If an AI agent calls an unauthorized tool or proposes an unsafe action, stop the call at the tool boundary: enforce authorization and validate arguments in trusted application code immediately before dispatch. Do not rely on a system prompt to prevent an action the application is able to execute. While you investigate, disable or narrow the implicated tool, use an isolated environment for reproduction, and require action-specific approval for consequential operations.
Why an agent can violate a policy
An agent’s answer and its actions are different things. A model can propose a tool call; the surrounding application decides whether that call is authorized and whether it executes. If the application dispatches calls without checking permissions, a mistake or manipulation in the model’s context can become a real side effect.
OWASP describes the broader risk as “Excessive Agency”: an agent may cause harm when it has more functionality, permissions, or autonomy than the task requires. The trigger might be an ordinary model error, an ambiguous instruction, prompt injection, a compromised extension, or a misleading tool result. The consequences depend not just on what the model says, but on what the tool can access and do.
Prompt injection is one route into this failure. Malicious instructions may be embedded in external content such as a web page, email, or document. OpenAI describes prompt injection as a third party misleading a model through instructions placed in its context. Retrieved text and third-party tool descriptions should therefore be treated as untrusted data—not as authority to override policy, grant permissions, or change who may approve an action.

A refusal in the final response does not establish that the agent did nothing. The tool may already have run before the model produced a refusal or explanation. Investigation must examine the call and resulting state, not only the final text. OWASP’s LLM Prompt Injection Prevention Cheat Sheet makes the same operational point: “A refusal in the final response does not undo an action already taken.”
Keep the distinction between model behavior and system behavior clear while diagnosing the incident. The model may have selected a tool or generated risky arguments, but the application’s decision to dispatch those arguments is the enforcement point. A clearer prompt may reduce some mistaken proposals; it cannot make a broad credential narrow, verify a caller’s authority, or undo an operation that has already changed state.
Contain the incident before changing prompts
Stop or narrow the risky capability
Disable the implicated tool or reduce its permissions while you investigate. If the agent has several ways to perform the same risky action, assess those paths too; leaving an equivalent capability enabled may not contain the incident. If there is evidence that credentials were exposed or misused, revoke or rotate them as appropriate. Avoid making broad changes that erase evidence before you understand what happened.
The Tool Desk

If a tool combines reading and writing, narrowing it to the required read-only use may be safer than leaving the full capability active. If you cannot confidently limit the operation or target, temporarily disable the tool while preserving the configuration and logs needed to reconstruct the event. The goal is to prevent further side effects without losing the facts needed to fix the actual control failure.
Preserve the evidence needed to reconstruct the event
Retain the agent version, tool configuration, relevant inputs, and available execution logs. Record what the initiating user or principal was allowed to do, what content was in context, which tool was selected, the arguments supplied, the permissions in force, and whether an external side effect occurred. Do not assume a model-generated summary captures the actual execution sequence.
Useful evidence follows the operation from request to outcome: the initiating principal, the model’s proposed call, the application’s authorization decision, the dispatched tool and arguments, any approval decision, and the resulting state. If a key stage is not logged, note that limitation rather than treating the final answer as a complete account. This helps distinguish a proposal that was blocked from an operation that ran and then received a refusal or explanation.
Separate a proposed call from an executed call
Trace the complete path from request to outcome: the model’s proposed tool call, the application’s authorization decision, the tool dispatch, and any resulting change. This distinction matters because a policy violation in generated output and an unauthorized operation are related but not identical failures. A model may propose something the application correctly blocks; conversely, a polite or compliant-looking answer may follow a call that already succeeded.

Find the control failure at the tool boundary
For every attempted call, answer a few concrete questions before choosing a fix:
- Who initiated it? Identify the user, service, or other principal on whose behalf the agent was acting.
- What entered the context? Review relevant user instructions, retrieved pages, emails, documents, tool outputs, and third-party descriptions. Distinguish trusted policy from untrusted content.
- What did the agent request? Capture the tool name and the full arguments actually presented for execution.
- What did the application authorize? Determine which checks ran, what permissions applied, and whether approval was required and verified.
- What happened to state? Establish whether the operation was read-only, changed data, or produced an externally visible effect. Identify whether any change can be reversed.
Use this reconstruction to locate the failure: excessive tool access, broad credentials, missing authorization checks, insufficient argument validation, absent approval, or inadequate isolation can each contribute. A prompt edit may help make the agent’s intended behavior clearer, but it does not repair an application that will execute a forbidden call anyway.
Classify the failure by the control that should have stopped it. If the agent could reach unrelated resources, the scope was too broad. If the caller lacked authority but the call still ran, caller authorization was missing or ineffective. If the tool was permitted but the model supplied a harmful target or operation, argument validation and target-level checks need attention. If an externally visible action proceeded without review, approval was missing, too broad, or not verified at execution. More than one weakness may be present, so correcting only the most visible prompt or model behavior can leave the same route open.
Enforce authorization in trusted code
Place the decisive check immediately before the tool is dispatched, in application or infrastructure code that controls the side effect. The check should evaluate the caller’s authority, the requested tool and operation, the target resource, and the arguments. Apply deny-by-default rules: an operation should not run unless the system can establish that it is allowed. Reject malformed or out-of-scope arguments rather than asking the model to repair or approve its own request.

Repair common Windows errors and clear accumulated junk for a smoother, more stable PC - no reinstall needed.Free scan · no reinstallMake authorization specific enough to the task. Permission to read one resource should not automatically imply permission to write to it or to access unrelated resources. Scope access by tool, operation, and target resource, and use credentials limited to the required capability. Where practical, separate read and write capabilities so that an agent that only needs to retrieve information cannot modify the underlying data.
The model may help interpret a request or prepare a candidate action, but its classification, confidence, or claim that an action is safe is not the authorization mechanism. The trusted boundary must make the decision using the actual caller, operation, target, and arguments. If the request is unclear or the authorization check cannot establish that it is permitted, do not dispatch it.
Validate the exact arguments that will be executed
Check the concrete arguments after the model has selected a tool and before the tool runs. Confirm that the target is within the caller’s scope and that the operation matches the permitted action. Validate required fields and reject malformed values. If a human approves an action, bind that approval to the actual tool and arguments rather than treating it as general permission for later calls. Any change to the proposed operation or its target should be checked again at the action boundary.

Validation must apply to the values that will actually reach the tool, not merely to an earlier description of what the agent intends to do. A check that sees one target while dispatch sends another does not enforce the intended restriction. Likewise, a valid-looking tool name does not by itself establish that the caller may perform every operation offered by that tool. Check the operation and resource together, then reject the call if required details are missing, malformed, or outside the allowed scope.
Match safeguards to the impact of an action
Not every tool call needs the same level of control. A read-only lookup, a reversible internal change, and an externally visible operation have different consequences. Choose controls based on permission scope, side-effect severity, reversibility, human-review needs, and whether the system can record what it decided and did.
| Action characteristics | Control emphasis | What to verify |
|---|---|---|
| Read-only access to a task-relevant resource | Keep the resource scope narrow and authorize the caller and target before dispatch. | The agent cannot read unrelated resources or use a read permission to write. |
| Write action with limited or reversible impact | Check the operation and target at the boundary; limit the credential and tool scope. | The exact arguments are within the caller’s authority and the change is observable. |
| Consequential or externally visible action | Require action-specific human review and validate approval immediately before execution. | The reviewer sees the actual tool and arguments, and approval applies to that action only. |
| High-impact action that is difficult to reverse | Use the narrowest permissions, human review, and isolation feasible for the task. | The system can block an unapproved call and record the decision and outcome. |
This is a decision framework, not a substitute for the authorization check. Human review adds a control for consequential actions; it does not make broad credentials or unchecked dispatch safe. Likewise, a read-only label is not enough if the tool can reach data outside the intended scope.
When comparing possible fixes, ask where enforcement happens, how narrowly permissions are scoped, how severe and reversible the side effect is, whether human review is needed, and whether tests can observe the attempted call, decision, and resulting state. A prompt change acts at the model-behavior layer; authorization at the application or infrastructure boundary governs whether the side effect can happen. For a consequential write, prefer the latter as a required control, with review and narrow permissions layered on top. For a task-relevant read, a narrower resource scope and boundary check may be proportionate, but it still needs a test that demonstrates unrelated resources cannot be reached.
Require meaningful approval for consequential actions
Pause high-impact or externally visible operations for human review. The reviewer should see the actual tool, operation, target, and arguments that will run—not a vague description such as “complete the task.” Approval should be tied to that proposed action and checked at execution time. If the arguments change after review, the system should treat the changed call as a new proposal and validate it again.
Quick wins for a faster PC:
Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →Keep the final authorization decision in the application boundary. The model should not be able to create, infer, or claim a human approval on its own. Record the approval decision alongside the attempted call and outcome so that later review can establish which action was authorized.
Approval is useful only when the human can evaluate the action that will occur and the system enforces the decision against that same action. A general approval of an agent’s goal does not necessarily authorize each tool call the agent might make to reach it. Do not let a later change to the tool, target, or arguments silently inherit an earlier approval. Recheck authorization even after approval: human review is an additional safeguard, not a replacement for the caller’s permissions or the tool-boundary check.

Reduce the agent’s blast radius
Remove tools the task does not need and limit the data those tools can reach. Use scoped credentials rather than broad credentials, and separate capabilities where that makes permissions easier to enforce. Run the agent in an isolated environment with restricted network access where feasible, especially when reproducing a suspicious behavior. These controls reduce the damage possible if the agent makes an unexpected call; they do not replace per-action authorization.
Treat retrieved content, tool output, and third-party tool descriptions as untrusted. A web page or document can supply information for the task, but it should not be allowed to redefine the agent’s permissions. Tool descriptions also help shape what the model attempts, so review them as part of the agent’s configuration; however, a well-written description is not an access-control boundary.
Think of blast-radius reduction as limiting both what the agent can ask for and what its tools can reach. Removing unused capabilities reduces the set of possible actions. Resource-scoped permissions limit the reach of the remaining capabilities. Scoped credentials constrain what the tool can do if the agent requests an unexpected operation. Isolation and restricted network access reduce exposure during reproduction. None of these measures makes every call correct, so keep the authorization decision at the boundary and make rejection observable.
Test the boundary safely
Reproduce the problem with dummy data or sandboxed substitutes for real tools. Avoid using production data or live credentials to test whether an unsafe call is blocked. Make the expected decision observable: record attempted calls, authorization outcomes, approvals, and state changes. A test that checks only the final response can miss an executed action.
Build tests for both blocked and allowed behavior
- Adversarial input: Include relevant cases where external content tries to instruct the agent to bypass policy, change permissions, or invoke a tool outside the task.
- Unauthorized operation: Try calls that exceed the initiating principal’s authority, target an out-of-scope resource, or contain invalid arguments. Confirm the boundary rejects them before dispatch.
- Approval required: Confirm that a consequential action pauses, that the reviewer sees the exact proposed call, and that execution does not proceed without approval tied to that call.
- Benign allowed task: Test ordinary requests that should remain permitted. A control that blocks every call may prevent misuse, but it does not demonstrate a workable fix.
- Outcome and state: Check logs and the sandboxed state to verify both that rejected actions did not run and that allowed actions produced the expected result.
Keep the incident pattern as a regression test. When tools, prompts, integrations, permissions, or models change, rerun the relevant blocked and allowed cases. Reassess the boundary whenever a change alters what the agent can request or what the application can execute.
Make the test answer two separate questions: did the system reject the unsafe call before dispatch, and did the allowed task still complete as intended? Confirm the first by checking the authorization decision and that the sandboxed state did not change. Confirm the second by checking the expected result, not just a success message. If the response says an operation was blocked but the state shows it ran, the control failed regardless of how appropriate the final wording sounds.

Monitor calls and review changes
Log tool attempts, rejected calls, effective permissions, approvals, and outcomes. These records help distinguish a model proposal from an executed operation and make it possible to see whether a control blocked the request. Include enough context to reconstruct the decision path, while handling logs according to the data protections appropriate to the environment.

Review the agent’s permissions when its tools, integrations, prompts, or model change. A new integration may expand the available actions; a change to a credential may expand the reachable resources. Keep the permission set aligned to the task rather than allowing capabilities to accumulate without review.
Monitoring should cover attempted calls as well as successful ones: rejected attempts can reveal a boundary doing its job, while a successful call without a corresponding authorization or approval record may signal a gap. Review the recorded tool, operation, target, decision, approval, and outcome together. Add the observed incident pattern to regression tests, then rerun those tests when a change affects the available tools, prompts, integrations, model, or permissions.
Common fixes that do not solve the authorization problem
- Adding “never call unsafe tools” to the system prompt: This may express intended behavior, but it cannot enforce a restriction if the application still dispatches an unauthorized call.
- Trusting a final refusal: A refusal after execution does not undo an action already taken. Inspect tool logs and resulting state.
- Approving a general plan: Broad approval is not a substitute for action-specific review. Show and validate the actual tool and arguments.
- Removing one tool without checking alternatives: Another enabled integration may provide the same capability. Review the complete set of available paths and effective permissions.
- Testing only adversarial cases: Confirm that the unsafe call is blocked, but also verify legitimate tasks still work.
A practical remediation checklist
- Disable or narrow the implicated tool; revoke or rotate credentials if exposure or misuse is indicated.
- Preserve the agent version, tool configuration, inputs, and available logs.
- Reconstruct the caller, context, proposed tool and arguments, authorization decision, execution, and resulting state.
- Enforce deny-by-default authorization and argument validation in trusted code immediately before dispatch.
- Remove unneeded capabilities, narrow resource scopes, and isolate reproduction from production data and credentials.
- Require action-specific review for consequential operations and verify approval at execution time.
- Test adversarial and benign cases with observable call decisions and state changes; add the incident pattern to regression tests.
- Monitor attempts, rejections, permissions, approvals, and outcomes, and revisit controls after configuration changes.
Frequently Asked Questions
Will changing the system prompt fix an agent that calls an unsafe tool?
A prompt change may clarify intended behavior, but it is not an authorization boundary. Enforce the caller’s permissions, operation, target resource, and arguments in trusted application or infrastructure code immediately before dispatch.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsWhat should I do first after discovering an unsafe call?
Disable or narrow the implicated tool while investigating. Preserve the agent version, tool configuration, relevant inputs, and available logs. Revoke or rotate credentials if there is evidence they were exposed or misused.
How can I tell whether an unsafe tool call actually ran?
Trace the proposed call through the application’s authorization decision and dispatch, then inspect execution logs and resulting state. A final refusal or explanation does not prove that no side effect occurred.
When should an agent’s action require human approval?
Pause consequential or externally visible operations for action-specific review. Show the reviewer the actual tool, operation, target, and arguments, and verify that approval at execution time. If those details change, validate the changed action again.
How should I test a fix without risking production data?
Use dummy data or sandboxed tool substitutes, not production data or live credentials. Check attempted calls, authorization decisions, approvals, and sandbox state. Test both adversarial cases that should be blocked and legitimate tasks that should remain allowed.
Do I need to remove every tool from the agent?
No. Remove tools and permissions the task does not need, but preserve the capabilities required for legitimate work. Scope remaining access to the needed tool, operation, and resource, then test that unrelated access is blocked.
The Bottom Line
Fix unsafe tool use where a proposed action becomes a real operation: enforce deny-by-default authorization and validate exact arguments in trusted code immediately before dispatch. Contain the incident, limit the agent’s permissions, require action-specific approval for consequential operations, and verify the result in a sandbox with observable logs and state. A prompt, a refusal, or a broad human approval is not a substitute for that boundary.
