Keep a structured, access-controlled audit trail that connects each AI agent action to the request that triggered it, the agent and tools involved, applicable policy and approvals, the action’s outcome, and reliable timestamps. Protect the record from unauthorized changes, minimize sensitive content, and set retention and review rules for the system’s purpose and jurisdiction. Logs can help reconstruct an incident; they do not, by themselves, prove that an action was lawful, authorized, complete, or admissible as evidence.

What an AI agent audit trail needs to show

A transcript records what was said. An action log must also show what the agent attempted to do and what happened outside the conversation: for example, whether a tool call was denied, an approval was granted, or an external system changed state. Record both decisions and effects, including failures. Otherwise, a reviewer may see an agent’s intention without being able to tell whether it acted.

Use structured events with stable identifiers to connect the triggering request, agent run, delegated work, tool call, and result. Record events at the point where an action is authorized and executed, and correlate them with records from the systems that receive the action. A NIST recommendation relevant to this design is that system-level logs may lack the application-level detail needed to understand a particular action.

Which fields should each event contain?

Choose a consistent event format and include only fields relevant to the system and the review purpose. The following is a practical baseline, not a mandatory universal schema.

Field group Record Review value
Time and correlation Event timestamp, duration, workflow or session ID, and correlation ID linking related events. Places events in sequence and connects a tool call to its originating run.
Identity and delegation Authenticated user or service identity, agent identity and version, and relevant delegation chain. Shows which identities participated and under whose authority.
Decision context Action requested, applicable policy or configuration version, execution decision, and approval state. Shows why an action was permitted, denied, or sent for review.
Tool and target Tool or integration name, target resource, and normalized parameters sufficient to identify the action. Distinguishes, for example, a read from a write and identifies the intended destination.
Model and information context Relevant model, prompt or configuration, dataset, and retrieved-document versions or references. Helps explain the conditions under which the agent made its decision.
Outcome and state Relevant input or output references, resulting state change, result, error, retry, or denial. Separates an attempted action from a completed effect and makes failures visible.
Logging health Delivery status, gaps, and logging-system errors. Prevents missing records from being silently treated as proof that no action occurred.

Singapore’s government addendum on securing AI systems recommends monitoring and logging across models, data stores, memory, agents, tools, MCP interactions, agent communications, external actions, and relevant systems. Its example fields include actions, inputs and outputs, internal state changes, errors, timestamps and duration, and contextual identifiers. Apply that coverage according to your architecture; a tool inventory can help identify boundaries that a conversation transcript misses.

How to build the logging workflow

1. Define scope, purpose, and ownership

Inventory the agents, tools, integrations, data sources, and actions each can take. Classify actions by impact, including whether they are destructive, financial, administrative, or externally visible. Identify the user or service identities that can authorize them, and assign owners for access, review, retention, and incident exports. This defines which events matter and who is accountable for the record.

2. Emit correlated events at action boundaries

Generate events when a request is evaluated, a policy decision is made, an approval is requested or recorded, a tool call is attempted, and the receiving system returns a result. Use correlation identifiers to join those records across agent runs, delegated subtasks, and systems. Normalize parameters where practical so reviewers can distinguish materially different actions without relying on free-form narrative.

  • 2K Resolution, Day and Night Clarity: Capture every event that occurs around your home in 3MP resolution. More than just daytime, 4 LED lights increase the light source by 100% compared to 2 LED lights, allowing more to be seen for excellent color night vision.

Capture enough context to reconstruct the action, but avoid copying secrets or entire sensitive payloads into widely accessible logs. Store controlled references to source records where that is sufficient; use redaction, encryption, role-limited access, and audited access for sensitive records. NIST cautions that audit records can themselves contain personal information that creates disclosure risk.

3. Bind approval to the action being approved

For high-impact actions, keep the agent’s decision separate from execution. OWASP recommends an independent policy or execution component that validates scope, privilege, and approval. Bind an approval to the actor, tool, target, normalized parameters, timestamp, and expiry so it cannot casually be reused for a different action. Log the decision and its result.

Decide in advance what happens when a required control is unavailable. For high-impact actions, fail closed if policy lookup, approval validation, risk classification, or required logging fails. Do not let an action proceed merely because the audit path is down.

4. Protect integrity and retain lifecycle records

Limit who can read, change, export, or delete logs. Where practical, separate routine log administration from review access, and record access to sensitive audit data. NIST identifies digital signatures and write-once devices as possible integrity controls; the UK government’s implementation guide recommends write-once, read-many (WORM) storage for critical systems with compliance requirements. These measures can help detect or prevent alteration, but no storage choice alone guarantees that a record will be admissible evidence.

Include material changes to models, datasets, prompts, and parameters in the relevant audit trail, not just runtime actions. The UK guide treats operation and lifecycle management as part of an AI system’s audit trail, including critical changes to training, datasets, prompts, and parameters.

5. Set retention and review rules for the context

Document the purpose and applicable retention requirement for each system and record class. Avoid a blanket rule to keep every agent record indefinitely: retain enough context for the intended review and investigation needs while applying privacy and deletion requirements. NIST advises managers to determine retention and emphasizes timely review.

Choose periodic review, event-triggered review, automated analysis, or a combination based on risk and volume. Name the alert triage owner and the people permitted to export records. Manual real-time review is generally impractical for large systems, NIST notes, so automated filtering and analysis can help direct attention to events that need human assessment.

6. Test whether the trail is complete

Exercise permitted and denied actions, expired approvals, errors, retries, delegated tasks, and external effects. Compare records from the action system with those in the log store to find unlogged boundaries. Test that a required logging failure blocks an action when policy says it must, and confirm that reviewers can query by identity, application, time, and workflow and export a readable package. These checks follow from OWASP’s enforcement guidance and NIST’s recommendations for audit review.

How long should AI agent logs be kept?

Retention depends on the system’s purpose, applicable law, and record type; the EU AI Act is not a global default. Article 12 of Regulation (EU) 2024/1689 requires high-risk AI systems to technically allow automatic event logging over the system’s lifetime. Those logs should support risk identification, post-market monitoring, and operational monitoring. Article 12 also sets additional minimum logging requirements for certain remote biometric identification systems. These provisions are scoped to high-risk systems, not every AI tool.

Under Article 19, providers must keep automatically generated logs under their control for a period appropriate to the intended purpose of the high-risk AI system, with a minimum of six months unless applicable Union or national law provides otherwise. That is not a universal rule for every organization’s logs: other requirements, including those concerning data protection, health, finance, employment, or a litigation hold, may affect the answer. The European Commission AI Act Service Desk describes its explanatory summaries as non-binding; consult the regulation and qualified counsel for a specific retention decision.

How to preserve logs for an incident or legal review

When an incident or anticipated legal matter calls for a record export, preserve the relevant source records and their context rather than relying on a transcript or a newly assembled summary alone. Document the source system, time of export, person responsible, and integrity checks. Restrict access to the review copy and follow counsel’s direction on preservation and chain of custody when litigation or an investigation is anticipated.

An audit trail supports reconstruction and accountability; it does not establish that model output was true, that processing was lawful, or that a court will admit the record. NIST specifically flags the need for legal counsel when logs may be used as evidence. The log also creates privacy and confidentiality risks: a NIST NCCoE commenter on agentic identity noted that linked identifiers and agent activity can expose sensitive information and enable profiling. Balance attribution and accountability with access controls and data minimization.

Choosing logging tools

There is no single tool or storage configuration that is right for every agent system, and government examples of monitoring products are not certifications for legal evidence. Evaluate any approach against the actual action path and review process. Useful criteria include:

  • Whether it captures events at tool and external-action boundaries, including denials and errors.
  • Whether identity, delegation, policy, and approval context can be correlated with each action.
  • Whether integrity controls can be independently checked and access is auditable.
  • Whether sensitive data can be minimized, redacted, or referenced rather than copied wholesale.
  • Whether retention, deletion, query, and export controls match the applicable jurisdiction and purpose.
  • Whether the system behaves as required when logging or approval services fail.
