R0–R5 can be a useful way to describe how much oversight an AI tool action needs, but it is not an established industry standard. Treat it as a proposed policy framework—not permission for an agent to act. Classify each action by its consequences and context, verify the actor’s authorization, require approval when policy calls for it, and enforce those checks at execution time.

What the R0–R5 tool risk model means—and what it does not

The available evidence does not establish a complete, authoritative R0–R5 specification. A recent community discussion using the matching title gives only a partial example: R0–R2 automatic, R3–R4 confirmed, and R5 blocked. That fragment does not define all six tiers or establish that this mapping is canonical. Do not treat it as a standard without a full specification from the model’s author.

NIST supports adapting oversight to an organization’s AI risks, not this particular six-level scale. Its Generative AI Profile (NIST AI 600-1) says organizations may apply existing risk tiers or revise them for generative AI, and may need different levels of oversight or human-AI configurations. NIST’s AI Risk Management Framework is voluntary; it is a framework for managing risks, not a prescribed R0–R5 taxonomy.

Why classify tool actions instead of assigning one risk to an agent?

An agent’s risk changes with what it is doing. Reading a document is not equivalent to editing it; editing a local draft is not equivalent to sending it to an external recipient. Code execution, data deletion, administrative changes, and financial transfers can have different impacts even when the same agent invokes them.

OWASP’s AI Agent Security Cheat Sheet illustrates action-based classification with four labels—low, medium, high, and critical—not R0–R5. In its example, mapped low-risk actions can skip human review, while medium-, high-, critical-, and unmapped-tool actions require review. This is OWASP’s example, not a universal taxonomy or a translation table for R0–R5.

What should determine an action’s risk tier?

If an organization adopts R0–R5, it should document its own tier definitions and classify the concrete action in context. Useful dimensions, synthesized from OWASP’s guidance, include:

  • Potential impact: What harm or disruption could result if the action is wrong, misused, or triggered by malicious input?
  • Reversibility: Can the action be reliably undone, including its effects on people or external systems?
  • Data sensitivity: What information can the tool read, change, or disclose?
  • Access scope: Which files, accounts, systems, or administrative functions are within reach?
  • External visibility: Could the action send a message, publish content, or otherwise affect someone outside the controlled environment?
  • Trust boundaries: Does execution cross from a constrained workspace into a production system, external service, or another security domain?

These are practical assessment dimensions, not a scoring formula prescribed by OWASP or NIST. A tier should represent the risk of the specific operation with its actual permissions and context—not merely the name of a tool or the identity of the agent.

How to enforce a tier without confusing it with permission

  1. Classify the proposed operation. Identify the exact action, target, data, and side effect. If the action is unmapped or its effects are unclear, route it for policy review instead of letting it inherit broad autonomy.
  2. Check the initiating actor’s authorization. Confirm that the user, agent, or service identity may perform that operation on that target. Approval by a person does not grant permissions the actor does not have.
  3. Apply the required approval. Require explicit approval for high-impact or irreversible actions, and add controls for destructive, financial, administrative, or externally visible operations. Approval should be tied to the specific action, not treated as a blanket authorization.
  4. Enforce policy in a separate execution component. The component that runs the tool should check authorization and any required approval before execution. OWASP states: “This classification does not grant permission to run a tool; the execution component must still check the actor’s authorization and any required approval for the exact action.”
  5. Log the resulting side effect. Record what was requested, which identity initiated it, what policy and approval were applied, and what the tool actually changed or sent. This creates an audit trail for investigation and policy improvement.

OWASP also says: “Require explicit approval for high-impact or irreversible actions.” The important design point is to enforce this at the point of execution; a label displayed in a prompt or policy document cannot stop an otherwise authorized tool from running.

How to compare two AI tools or actions

Compare capabilities and enforcement, not just product names or claimed autonomy. The same comparison works for two tools and for two operations available through one tool.

Dimension Questions to ask
Data and systems What information can each action read or change, and which accounts, files, or systems can it reach?
Consequences and reversibility What could go wrong, how serious would the impact be, and can the result be undone?
External communication Can it send messages, publish material, or otherwise create effects outside the controlled environment?
Enforcement Where are actor authorization and any required human approval checked: in the actual execution path or only in instructions to the agent?

OWASP’s DevSecOps guidance for AI Agent and MCP Security recommends least agency: give an agent only the autonomy, tools, and access required for its task, and only for as long as needed. Narrow access reduces the consequences of a mistaken classification or compromised agent.

How R0–R5 relates to established guidance

NIST’s AI Risk Management Framework helps developers, users, and evaluators manage AI risks to individuals, organizations, society, and the environment. It is voluntary and NIST says version 1.0 is being revised. The framework supports organizations adapting risk management to their circumstances; it does not supply definitions for R0, R1, R2, R3, R4, or R5.

That distinction matters operationally: a team may adopt six tiers, four categories, or another policy structure, but must define what each level requires and ensure the execution system enforces it. An R-number without documented consequences, approval rules, authorization checks, and handling for unknown actions is only a label.
