The safest way to evaluate an AI medical scribe is to treat it as a clinical-documentation system, not a transcription gadget. Define the work you need it to do, test it on representative visits in your own workflow, and compare candidates on note accuracy, omissions, attribution errors, editing effort, EHR integration, privacy, consent, patient experience, support, and contract terms. Keep a clinician in control: every generated note must be reviewed and corrected before it is signed or becomes part of the medical record.
Ambient scribes typically listen to a clinical conversation, create a transcript, and produce a draft note. They can reduce documentation work, but they can also omit findings, add unsupported details, or misattribute statements. The decision framework below is designed to expose those risks before a purchase or broad rollout.
What an AI medical scribe does—and what it does not do
An AI medical scribe processes a visit conversation and turns it into documentation such as a SOAP, DAP, or BIRP note. Depending on the product, it may run inside an electronic health record (EHR), connect through an application programming interface (API), or operate as standalone software. The output is a draft, not an autonomous clinical record.
The clinician remains responsible for clinical decisions and documentation accuracy. The American Psychological Association’s Guidance for the Evaluation of AI Scribes (December 2025) states: “You must review AI generated documentation for accuracy and edit as needed prior to signing and the documentation becoming part of the clinical record.” It also says, “While AI may assist with documentation, you remain responsible for all clinical decisions and documentation accuracy.” The Australian Commission on Safety and Quality in Health Care makes the same point in its AI Safety Scenario – Ambient scribe (Version 1.0, August 2025): responsibility for the quality and accuracy of information and summaries placed in health records remains with the clinician.

That division of responsibility should shape procurement. A product that saves clicks but makes errors difficult to find is not a safe improvement. Your evaluation must measure the complete path from conversation to signed note.
Start by defining the job and the practice context
Write a short intended-use statement before watching vendor demonstrations. It should describe the encounters, users, systems, and documentation standard the tool must support. A generic demo can hide gaps that appear immediately in your practice.
Specify the encounters
- List the specialties and subspecialties that will use the system.
- Identify visit types, including new-patient assessments, follow-ups, urgent visits, procedures, behavioral-health sessions, and group or multi-party encounters.
- State whether visits are in person, by telehealth, or both.
- Describe the patient population, including age ranges, communication needs, languages, dialects, accents, and use of interpreters.
- Note situations in which recording or ambient capture will not be appropriate.
Specify the documentation output
- Choose the required note formats, such as SOAP, DAP, or BIRP, and identify specialty-specific sections.
- List templates, macros, problem-list conventions, medication fields, diagnosis coding steps, and follow-up instructions that must be preserved.
- Define what clinicians must be able to edit, delete, or add before signing.
- Decide whether the practice needs one template or different templates by clinician, specialty, visit type, or payer requirement.
Specify workflow and technical constraints
- Name the EHR and version, authentication method, available APIs, supported devices, and network restrictions.
- Map when the recording starts and stops, who initiates it, where the draft appears, and where the final author signs it.
- Include downtime, unsupported visit types, shared rooms, interpreter use, and service interruptions in the requirements.
- Document staffing, training time, support expectations, and the person who will administer templates and permissions.
Ask each vendor to demonstrate these exact scenarios rather than a polished generic appointment. If a product cannot support a required encounter or note element, record that as a fit issue instead of assuming an update will solve it.
Crashes, No Sound, or Screen Glitches?

Request product-specific validation evidence, not only a general accuracy claim. The Australian Commission and APA guidance both emphasize checking intended use, safety, and validation for the target population. Ask:
- Who was included in testing, and how closely did those patients, clinicians, specialties, languages, and visit modes match yours?
- Were encounters real, de-identified, or simulated? What workflow and note templates were used?
- How were omissions, fabricated or unsupported details, incorrect attribution, and other errors defined and counted?
- Were clinicians asked to review complete notes, or were only transcripts or isolated sentences scored?
- How were performance differences across patient groups, accents, languages, and multi-party visits assessed?
- How are model or product updates tested, approved, documented, and communicated to customers?
- What evidence covers the specific version and configuration you would deploy?
Be cautious with a single headline score. A 2025 evaluation published in Mayo Clinic Proceedings: Digital Health compared five platforms across 14 simulated ambulatory encounters and found variability in transcript and note quality. That study demonstrates why products can differ; its simulated design is not a performance estimate for your practice and cannot replace local validation.
Ask for limitations as explicitly as strengths. A vendor should be able to identify unsupported languages, visit types, noisy environments, speaker-attribution limitations, and known failure modes. Refusal to provide that information is itself a procurement risk.
Run a representative, structured pilot
A pilot should reproduce normal work and create evidence that clinicians can inspect. Do not select a tool from a demo alone or from a trial involving only unusually straightforward visits.

1. Select the sample
Include encounters that reflect your intended deployment: different clinicians, specialties, visit lengths, patient ages, communication styles, languages or accents, in-person and telehealth sessions, and single- and multi-party visits. Include both routine and complex documentation. Document which scenarios were included so that a favorable result cannot be mistaken for universal performance.

2. Establish the reference
For each encounter, preserve what was actually said and the documentation standard the practice expects. The reference may include the authorized recording or transcript, clinician-authored note, required template fields, and information that must not appear because it was not discussed. Follow your privacy and retention rules when creating pilot materials.
3. Use the same workflow for every candidate
- Configure each candidate with the intended templates, permissions, devices, and EHR connection.
- Have clinicians conduct representative encounters without changing their normal communication solely to help the tool.
- Generate the draft through the ordinary workflow.
- Have the clinician compare the draft with what was said and with the practice’s documentation standard.
- Record corrections, deletions, additions, approval time, failures, and any need to leave the EHR to complete the task.
- Repeat scenarios when a defect appears so you can distinguish a one-off failure from a pattern.
4. Score the output
Track more than whether a note “looks good.” Record:
- Omissions: clinically or operationally important information from the encounter that is absent.
- Unsupported additions: details the patient or clinician did not state.
- Hallucinations: plausible-sounding but invented facts, findings, medications, diagnoses, or plans.
- Attribution mistakes: a statement assigned to the wrong speaker, especially in family, group, or interpreter-mediated visits.
- Over-summarization: compression that removes context, uncertainty, chronology, or the patient’s own concerns.
- Template completeness: required sections, fields, and formatting present in the correct location.
- Editing burden: time and number of changes needed before the clinician can approve the note.
- Approval control: whether the clinician can correct and authorize the final record before it enters the chart.
Review errors by clinical significance, not only by count. A single invented medication instruction can matter more than many harmless punctuation errors. Have more than one experienced clinician inspect a subset of notes when possible, and compare performance across patient groups and visit modes rather than reporting only an overall average.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errors5. Set decision rules in advance
Before seeing results, define the minimum acceptable performance for each required scenario, the maximum editing time, and which failures are disqualifying. Require remediation or exclusion for unsafe unsupported additions, persistent attribution errors, inability to review before signing, or a workflow that bypasses the practice’s record controls. Keep the raw examples and correction logs so the governance team can reassess the decision later.
Compare integration and workflow fit
Integration is not just a list of supported EHR names. It determines where clinicians work, how much copying is required, and whether the final note is authorized correctly.
| Option | What to examine | Questions for the vendor |
|---|---|---|
| EHR-bundled | Where capture starts, where drafts appear, template support, permissions, and upgrade dependencies | Can clinicians review, edit, and sign in the normal chart workflow? Which features require a separate application? |
| API-connected | Data exchanged, authentication, synchronization, failure handling, and maintenance when either system changes | Which fields move automatically? What happens when the API, network, or EHR is unavailable? |
| Standalone | Device setup, copying or exporting notes, duplicate records, and access controls outside the EHR | How is the final note transferred and verified? Can drafts be prevented from entering the chart before approval? |
Walk through a complete encounter with each option: patient arrival, consent, capture, pause or stop, draft generation, clinician editing, final authorization, follow-up documentation, and an interruption. Count clicks and screen changes, but also note cognitive friction. A technically connected product can still be unsuitable if clinicians must switch contexts, reconcile duplicate fields, or manually verify every imported element.
Confirm how the system handles unsupported visits, multiple speakers, interpreters, poor connectivity, and service outages. The practice needs a documented fallback, such as its existing manual documentation process, rather than an assumption that the service will always be available.
Test privacy, security, and data handling in the contract
Audio, transcripts, generated notes, metadata, and correction history can all contain sensitive health information. Review the actual privacy policy, terms of service, security documentation, and contract; do not rely on a marketing statement that a product is “HIPAA compliant.” Compliance depends on your configuration, obligations, and the agreement as well as the vendor’s controls.

Data lifecycle questions
- What data are collected during a session: audio, transcript, draft, final note, device identifiers, timestamps, or user activity?
- Where are each type of data stored, and in which countries or regions?
- How long are audio and transcripts retained by default, and can retention be shortened or disabled?
- How do you request deletion, and what copies remain in backups or logs?
- Are data de-identified, and what does that process remove or preserve?
- Are customer data used to train, fine-tune, evaluate, advertise, or improve a model? Is that use opt-in, opt-out, or prohibited by contract?
- Are data sent to third-party model providers, subprocessors, analytics services, or support personnel? Who can access them?
Security controls
- Encryption in transit and at rest, with clear scope and key-management responsibilities.
- Role-based access, least privilege, audit logs, account lifecycle controls, and multifactor authentication.
- Independent security attestations, penetration testing, vulnerability management, and incident-notification commitments.
- Administrative controls for recording devices, shared workstations, remote access, and exported files.
- Business continuity, backup, disaster recovery, and a usable process for service interruptions.
United States and other jurisdictions
For a U.S. practice, the APA recommends determining whether a business associate agreement (BAA) is required and obtaining a signed BAA when applicable. Review the BAA alongside the privacy policy and service terms, including subcontractor coverage, permitted uses, breach duties, return or destruction of data, and termination assistance. A BAA does not remove the practice’s responsibility to configure and use the system appropriately.
Outside the United States, apply the privacy, health-record, recording, cross-border-transfer, and medical-device rules that govern the practice’s jurisdiction. Guidance from the APA, NHS England, and the Australian Commission is context-specific; none establishes a universal legal requirement for every country or specialty. Obtain local legal or privacy advice when the rules are uncertain.
Plan patient communication and consent
Patients should understand when an AI scribe is being used and what it does. Explain that the visit may be recorded or otherwise processed, that the system creates a draft for clinician review, and that the clinician remains responsible for the final record. Tell patients how the information is handled and what alternative documentation process is available.
Recommended Free Tools
Create a short, plain-language script and a consistent workflow for documenting the patient’s choice. The process should make it easy to decline or withdraw consent where appropriate, without disadvantaging the patient. Clarify whether consent is obtained at each visit or under a continuing policy, and how a change of mind is recorded.
Recording and consent requirements vary by jurisdiction and can change. The APA notes that state rules may impose specific consent requirements; the Australian Commission highlights consent and the handling of sensitive information. These examples do not determine the law where your practice operates. Confirm the applicable rules before the pilot and again before launch, including requirements for telehealth, minors, interpreters, and group visits.
Measure patient experience during the pilot. Ask whether the explanation was understandable, whether the device or conversation changed how comfortable patients felt, and whether any group was less willing or able to participate. A technically accurate tool is not a good fit if its use undermines trust or access.
Assess usability, training, support, and accessibility
Have representative clinicians use the product after a short, realistic orientation. Observe whether they can start and stop capture, correct a draft, find the source context they need, and approve the final note without assistance. Include nurses, medical assistants, scribes, interpreters, and practice managers when their work changes.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Ask the vendor for:
- Role-specific training materials and an administrator guide.
- Support hours, response targets, escalation contacts, and procedures for safety incidents.
- Release notes and advance notice for model, template, integration, or privacy changes.
- Accessibility information for clinicians and patients who use assistive technology.
- Guidance for downtime, unsupported encounters, device replacement, and account recovery.
- A process for submitting corrections or feedback and learning whether reported defects were fixed.
Usability includes the editing experience. A draft that is usually correct but hard to inspect can create more risk than a less polished draft with clear source context and simple correction controls. Confirm that clinicians can see what the system produced, change it, and explicitly approve only the version that becomes part of the record.

Compare candidates with a weighted decision matrix
Use the same evidence for every candidate and keep separate scores for mandatory requirements and preferences. Weight the criteria according to your risk and workflow rather than accepting a vendor’s ranking.
| Evaluation axis | Evidence to collect | Decision prompt |
|---|---|---|
| Accuracy and completeness | Pilot examples, omissions, unsupported additions, attribution errors, clinically significant defects, editing time | Does the output meet the practice’s safety and documentation standard in representative visits? |
| Population and specialty fit | Languages, dialects, accents, age groups, specialties, visit modes, multi-party and interpreter scenarios | Does performance hold for the people and encounters we actually serve? |
| EHR and workflow integration | Supported version, API or native connection, template mapping, approval path, downtime behavior | Can clinicians complete the job without unsafe copying, duplicate entry, or context switching? |
| Privacy and data terms | Retention, deletion, training use, subprocessors, storage location, access, BAA or local agreements | Do the controls and contract match our legal and operational obligations? |
| Consent and patient experience | Disclosure language, decline process, documentation, accessibility, feedback | Can patients make an informed choice without disrupting care? |
| Clinician control | Editability, source visibility, approval gate, audit trail, correction workflow | Is clinician review unavoidable before the note becomes the record? |
| Monitoring and support | Incident process, update testing, release communication, training, support response | Can we detect and address performance changes after launch? |
| Total cost and contract | License or usage charges, implementation, devices, integration, training, support, renewal, termination, data return | What is the complete cost and what obligations continue if we leave? |
The available guidance supplies evaluation criteria, not a universal winning product. Choose the candidate that satisfies mandatory safety, privacy, consent, and workflow requirements and demonstrates acceptable local performance—not the one with the most impressive general claim.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Set governance before rollout
Assign a named owner or committee before signing. Its charter should cover approval, training, configuration, audit, incident escalation, patient communication, and review after updates. Include clinical leadership, privacy or compliance staff, information technology, frontline users, and procurement as appropriate.
Monitor the live service
- Sample records from every supported specialty and visit mode for omissions, additions, attribution errors, and template failures.
- Track clinician editing time, corrections, overrides, user complaints, patient feedback, and near misses.
- Review incidents promptly and preserve enough context to determine whether the cause was the model, configuration, workflow, user action, or integration.
- Compare results after model, template, EHR, device, or policy changes.
- Retrain staff when recurring errors or workflow drift appears.
Define escalation and pause criteria
Document who can suspend a template, disable capture for a visit type, notify affected clinicians, and investigate a potentially incorrect record. Establish how corrected notes are handled and how patients are informed when an error could affect care. Do not wait for an annual review to address a pattern of unsafe output.
Recheck contracts and privacy materials
Revisit retention, subprocessors, model-training language, security attestations, and BAAs or equivalent agreements when the vendor changes its service. Keep the deployed configuration, version, and approval record so an audit can reconstruct what produced a note.
Common failure modes and practical responses
The demo works, but complex visits fail
Cause: The evaluation used short, single-speaker, routine encounters. Response: Add long, multi-party, interpreter-mediated, telehealth, and specialty-specific visits to the pilot; require evidence by scenario rather than one blended score.
The note sounds fluent but contains invented details
Cause: A plausible summary masks unsupported additions or overconfident wording. Response: Compare every clinical assertion with the encounter, classify unsupported content by severity, and require an approval gate that makes review unavoidable.
Important information is missing
Cause: The system over-summarizes, loses chronology, or does not map a required field. Response: Test the exact template, list must-have elements, inspect omissions by visit type, and reject configurations that cannot reliably preserve them.

Speakers are misattributed
Cause: Family members, interpreters, clinicians, and patients overlap or change speakers. Response: Include multi-party scenarios, verify attribution line by line, and define when ambient capture is inappropriate.
Clinicians stop reviewing carefully
Cause: Trust grows faster than evidence, or the interface hides source context. Response: Train users that they remain responsible, audit signed notes, display clear approval controls, and use incident feedback to reinforce review behavior.
The integration creates duplicate or incomplete records
Cause: API mapping, permissions, or downtime behavior was not tested. Response: Trace fields through a complete encounter, test failures deliberately, and provide a documented manual fallback.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Patients are surprised or uncomfortable
Cause: Disclosure is rushed, unclear, or does not offer a practical alternative. Response: Use plain-language notice, make declining straightforward, document the choice, and monitor feedback by patient group.
A vendor’s “HIPAA compliant” claim ends the review
Cause: Marketing language substitutes for contract and configuration analysis. Response: Review the privacy policy, terms, BAA where applicable, retention and training provisions, subprocessors, access controls, and your own obligations with qualified privacy or legal advisers.
A launch checklist
- Intended use, specialties, visit types, languages, patient groups, templates, and exclusions are documented.
- Product-specific validation evidence and known limitations have been reviewed.
- A representative pilot has measured omissions, additions, hallucinations, attribution, completeness, editing time, and approval controls.
- The EHR path, API or standalone workflow, permissions, downtime process, and manual fallback have been tested.
- Audio, transcripts, drafts, final notes, retention, deletion, de-identification, training use, subprocessors, storage, encryption, access, and incident terms are documented.
- A BAA or other required agreement is signed, and local privacy and recording rules have been checked.
- Patient disclosure, consent, decline, withdrawal, and documentation procedures are ready.
- Clinician and staff training, accessibility, support, release communication, and escalation contacts are in place.
- A governance owner, audit schedule, incident process, update review, and pause criteria are assigned.
- The practice has defined what evidence would trigger remediation, restricted use, or discontinuation.
What a defensible decision looks like
A defensible purchase decision connects a specific practice need to evidence from the same practice context. It explains which encounters were tested, what errors occurred, how much correction was required, how patients and clinicians experienced the workflow, what data the vendor retains, and how the practice will monitor changes. It also states where the tool will not be used.
If the pilot cannot establish safe performance for an important population or visit type, limit deployment to the scenarios that were validated or defer adoption. A smaller, well-governed rollout is safer than broad use based on a generic demonstration. The 2025 platform study’s variability across five products and 14 simulated encounters is a reminder that product names alone do not answer the practice’s question; local evidence and continuing clinician oversight do.
