Phantom smells—odors you perceive when nothing nearby is producing them—do not have one universal fix. The right approach depends on the cause: some smell changes improve as an infection or allergy settles, while persistent symptoms deserve medical assessment. If the smell continues for more than a few weeks, contact a healthcare professional.
Phantosmia means smelling an odor that is not present. Parosmia is different: a real odor is present, but it smells distorted or changed. Both are smell disorders, but the distinction can help describe what you are experiencing. Cleveland Clinic and the National Institute on Deafness and Other Communication Disorders (NIDCD) explain these conditions.
What can you do about phantom smells?
Focus on the possible underlying cause rather than trying to mask the odor. If the symptom began with a respiratory infection or allergy, it may ease as that condition improves. Saline nasal rinsing may help some smell changes related to infection or allergy, but it is not a cure for every case of phantosmia. - If you use a saline rinse, follow the product instructions and keep the device clean.
- If you think a prescribed medicine may be contributing, discuss it with your doctor. Do not stop or change it on your own; a clinician can decide whether a change is appropriate, as the NIDCD advises.
- Do not assume a decongestant spray will treat phantosmia. It is not established as a general treatment for phantom smells.
When should you see a doctor?
Arrange an appointment if the smell persists for more than a few weeks. Consider seeking advice sooner if it began after a head injury, comes with other concerning symptoms, or is accompanied by significant nasal symptoms. A clinician can determine whether an examination or further assessment is warranted. The Cleveland Clinic and NHS advise assessment for persistent smell changes.

A phantom smell by itself does not establish a neurological emergency or a particular diagnosis. If it occurs alongside sudden or severe symptoms, use your local urgent or emergency care guidance for those symptoms rather than trying to judge the situation from the smell alone.
What might cause a phantom smell?
Possible associations include upper respiratory or sinus infections, allergies, nasal polyps, medication effects, dental problems, smoking, head injury, migraine, and conditions affecting the nervous system. COVID-19 can also precede smell disorders. These are possibilities, not a way to diagnose the cause yourself. Many cases have no identified cause, and serious structural explanations are much less common than the broader list of possible associations suggests. Note whether you perceive the smell in one nostril or both and tell the clinician. One-sided symptoms can be a clue to nasal involvement, but this detail is not enough to identify the cause on its own. Cleveland Clinic notes clinicians may ask about this, and NHS Scotland discusses it as a possible clue.
What happens during an evaluation?
A clinician may ask what the odor is like, when it started, how long episodes last, whether it seems to come from one or both nostrils, and whether you recently had an infection, injury, medication change, or other symptoms. An examination may include the nose, ears, head, and neck. Depending on the history and findings, the clinician may consider nasal endoscopy, CT or MRI imaging, or an EEG; these are possible tests, not routine steps for everyone. Cleveland Clinic describes this individualized approach.

For context, NHS Scotland’s Centre for Sustainable Delivery fact sheet, version 2 reviewed December 20, 2024, says 5% of 2,569 Swedish adults over age 60 reported phantosmia. Its summary of a six-year follow-up of 44 people with idiopathic phantosmia reports that 30% resolved, 25% improved, 40% stayed the same, and worsening was rare; the reported percentages total 95%, so the remaining outcome is not specified. NHS Scotland primary-care guidance says isolated phantosmia without focal neurological symptoms or signs generally does not require neurological investigation or treatment; associated symptoms and examination findings guide referral decisions.
