If a smoke alarm sounds or you suspect a fire, guide the person with Alzheimer’s out by the safest available route now. Give one calm, brief instruction, stay with them if you can do so safely, and go to your outdoor meeting place. Call 911 or your local fire department from outside and stay out. If you cannot reach them without entering smoke or flames, leave and tell dispatch exactly where they are. Do not go back inside or delay to collect medication or belongings.
What to do when the alarm sounds
- Give a short direction and start moving. Say something like, “There is a fire. Come with me outside.” Stay calm and avoid a lengthy explanation or argument; an emergency can be confusing and distressing for someone with dementia. [Alzheimer’s Association emergency guidance]
- Take the safest available exit. If smoke blocks the planned route, use another safe exit. If you must move through smoke, get low and go under it toward the exit. Close doors behind you only if doing so does not slow your escape. [USFA fire response guidance]
- Help the person directly outside. Do not assume they can interpret the alarm, find the exit, or use a phone without help. - If reaching them would put you in danger, get out. Call 911 from outside and tell the dispatcher that a person remains inside, giving their precise location. Do not enter smoke or flames to attempt a rescue. [USFA fire response guidance]
- If you are trapped, call and signal for help. If possible, close the door between you and the fire, cover vents and gaps, call the fire department, state your location, and signal at a window. Follow the dispatcher’s instructions. [USFA fire escape pictographs]
- Once outside, go to the agreed meeting place and stay out. Tell responders if anyone is still inside and where to find them. Leave rescue to firefighters; do not re-enter for a person, pet, medication, or possession. [USFA home escape guidance] [USFA emergency instructions]
The U.S. Fire Administration says residents could have less than two minutes to escape after a smoke alarm sounds. Treat this as a safety estimate that calls for immediate action, not a countdown or guarantee for a particular fire. [USFA home escape guidance]
How to prepare an Alzheimer’s-aware escape plan
Map exits and choose a meeting place
Draw a simple map showing doors and windows, identify two ways out of each room where feasible, and keep the routes clear. Choose a fixed meeting place outside. Practice the route with household members and adapt practice to the person’s abilities. [USFA home escape guidance]
Assign help and plan for mobility
Decide who will guide the person, including when the usual caregiver is away. Account for stairs, a walker or wheelchair, transfer needs, and what to do if the usual route is blocked. Assess whether the person can follow a brief direction, hear or see an alarm, and stay with a helper; abilities and safety needs can change as Alzheimer’s progresses. Make alarms and routes usable
Keep exits clear and easy for the caregiver to operate. The National Institute on Aging notes that someone with Alzheimer’s may not smell smoke or an unlit gas stove and may forget how to use a phone in an emergency, so do not rely on them to detect danger or call for help. USFA recommends smoke alarms inside and outside sleeping areas and on every level, with interconnected alarms so one sounding activates the others. Prepare essential information in advance
Keep a quick-grab bag accessible, but take it only if it is immediately at hand and does not delay leaving. The NIA suggests having a medication list, insurance cards, provider contacts, and advance directives available. Share copies of medical history, medication information, doctor details, and family contacts with someone besides the primary caregiver. Adapt the plan to your household
There is no single evacuation method that suits every home. Review the plan against the person’s current abilities and the building:

U.S. federal and Alzheimer’s Association guidance supports planning around mobility and special needs, but does not establish one universally safe way to lift or carry someone during a fire. Adapt the plan to the actual home and current care needs, and ask the local fire service for guidance where available. Emergency numbers and building evacuation procedures vary by location.
