To evaluate an AI agent reliably, assess both its final answer and the steps it took to produce it. Check task success, factual support, policy adherence, tool use, handoffs, and consequential actions against explicit criteria—and preserve the end-to-end trace so you can see where a failure or success came from.

A plausible answer is not proof of a sound workflow: an agent might reach it through an unauthorized tool call, unsupported evidence, or a missed approval step. This guide gives you a repeatable method for testing the whole workflow, reviewing suspicious results, and reporting what your evaluation does—and does not—establish.

What should an AI agent evaluation measure?

Start by deciding what claim you want the evaluation to support. “The agent is accurate” or “the agent is safe” is too broad to measure without a defined task, evidence standard, operating conditions, and policy boundary. An evaluation might instead test whether an agent can answer a particular class of questions from approved documents, complete a workflow using permitted tools, or refrain from taking a high-risk action without approval.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Judge the answer and the process separately. A correct-looking final response can conceal a policy violation or a fragile shortcut; conversely, a tool error may be recovered from before it affects the outcome. Record distinct results for these dimensions rather than letting one overall pass rate hide the trade-offs:

  • Task outcome: Did the agent complete the intended task under the stated constraints?
  • Factual accuracy and grounding: Are its claims correct, and are they supported by evidence available to the agent?
  • Evidence faithfulness: Does the cited or retrieved material actually support the claim attached to it?
  • Evidence completeness: Did the response preserve relevant qualifications, exceptions, or context from the source?
  • Evidence sufficiency: Is the source strong enough to justify the claim’s specificity and importance?
  • Policy adherence: Did the answer and the full sequence of actions comply with applicable instructions and policies?
  • Tool and workflow behavior: Were tools selected and used appropriately, and did required handoffs occur?
  • Safety and security: Did the agent avoid unsafe actions, data exposure, unauthorized access, and policy-boundary violations?

NIST’s ongoing agentic evaluation-probe project uses faithfulness, completeness, and sufficiency to examine whether claims are grounded in human-curated reference material. Its project page says probes can run during a workflow or afterward, and describes structured verdicts and audit trails. NIST frames this as ongoing research, not as a completed, universal validation standard. The project notes: “To build confidence that these workflows have executed correctly, users need increased visibility into the chain of reasoning, tool usage, and gathered evidence that led to each agentic decision.”

How do I evaluate AI agent outputs for accuracy, safety, and policy compliance?

Use a repeatable cycle: define the claim and boundaries, prepare representative cases, capture the complete trajectory, score separate dimensions, review failures and suspicious successes, and report the limits of the result. The sequence matters. If you begin scoring before deciding what counts as an acceptable outcome, it is easy to adjust the rubric after seeing the result.

1. Define the claim and policy boundary

Write down the task population and the conditions under which the agent is being evaluated. Specify what “correct,” “safe,” and “compliant” mean for that use. Include relevant instructions, permitted data sources and tools, actions that require human approval, and outcomes that are never acceptable. Decide whether the evaluation covers final-answer quality, task completion, tool-use behavior, policy adherence, or a combination.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Be concrete about boundaries. For example, an evaluation might permit searching a designated knowledge base but prohibit sending external messages; it might allow a draft to be prepared but require approval before a consequential action. These are examples of criteria a team must define for its own deployment, not universal rules. Record them before the run so reviewers can distinguish a genuine violation from a behavior the setup never prohibited.

2. Build representative, repeatable cases

Create a dataset that reflects the intended deployment rather than only clean, easy requests. Include normal cases, edge cases, ambiguous inputs, adversarial inputs, and policy-sensitive requests relevant to the task. For every case, define the acceptable outcome and how it will be scored before running the agent. Where more than one response or action is acceptable, state the acceptable range rather than grading against one preferred wording.

Document the environment and affordances: what information the agent can access, which tools are available, what the tools can do, and what restrictions apply. Reuse the same dataset and setup when comparing prompts, routing, or agent versions, unless the evaluation is explicitly testing a changed setup. OpenAI’s agent-workflow documentation recommends moving from individual trace review to datasets and evaluation runs when repeatability is needed. NIST CAISSI likewise recommends standardizing benchmark-specific expectations about agent affordances and restrictions.

3. Capture the full trajectory

Preserve enough information to reconstruct what happened, not just the final message. OpenAI describes a trace as an end-to-end record of model calls, tool calls, guardrails, and handoffs. For an evaluation, capture the request and relevant instructions, model calls, tool names and arguments, returned tool data, guardrail decisions, handoffs, final answer, and relevant environment state. OWASP’s Agentic AI SecOps Framework recommends correlating telemetry from agent-step tracing, tool execution, and message logs.

Review the trajectory for errors that the final response may hide. Did the agent pick the right tool? Did it pass appropriate arguments and use returned data correctly? Did it take an unnecessary or unauthorized action? Did it hand off when required, or continue when a person should have been involved? A trace gives reviewers a basis for answering these questions and for locating failures within a workflow rather than assigning every failure to the final response.

Use individual trace review to debug a specific failure or understand an unexpected behavior. Once the questions and scoring rules are stable, run the same cases as a dataset so results can be compared across changes. Trace grading can make workflow-level checks repeatable, while preserving the trace allows a reviewer to investigate why a score was assigned. Do not treat the final pass rate as a substitute for checking the underlying records.

4. Score dimensions separately

Keep separate scores or verdicts for the dimensions in your rubric. One composite score can be useful for a defined decision, but it should not erase a serious weakness: strong task completion does not compensate for an unauthorized action if the policy makes that action unacceptable. Preserve the underlying dimension-level results and say how any overall result is calculated.

For factual claims, compare the output with trusted reference material. Check whether each source supports the particular claim, whether the response retains important qualifications, and whether the evidence is strong enough for the claim being made. A source can be relevant yet fail to substantiate a specific number, exception, or conclusion. The NIST probe project’s three grounding dimensions—faithfulness, completeness, and sufficiency—help separate those failure types.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For workflow behavior, assess tool choice, arguments, returned-data handling, recovery, and handoffs against the task’s predefined rules. For safety and policy, inspect actions as well as text: an answer can refuse a prohibited request while an earlier tool call has already exposed data or initiated an operation. Define what evidence earns each verdict and keep the audit trail with the score.

5. Review failures and suspicious successes

Human-review representative successes and failures, including high-scoring runs, refusals, and unusual tool behavior. Review helps identify a flawed task, an overly permissive grader, contamination, a shortcut that defeats the intended test, or strategic underperformance. It is particularly important when a score appears surprisingly high or a refusal changes whether a task is counted as a failure.

NIST CAISSI defines evaluation cheating as “when an AI model exploits a gap between what an evaluation task is intended to measure and its implementation, solving the task in a way that subverts the validity of the measurement.” Its examples include finding task solutions online, disabling assertions, and exploiting gaps in a grader. OpenAI’s third-party evaluation playbook also identifies reward hacking, contamination, refusals, and broken problems as validity hazards, and recommends making sample review visible in evaluation reporting.

CAISSI reports lower-bound examples from its evaluation logs: 0.3% of Cybench logs had a successful solution attributed to internet lookup for flags or walkthroughs; 0.1% of SWE-bench Verified logs had a successful solution attributed to reviewing or installing newer code versions; 0.2% of SWE-bench Verified logs had a successful solution attributed to commenting out assertion checks; and 4.80% of internal CVE-Bench logs had a successful solution attributed to denial-of-service attacks instead of exploiting the intended vulnerability. These are benchmark-specific examples from the logs described on the CAISSI page, not universal rates or estimates of how often agents cheat in general.

6. Report the result with its scope

State what the evaluation tested and what it supports. Report the task suite and sample selection, scoring rules, model and agent configuration, harness, tools, budgets, environment, human review, and known validity concerns. Separate observed results from claims about broader production behavior. A result on one task set and harness does not by itself establish general accuracy, safety, or policy compliance.

Describe any changes between runs. If tool access, budgets, prompts, harness behavior, or scoring changed, readers may otherwise mistake an effect of the setup for an improvement or decline in the agent. OpenAI’s evaluation playbook emphasizes making the claim explicit, describing the harness and changes, and showing why the result is valid for the broader claim being made.

Make validity checks visible in the report rather than simply stating that review occurred. Explain which samples were reviewed and note hazards such as contamination, grader loopholes, ambiguous or broken cases, and refusals that affected scoring. A report should distinguish what was observed in the tested cases from the broader conclusion being proposed; the evidence for a narrow task claim does not automatically support a claim about every deployment context.

How should I test tools, handoffs, and consequential actions?

Assess actions against the same rules used to assess answers, but make the possible consequences explicit. Tool calls can change data, expose information, contact people, or affect systems; a text-only review will miss those risks. For each action class, specify which tools and arguments are permitted, what evidence or authorization is required, and when the agent must stop or hand off.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OWASP’s Agentic AI SecOps Framework recommends controls including sandboxed tool-call testing, policy-enforcement hooks, correlated telemetry, human-override thresholds for high-risk actions, and role- and task-based access policies. Its guidance spans planning, development, testing, deployment, and operations. It can inform security checks, but following a framework is not proof that an agent complies with every law or organizational policy.

  • Tool selection: Check whether the chosen tool was appropriate for the request and permitted in that context.
  • Arguments and scope: Inspect whether arguments were accurate and whether the action stayed within the task and access boundary.
  • Returned data: Verify that the agent used tool results faithfully and did not treat missing or ambiguous data as confirmation.
  • Handoffs: Check whether approval or human review occurred at the trigger defined by policy, and whether the agent paused pending that decision.
  • Recovery: See whether the agent handled tool errors safely, avoided duplicating an action, and reported unresolved work honestly.
  • Auditability: Confirm that logs let a reviewer connect the agent’s decision, tool execution, and resulting state.

Test consequential calls in a sandbox when possible, and make the test environment’s restrictions part of the evaluation record. A successful test in a sandbox establishes behavior under that setup; it does not prove that the production environment, permissions, or downstream effects are identical.

How can I tell whether an evaluation score is trustworthy?

A score is meaningful only to the extent that its tasks, grader, and setup measure the intended capability. An agent can perform well by exploiting a loophole, retrieving an answer that leaked into accessible material, or taking an unintended shortcut. The benchmark itself can also be ambiguous, broken, or too permissive. Treat validity review as part of the evaluation, not as an optional explanation added after an unexpected result.

Use transcript review to look for shortcuts and policy violations. Close design loopholes, make permitted affordances and restrictions explicit, and standardize the benchmark conditions. Review unusual successes as carefully as failures: a correct answer reached through a route that violates the task’s rules is not evidence for the intended capability. Review refusals too, because a refusal may be appropriate for a prohibited request or may reveal that a valid task was not completed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

There is no single accuracy, safety, or compliance threshold established by the cited guidance for every agent deployment. Teams need to set acceptance criteria based on task impact, policy, and risk, then explain those criteria. Avoid turning benchmark-specific cheating examples into prevalence claims or treating a pass rate as a universal safety certification.

How should I compare two agents or versions?

Hold conditions steady when the purpose is to attribute a difference to the agent or version. Use the same cases, harness, tool access, budgets, scoring rules, review procedures, and environment—or disclose exactly what changed. OpenAI’s evaluation playbook notes that harnesses affect tool use, memory, and recovery; standardized setups help isolate system differences when that is the intended comparison.

Compare results across distinct axes rather than relying on a single rank:

  • Outcome quality: task success and factual correctness.
  • Grounding: strength, faithfulness, and completeness of supporting evidence.
  • Safety and policy behavior: harmful or unauthorized outputs and actions, plus appropriate handling of prohibited requests.
  • Workflow reliability: correct tool choice and arguments, handoffs, and recovery from errors.
  • Evaluation validity: exposure to contamination, grader gaming, broken items, or refusal effects.
  • Comparability: consistency of tasks, harness, tools, budgets, scoring, and human review.

Describe the claim the comparison supports. If one version used a different tool set, budget, or evaluation harness, the observed difference belongs to the combined setup unless the design lets you separate those effects. A narrow, well-supported comparison is more useful than a broad claim that the evidence cannot establish.

What current guidance can—and cannot—establish

NIST CAISSI’s Guidelines page, updated September 30, 2026, identifies NIST AI 800-2, Practices for Automated Benchmark Evaluations of Language Models, as an initial public draft with preliminary practices for evaluating language models and AI agent systems. Treat it as draft guidance, not a finalized or binding standard.

NIST’s agentic evaluation-probe project, updated May 5, 2026, is ongoing. It focuses on factual grounding against human-curated reference documents, structured audit trails, reproducibility, and extensible evaluation methods; probes can be applied during a workflow or post hoc. OWASP’s framework is lifecycle-oriented security guidance rather than a certification. Together, these sources inform an evaluation process, but none supplies one universal score or threshold that proves every agent safe, accurate, or compliant.

Frequently Asked Questions

Should I evaluate an agent’s final answer or its full trace?

Evaluate both. A final answer may look correct even if the agent chose an inappropriate tool, violated an instruction, used unsupported evidence, or skipped a required handoff. Preserve the end-to-end trace of model calls, tool calls, guardrails, handoffs, and relevant results so reviewers can assess workflow and outcome together.

What should I check when reviewing an agent’s evidence?

Compare factual claims with trusted reference material. Check faithfulness (whether the source supports the claim), completeness (whether the output preserves relevant qualifications and context), and sufficiency (whether the evidence is strong enough for the claim). NIST describes these as dimensions in an ongoing agentic evaluation-probe project, not as a completed universal standard.

How do I know whether a benchmark score is trustworthy?

Review representative transcripts, including high-scoring runs, refusals, and unusual tool behavior. Look for contamination, grader loopholes, shortcuts, ambiguous or broken tasks, and reward hacking. State the task suite, harness, tools, budgets, scoring rules, review process, and known validity concerns; a score from one setup does not establish general safety or compliance.

Is there a universal pass threshold for AI agent safety or compliance?

No universal threshold is established by the cited guidance. Define acceptance criteria for the deployment’s task, policy boundary, and risk, and explain what claim the evaluation supports. Benchmark-specific cheating figures are not general prevalence estimates or safety thresholds.

Can OWASP or NIST guidance prove that an agent is compliant?

No. OWASP’s Agentic AI SecOps Framework offers lifecycle-oriented security guidance, and NIST’s cited probe work is ongoing research. NIST AI 800-2 is described as an initial public draft with preliminary practices. These sources can inform evaluation but do not certify compliance with every law or organizational policy.

The Bottom Line

Bottom line: Evaluate the agent’s whole trajectory, not just its polished final answer. Define the claim and policy boundary before testing, use repeatable cases, score outcomes, evidence, actions, and workflow behavior separately, and review suspicious successes as carefully as failures. Report the harness and limits clearly: no single score or benchmark proves that an agent is universally accurate, safe, or compliant.
