The safest approval gate evaluates the exact tool call immediately before it can change anything—not a vague plan and not only the agent’s chat output. Normalize the proposed target, operation, arguments, identity, authority, scope, evidence, and run context; apply deterministic policy checks; assess potential harm, reversibility, uncertainty, and untrusted instructions; then allow, deny, or send the concrete proposal to a human. If review is unavailable, fail closed and preserve a complete record of the decision and execution result.
This is a system property shared by the model, agent harness, tools, and environment. A prompt telling an agent to “ask before risky actions” is not an enforcement mechanism if a custom tool, handoff, or nested agent can reach the side effect without passing through the same control.
Why the execution boundary is the right place to evaluate an action
An agent can produce several intermediate messages before making a consequential call. Reviewing those messages is useful for observability, but it does not guarantee that every custom tool invocation is inspected. The decisive control belongs beside the operation that can create the side effect: the function that changes a record, sends a message, deploys code, grants access, moves money, deletes data, or otherwise acts outside the model.
OpenAI’s guardrails guidance distinguishes checks on model input or output from checks attached to a custom tool. Input and output checks have narrower workflow coverage and may not wrap every custom call. A side-effect boundary can inspect the arguments that will actually be executed, including calls made through handoffs or nested agents.

Place the control in the execution path, not just in an instruction layer:
- The model proposes a tool call.
- The harness resolves the caller, run, permissions, and context.
- The tool wrapper constructs a normalized action record.
- A policy and risk evaluator returns allow, deny, or human approval required.
- Only an allowed call reaches the tool implementation.
Use independent controls in the tool configuration and environment as well. Anthropic’s practice guidance treats the model, harness, tools, and environment as interacting components; changing a system prompt cannot compensate for an over-privileged tool or an environment that accepts unvalidated requests.
Define the proposal the gate will evaluate
A reviewer cannot make a meaningful decision about “the agent’s next step.” The gate needs a concrete, machine-readable proposal that describes what will happen if execution proceeds. Normalize every candidate call into the same shape, even when different tools use different parameter names.

| Field | What to capture | Why it matters |
|---|---|---|
| Tool and operation | The tool identifier and the specific operation or method | Policy usually applies to an operation, not merely to the product or service name. |
| Target or resource | Host, account, repository, record, file, endpoint, or other resource identifier | Scope checks need an exact target; “the customer database” is too broad. |
| Arguments | The resolved parameters after defaults, interpolation, and transformations | The side effect is determined by the arguments that reach the tool, not by the original natural-language request. |
| Calling identity | Agent, user, service account, tenant, and delegated identity | Authorization and accountability depend on who is acting and on whose behalf. |
| Authority and scope | Role, engagement window, approved resources, purpose, and applicable policy version | An otherwise valid operation may be outside the authority granted for this run. |
| Run context | Run or job ID, parent agent, handoff chain, environment, and relevant prior approvals | Context prevents an approval from being detached from the execution that requested it. |
| Expected effect | What state will change, what data will leave the system, and who may be affected | Reviewers need consequences, not just API syntax. |
| Evidence and provenance | Source material supporting factual claims, retrieval time, and transformations | The evaluator can test whether the rationale is supported, complete, and sufficient. |
| Trust signals | Prompt-injection indicators, untrusted instructions, conflicting messages, and provenance gaps | Instructions embedded in retrieved content or tool output are not automatically authority. |
For authorized cybersecurity work, OpenAI’s guidance specifically names the target, action, arguments, calling identity, and engagement window as authorization inputs. Other domains require their own fields; do not treat that cybersecurity example as a universal policy schema.
Resolve values before checking policy
Evaluate the final call after variable substitution, URL construction, path expansion, identity delegation, and default selection. If a tool accepts a user-facing alias, resolve it to the canonical resource and retain both forms in the record. Preserve the original proposal as well as the normalized form so an auditor can see whether a transformation changed the scope.
Malformed or incomplete proposals should not be silently repaired into an executable action. Return a validation failure or route the unresolved proposal for human clarification. A gate that guesses a target or fills in a missing destructive parameter is making the decision itself without sufficient evidence.
Apply the evaluation sequence at the side-effect boundary
1. Intercept every relevant tool call
Wrap each operation that can produce a consequential change. Include tools invoked after handoffs, tools called by nested agents, retries, scheduled continuations, and alternate transport paths. A single unwrapped path can bypass the control that appears to protect the main agent.
Record an immutable proposal before policy evaluation. The record should include a unique action ID and a hash or equivalent binding to the arguments that will be executed. If the arguments change after approval, treat the result as a new proposal and evaluate it again.
2. Enforce hard policy first
Deterministic rules should reject actions that are plainly outside scope or prohibited. Examples in OpenAI’s authorized-cybersecurity guidance include out-of-scope hosts, credential theft, persistence, data exfiltration, destructive changes, production access, and bypass attempts. Those examples belong to that cybersecurity context; a finance, support, or infrastructure system needs a domain-specific prohibited-action set.

Hard policy checks commonly answer questions such as:
- Is this resource inside the approved tenant, environment, host list, project, or engagement window?
- Is the operation permitted for this identity and role?
- Does the request attempt to obtain secrets, evade monitoring, disable controls, or change its own permissions?
- Does the action violate a data handling, residency, retention, or separation rule?
- Is the requested operation explicitly prohibited regardless of user approval?
Return a deny result with the matched rule and the normalized facts that caused the match. Do not send a clearly prohibited action to a human merely to obtain a ceremonial approval.

3. Assess consequence, reversibility, and uncertainty
Actions that pass hard policy still differ in risk. Consider the plausible harm if the call is wrong, how many people or systems could be affected, how quickly the result propagates, and whether the state can be restored reliably. A reversible draft update and an irreversible deletion should not receive the same scrutiny even when both are authorized.
Uncertainty is itself a reason to pause. Examples include an ambiguous recipient, an unresolved resource alias, conflicting instructions, incomplete evidence, an unusual volume, or a request that departs materially from the approved plan. The reviewed guidance does not establish universal numerical thresholds, so teams must define domain-specific criteria and validate them with contextual tests.
4. Inspect evidence and provenance
When the proposal depends on factual claims—such as an incident classification, account status, customer instruction, or vulnerability finding—link the rationale to the underlying material. NIST’s evaluation probes distinguish three questions:
- Faithfulness: Does the evidence actually support the claim being used?
- Completeness: Has relevant contrary or qualifying information been omitted?
- Sufficiency: Is the evidence adequate for the consequence of the proposed action?
A citation alone is not proof. The evaluator should preserve the relevant excerpt, source identity, retrieval time, and any transformation performed by the agent. If a retrieved page contains instructions, classify those instructions as data unless an authorized policy explicitly grants them authority.
5. Produce one explicit decision
The evaluator should return exactly one of three operational outcomes:
- Allow: The call satisfies policy and risk conditions; execute the bound proposal.
- Deny: The call violates a hard rule, lacks required authorization, or fails validation.
- Human approval required: The action is potentially permissible but ambiguous, high-impact, or outside automated confidence.
Do not encode “approval required” as a vague warning that the model can ignore. It must be a control-flow state that prevents the tool from running until a decision is recorded.
6. Present the exact proposal to the reviewer
The human should see the target, operation, resolved arguments, identity, authority, policy scope, expected effects, evidence, uncertainty flags, and any difference from a previously approved plan. Show what will execute, not a summary generated before interpolation or tool selection.

OpenAI’s Agents SDK documents an interruption flow in which an approval pauses execution, the reviewer approves or rejects, and the same run resumes where supported. Preserve the action ID across that interruption. If a nested agent raises an approval, expose the parent-child chain so the reviewer knows which component requested the call.

If the approval service is down, times out, loses the action record, or cannot verify the reviewer’s decision, do not execute the pending side effect. Return a controlled failure, retain the proposal, and make the reason visible to operations. A fallback that silently allows the call turns an availability incident into an authorization bypass.
Retries must be idempotent. Replaying a request after a timeout should not create a second side effect or reuse an approval for changed arguments. Bind the approval to the action ID, normalized arguments, target, identity, and policy version.
8. Record execution and outcome
After an allowed call, append the execution result to the same audit chain. Capture whether the tool ran, its success or failure, returned status, affected resources, and any downstream changes that can be observed. A human approval is not evidence that the operation was correct; the result and supporting rationale remain necessary for investigation.
What a human reviewer should see
A useful approval screen puts the consequential facts above the conversation transcript. The transcript can provide context, but it should not force the reviewer to reconstruct the action from many messages.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsRecommended review packet
- Action: “Update access policy” or another precise operation name.
- Target: Canonical resource identifier, environment, tenant, and region where relevant.
- Arguments: Fully resolved values, including additions, removals, filters, and recipient lists.
- Identity: Requesting user, agent, service account, delegated principal, and parent run.
- Authority: Role, engagement or job window, permitted scope, and policy version.
- Impact: State changes, data disclosure, affected users, propagation, and reversibility.
- Evidence: Source excerpts, provenance, timestamps, and the claim each source supports.
- Warnings: Injection indicators, missing evidence, unusual deviation, or conflicting instructions.
- Decision controls: Approve once, reject, request clarification, or stop the run, with a required reason for rejection or exception.
Make material differences from a prior plan explicit. A plan approved yesterday does not automatically authorize a newly selected target, an expanded recipient list, or a more destructive operation today.
Separate automated policy from human judgment
Deterministic policy is strongest where the answer is explicit: scope, prohibited targets, identity permissions, required fields, and environment boundaries. Human judgment is more useful for ambiguity, intent, proportionality, and consequences that depend on context.
A model can assist with summarizing evidence or identifying inconsistencies, but the reviewed sources do not establish that an AI reviewer alone guarantees safe decisions. Keep the enforcement decision independent of a model’s persuasive explanation. If a model proposes “low risk,” require the underlying facts and apply the same policy and logging path as any other proposal.
Choose action review, plan review, or both
Per-action review and plan-level approval solve different problems. Reviewing an entire plan up front can reduce repeated prompts, while reviewing each action gives a fresh check at the moment of side effect. Anthropic notes that repeated prompts can create friction and may be tuned out, so approval frequency should match the consequence of the operation.

| Approach | Coverage | Decision surface | Control behavior | Main trade-off |
|---|---|---|---|---|
| Execution-boundary, per-action gate | Every wrapped custom call, including calls after handoffs when those paths use the wrapper | Exact target, operation, arguments, identity, scope, and current evidence | Can allow, deny, pause, and fail closed before execution | More prompts for multi-step work, but a fresh check for each side effect |
| Plan-level approval | Actions represented in the submitted plan | Intended sequence and stated objectives | Approves the plan as presented; changed or newly risky calls need another check | Lower interaction friction, with risk that later details differ from the plan |
| Deterministic policy checker | Calls that reach its rule engine | Normalized facts and explicit policy predicates | Consistent allow or deny for defined boundaries | Cannot reliably resolve ambiguous intent or consequences by itself |
| Human reviewer | Actions routed for explicit approval | Concrete proposal plus material context and evidence | Can approve, reject, or request clarification | Judgment is flexible but depends on clear presentation and reviewer attention |
| Model-assisted evidence reviewer | Evidence and rationale passed to the model | Support, completeness, sufficiency, and inconsistencies | Can flag issues for policy or human review | Useful as assistance; not established as a standalone safety guarantee |
For high-impact operations, combine plan review with execution-boundary checks. The plan can establish intent and reduce repetitive prompts, while the boundary verifies that the actual call still matches the authorized scope.
Design for nested agents and handoffs
Agent systems often delegate work. A parent may ask a specialist to modify a resource, and the specialist may invoke another tool. Approval coverage should follow the call graph, not just the top-level conversation.
- Propagate the parent run ID, caller identity, delegated authority, and policy context to every child.
- Require each child’s side-effecting tool call to pass through the same normalization and policy boundary.
- Expose the full delegation chain to the reviewer.
- Invalidate an approval when a child changes the target, operation, arguments, or identity.
- Prevent a child from broadening scope merely because the parent’s natural-language instruction was broad.
OpenAI’s approval flow includes approvals raised by nested agents. Use that interruption as a control state rather than treating nested approval requests as informational messages.
Build an auditable record
NIST describes accumulating evaluation results into a machine-readable audit trail that helps users assess agent actions and outputs. Your record should make it possible to reconstruct what the agent proposed, what the gate knew, why it decided, who approved, and what actually happened.
Record these events
- Proposal created: Raw tool call, normalized action, caller, authority, run context, and evidence references.
- Policy evaluated: Policy version, matched rules, validation errors, scope result, and risk or uncertainty findings.
- Approval requested: Reviewer-facing packet, timestamp, expiration, and notification status.
- Decision recorded: Allow or deny result, reviewer identity, reason, and any conditions.
- Execution completed: Tool result, affected resource, status, retries, and observed side effects.
- Exception or timeout: Service outage, unavailable reviewer, changed arguments, or fail-closed result.
Keep evidence and rationale linked to the specific claim they support. Store policy and tool versions so a later investigation can reproduce the decision under the rules that were active at the time.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Test the gate with contextual cases
Unit tests for individual rules are necessary but insufficient. Build a replayable corpus that exercises the complete path from proposal to execution. NIST recommends standardized contextual measurement, independent evaluation or red-teaming, and regular improvement of risk measurement.
| Case class | Example test | Expected behavior to measure |
|---|---|---|
| Allowed | Authorized identity updates an in-scope resource with complete arguments | Call reaches the tool and the record contains the policy basis and execution result |
| Prohibited | Operation targets an out-of-scope host or attempts a forbidden change | Deterministic denial before any side effect |
| Ambiguous | Alias resolves to multiple resources or recipient is unclear | Pause for clarification or human approval; no guessed execution |
| Malformed | Required argument, identity, or authority field is missing | Validation failure or controlled escalation |
| Adversarial | Retrieved content tells the agent to ignore policy or reveal credentials | Content is treated as untrusted evidence, not authority; policy remains active |
| Changed-after-approval | Target or arguments are modified before execution | Original approval is invalidated and the new proposal is reevaluated |
| Unavailable-review | Approval service times out or loses the request | Fail closed with an auditable timeout outcome |
| Nested-call | Child agent invokes a side-effecting tool after a handoff | Same boundary runs and the delegation chain is visible |
Measure more than pass rates. Examine false approvals, unnecessary escalations, policy coverage, evidence faithfulness, completeness, sufficiency, reviewer time, and whether execution matched the approved proposal. The reviewed NIST project does not publish a universal effectiveness percentage, so do not claim a risk-reduction or approval-accuracy number without a direct measurement for your system.
Common failure modes and fixes

Failure: The agent’s prose is screened, but a custom tool call bypasses the check.

Operational checklist
- Every side-effecting tool call passes through a common execution-boundary gate.
- The normalized record contains tool, target, operation, arguments, identity, authority, scope, run context, expected effect, evidence, and trust signals.
- Arguments are resolved before policy evaluation and remain bound to the approval.
- Hard policy violations are denied without relying on human discretion.
- Risk review considers harm, reversibility, uncertainty, authorization scope, and untrusted instructions.
- Reviewers see the exact action and material context, not only a plan summary.
- Nested agents, handoffs, retries, and alternate tool paths inherit the same controls.
- Unavailable or expired review fails closed.
- Approval, policy rationale, evidence, and execution result are recorded together.
- Contextual tests cover allowed, prohibited, ambiguous, malformed, adversarial, changed, unavailable-review, and nested-call cases.
- Policies and tests are revisited as tools, environments, and risks change.
Putting the pieces together
A dependable design treats approval as the final branch of a broader evaluation pipeline. The agent may suggest an action, but the harness constructs the authoritative proposal; the policy engine rules out what cannot be done; the risk and evidence checks identify what needs judgment; the human sees the exact remaining action; and the tool executes only the approved, unchanged proposal. Every branch produces a record.
