Make your dog’s familiar routes safer and easier to follow: block stairs and other hazards, clear clutter, improve traction, and add nightlights where your dog walks after dark. Keep daily routines steady and make toileting easier. Because pain, infection, sensory loss, and other health problems can resemble dementia, arrange a veterinary assessment for new or worsening behavior changes or house soiling.
Start by blocking hazards and escape routes
A disoriented or unsteady dog may wander into danger or become stuck somewhere familiar. Walk through the home and yard at your dog’s level, looking for routes that lead to stairs, drops, tight spaces, or an unsecured exit. Use a suitable barrier, such as a gate, to restrict access to hazards; check that it stays in place and does not create a new obstacle.
- Stairs and drop-offs: Block access if your dog is disoriented or unsteady.
- Furniture and narrow spaces: Check behind sofas and in tight corners where your dog could wander in and struggle to get out.
- Yard and exits: Use barriers or fencing where needed to limit unsafe access and reduce escape risk.

Reduce slips and assess ramps
Merck recommends nonskid flooring on slippery steps and identifies ramps as a possible accommodation. Choose changes according to your dog’s mobility and comfort, and the layout of the particular route. A ramp is useful only if your dog can approach and use it safely; do not assume it is suitable for every dog or set of steps.

Light the route after dark
If your dog roams at night, place nightlights along the paths the dog actually uses. Cornell recommends them to improve visibility and reduce the chance of a dog getting stuck. Keep lights and cords positioned so they do not obstruct the route; Cornell does not compare specific light designs.
Make toileting easier without overlooking the cause
For house soiling, Cornell recommends seeing a veterinarian first to rule out a bladder infection. More frequent outdoor trips may help. If you use an indoor toileting option, put it somewhere convenient and make sure your dog can enter and leave it easily.

A diaper may be a management option in some situations, but it does not establish why accidents are happening. Cornell advises changing diapers frequently and cleaning the dog to avoid prolonged dampness and skin sores.
Keep the environment predictable
A consistent daily routine can help a dog with cognitive changes orient to the day. Keep familiar routes and everyday cues as steady as practical, including familiar sounds, smells, or tactile cues. Provide exercise and mental enrichment appropriate to your dog’s comfort and mobility rather than pushing through signs of distress or difficulty.

Pay attention to whether your dog seeks company or prefers quiet, and make a comfortable resting place available accordingly. Merck and Cornell describe routine, suitable activity, and environmental management as parts of care for dogs with cognitive dysfunction syndrome.

Dementia, also called cognitive dysfunction syndrome (CDS), can involve disorientation, altered interactions, changes in sleep and activity, anxiety, or house soiling. These signs can develop gradually, but they do not prove that dementia is the cause. Pain, arthritis, seizures, systemic illness, and loss of vision or hearing can produce similar changes.

Cornell notes that a veterinarian may recommend a physical examination and blood and urine testing to investigate other causes. Do not diagnose dementia at home, especially when behavior changes are new or worsening. Cornell also describes the DISHAA assessment guide as a way to track signs; tracking observations can support a veterinary discussion, but it does not replace an examination.
