can be monitored with the same logs. NIST’s August 5, 2025 article on tool-use lessons notes that some tools can use existing logs or transcripts, while other tool-enabled effects require new ways to observe them. It presents risk-classification dimensions rather than a comprehensive, settled taxonomy. Assess each deployment’s tool function, access pattern, action criticality, reversibility, reliability, modality, observability, and autonomy. For a robot or other physical system, software events alone may not reveal what changed in the environment.

Prepare an operator-controlled stop procedure

Decide before launch who can stop the agent and make sure that person or team can reach the controls throughout the agent’s operating hours. A stop signal must be enforced at the action path; a model instruction to stop is not an operational control.

NCSC cautions that shutdown may require more than stopping the agent process. Prepare a containment path that can, as appropriate to the deployment:

  • Stop the agent process or suspend the run.
  • Revoke or narrow the credentials the agent uses.
  • Disable the specific tool or integration involved.
  • Restrict network access or communications with model infrastructure.
  • Prevent queued or in-flight work from starting additional actions, where the connected system supports it.

Test who can activate each control and what it actually blocks. Include the agent in existing security operations monitoring and the organization’s incident plan for failures, misuse, and loss of control.

Investigate and recover from an unsafe action

Contain further access before attempting to restore data or configuration. Preserve relevant audit records and environment telemetry, then establish what the agent and its credentials touched. There is no universal rollback workflow: the correct recovery method depends on the connected system and the kind of change.

  1. Contain the agent and its access. Use the prepared stop procedure to suspend activity, revoke or narrow credentials, disable the affected tool, or restrict connectivity as needed.
  2. Preserve evidence. Protect relevant agent, tool, identity, approval, sandbox, proxy, network, and target-system records so the investigation can reconstruct both the request and its effects.
  3. Establish scope and impact. Identify the agent, human or service identity, and credentials involved. Trace tool calls to affected systems, resources, records, and any consequential downstream actions.
  4. Compare current state with a trusted source. Determine what changed by comparing observed state with a known-good source, such as an authoritative record, backup, or version history available for that system.
  5. Restore using the affected system’s established mechanism. Use that system’s supported recovery process where available. Avoid assuming that replaying or reversing a tool call will safely undo every side effect.
  6. Verify the result. Check restored data or configuration and the relevant permissions against the trusted state before considering the incident contained.

OWASP, Microsoft, and NCSC guidance supports controls such as interruption, protected logs, incident response, and rollback planning, but does not define one recovery procedure that fits every connected system. Tailor the plan to the systems the agent can affect.

Set a gate for resuming autonomy

Do not restart an agent solely because the visible error has been corrected. Resume only after the incident owner has determined that the cause is understood, the relevant systems and permissions have been checked, and the controls that failed have been corrected and tested. Confirm that the agent’s permitted scope and approval rules remain appropriate. If the available evidence cannot establish what happened or whether the environment is safe, keep the agent disabled and continue the investigation.

Assign accountability before deployment

Name the people or teams responsible for system ownership, approving access, monitoring activity, reviewing incidents, and stopping the agent. Make these roles explicit even when a vendor hosts part of the service. Microsoft’s shared-responsibility guidance varies across IaaS, PaaS, and SaaS, but lists human approval for high-impact actions as a customer responsibility in all three models. Treat that matrix as general guidance: check the actual service configuration and terms.

When choosing or assessing a monitoring approach, compare whether it covers the full action path, integrates with identity and authorization, supports real-time alerting, protects event retention and integrity, exposes sandbox and egress activity, supports approval and credential-revocation workflows, and fits the deployment’s recovery needs. Also consider the agent’s ability to affect state, including action impact and reversibility, not only its nominal permissions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Start with a bounded, low-risk pilot. Expand the agent’s scope or autonomy only after the monitoring, authorization, approval, stop, and recovery controls have been checked in the actual deployment context. NIST’s AI Agent Standards Initiative page, created February 17, 2026 and updated August 14, 2026, describes voluntary standards, agent identity, and security-evaluation activity; it is not a finished mandatory standard.
