Limited rural internet does not automatically rule out telehealth. Contact the clinic before your appointment: a phone call, secure message, exchange of files or images, or visit from a supported local site may work instead of video, depending on the health concern. If video is needed, test it from the place you plan to use, and agree in advance what to do if the connection fails.

First, ask whether this visit needs video

Explain your connection limits to the clinic and ask which format the clinician considers appropriate for your specific concern. Telehealth can include phone calls, secure messages, text, asynchronous sharing of files or images, and connections from a local clinic—not just live video. HHS notes that some services can be provided without broadband, but the clinician should determine whether a particular format is suitable for your care.

A phone visit may be simpler on a weak connection, but it is not appropriate for every health problem. Ask before switching formats rather than assuming audio alone will be enough.

Telehealth.HHS.gov’s rural telehealth guide says: “Even in communities without access to broadband, some health care services can be provided by phone, secure messaging, and asynchronous care.”

Compare the practical options

Option When it may fit What to check
Phone call The clinician agrees the specific visit can be handled by audio. Phone care may not work for every concern; confirm with the clinician. See HHS patient guidance.
Secure message or asynchronous exchange The issue may be addressed through messages, documents, images, or follow-up. Ask whether the clinician needs a real-time assessment instead. HHS describes these formats among rural telehealth options: internet and other telehealth resources.
Video from home You have a video-capable device and a connection that works with the clinic’s platform. Test from the actual location and arrange a fallback in case audio or video fails. See HHS preparation guidance.
Mobile hotspot Fixed internet is unreliable but a strong cellular signal is available where you will take the visit. A hotspot depends on usable cell coverage; check your signal, plan, and any data limits. HHS says, “You may consider using a mobile hotspot that can bounce off a strong cell signal, if available.” See HHS’s hotspot guidance.
Local clinic or community site Home connectivity is not available and a supported location is accessible. Confirm the site can support the appointment and provide a private place; availability varies locally. See HHS internet resources and patient guidance.

Choose based on clinical suitability, reliability at the actual location, privacy, accessibility, technical support, and any service or device costs. No option is universally best.

Prepare for a video appointment

  1. Confirm the format and platform. Ask the clinic whether video is needed, what app or website it uses, and whether you need an account. Use the clinic’s instructions rather than relying on a general speed target; official rural guidance does not set one minimum internet speed for every platform and visit type.
  2. Set up the device. Launch or install the clinic’s app as directed, sign in if necessary, and check that the camera and microphone work. If this is your first telehealth visit, ask the clinic whether it offers step-by-step help or troubleshooting support.
  3. Test from your planned location. Use the same device, connection, and room you expect to use for the appointment. A test in a different place may not reflect the connection you will have during the visit.
  4. Choose a private setting. If you use a library or another community location for internet, arrange a private space and check that the clinic’s platform and privacy requirements can be met. Free access to a connection does not itself make a conversation private.
  5. Agree on a failure plan. Before the visit, ask which number to call, whether the clinician will call you if the connection drops, or whether the appointment should be rescheduled. HHS recommends settling how connectivity problems will be handled in advance.

What to do if the connection breaks up

  • Tell the clinician promptly if the sound or picture is breaking up.
  • Use the fallback you agreed on—such as switching to a phone call or rescheduling—instead of repeatedly reconnecting without a plan.
  • Before ending the contact, confirm the next steps and how to reach the clinic afterward with questions. HHS’s patient-preparation guidance includes follow-up contact and care instructions.

Check connectivity and cost assistance directly

For eligible households, HHS points to the federal Lifeline program as a possible source of help with internet and connected-device costs. Eligibility and current program terms must be checked before relying on assistance; do not assume you qualify.
