If a hospital bill in India seems incorrect, write to the hospital’s grievance officer or management with the specific charges you dispute and ask for an itemised explanation and review. If the bill is unaffordable, make a separate written request to the billing or finance team for instalments or another arrangement. Keep copies of the bill, estimates, receipts, relevant medical records and every message. If the hospital does not resolve the matter, the National Consumer Helpline is a pre-litigation option; a Consumer Commission is a formal route. The right process can differ for PM-JAY beneficiaries, insurance claim disputes and state-specific patient-rights rules.
How do I dispute a hospital bill in India?
Start with the hospital and make a dated written complaint. The NHRC’s 2019 Charter of Patients’ Rights says patients and caregivers may give feedback, make comments or lodge complaints and seek redressal through a designated hospital official. The Charter’s implementation and grievance arrangements depend on the relevant State or Union Territory and the applicable rules; the Ministry of Health and Family Welfare has described health as a State subject.
1. Collect the records that explain the bill
Keep the final itemised invoice, any admission estimate or package terms, deposit and payment receipts, discharge papers, and prescriptions or test records relevant to the charges in question. Include insurer or third-party administrator (TPA) communications and messages about prices or authorisations if they relate to the dispute. If you need further documents to understand a charge, ask the hospital for them in writing. Keep originals and submit copies where possible.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errors2. Identify the exact charges and the remedy you want
Make a short list or table showing each disputed bill line, its amount, why it appears wrong or unclear, and what you want the hospital to do. For example, you might query a duplicated charge, a service you believe was not provided, an amount that differs from a written estimate, a package item billed separately, or a mismatch between an insurer-approved amount and the hospital’s demand. A confusing invoice alone does not establish fraud, so describe what you can document rather than making unsupported allegations.

Ask the hospital to explain each entry and identify the tariff, estimate, package terms or authorisation it used. If you can calculate the amounts accurately, distinguish the amount you accept from the amount under review. Request a complaint reference and the name or designation of the person handling the matter.
3. Submit a written complaint to the hospital
Use the hospital’s published complaint process and address the message to its grievance officer, patient-relations desk, administrator or management, as appropriate. Include the patient and admission identifiers in the private submission, admission and discharge dates, invoice number, disputed amount, a concise explanation and the outcome you are seeking. Attach copies of relevant records. Ask for a written response by a reasonable date, but do not assume there is one nationwide response deadline.

Keep proof that you sent the complaint and save the hospital’s reply. A clear subject line such as “Request for review of invoice [number]” can help the hospital route it; do not put sensitive patient identifiers in a public post or an unsecured message.
What to write
A useful complaint is specific and restrained: “I am requesting a review of the [date] invoice, number [number]. I query the [charge and amount] because [brief reason]. Please explain the basis for this charge, including the applicable tariff, estimate or package terms, and confirm whether the invoice will be corrected. The amount I understand to be under review is [amount]. Please send a written reply and complaint reference.” Attach only records relevant to the issues you raise.

Can I ask a hospital to reduce my bill or let me pay in instalments?
Yes. You can ask, but the official sources covered here do not establish a general nationwide legal entitlement to a discount, instalment plan or specific payment schedule. Treat a lower amount, deferred due date, fee waiver or settlement as a negotiation, not an assured outcome.
Make the request separately from the billing review
If you cannot pay the balance at once, contact the billing or finance team and propose an amount and schedule your household can realistically manage. Ask whether the hospital will accept a down payment followed by instalments, extend a due date, waive a fee or consider financial assistance. Keeping this request separate from the charge-by-charge complaint makes clear that affordability and whether a charge is correct are different questions.

Before agreeing, request written confirmation of the balance, each payment date and amount, any interest or other charges, what happens if a payment is missed, and whether the arrangement affects collection or records procedures. Keep the agreement and proof of every payment. Do not assume that paying part of a bill settles the disputed remainder or admits every charge; if you must pay while contesting part of the bill, seek local legal advice about the wording and consequences. Do not delay urgent care or withhold all payment based only on general guidance.
Where can I complain about a hospital bill?
The correct route depends on who made the decision and what kind of resolution you need. A hospital charge dispute, an insurer’s coverage decision and a grievance about PM-JAY treatment at an empanelled hospital are not the same complaint.

| Situation | Where to start | What the route is for |
|---|---|---|
| A hospital charge, service or estimate is disputed | Hospital grievance process; if unresolved, consider the National Consumer Helpline and then the appropriate Consumer Commission. | Request an explanation or correction first; a Commission is the formal consumer-redressal forum. |
| An insurer has denied or reduced a claim | Complain to the insurer first; if unresolved, use the insurer grievance route described by IRDAI. | Challenges the insurer’s claim decision, rather than the hospital’s billing process. |
| A PM-JAY beneficiary reports a specified irregularity or denial at an empanelled hospital | Use the PM-JAY grievance redressal mechanism (CGRMS) or helpline 14555. | For the specified beneficiary grievances; it is not a general channel for every hospital invoice. |
| Patient-rights or clinical-establishment rules may apply | Check the relevant State or UT health department and the local rules. | State adoption, implementation and grievance arrangements are not uniform nationwide. |
National Consumer Helpline: a pre-litigation step
The Department of Consumer Affairs describes the National Consumer Helpline (NCH) as a pre-litigation grievance mechanism. Its official information lists toll-free 1915 and online and other channels. Contact details can change, so confirm the current options on the official NCH service before submitting a grievance. NCH is not a Consumer Commission: its guidance says a consumer who remains dissatisfied may approach the appropriate Commission.
Consumer Commission: a formal consumer-redressal route
The Consumer Protection Act provides for District, State and National Consumer Commissions. Department of Consumer Affairs guidance describes available relief, including specific relief and, where appropriate, compensation, and online filing. It also describes jurisdiction connected to where a consumer resides or works. Which Commission and filing process apply depends on the current rules and facts of the case; do not assume that a particular outcome or refund is guaranteed.
Section 69 of the Consumer Protection Act, 2019, generally bars admission of a complaint unless it is filed within two years from the date the cause of action arose. A Commission may admit a complaint later if sufficient cause is shown and it records reasons for condoning the delay. Do not let extended discussions with a hospital cause you to miss a deadline; seek timely, case-specific advice if the period may be close or disputed.

PM-JAY and insurer complaints
For the specified grievances of beneficiaries involving irregularities or denial of treatment at an AB-PMJAY empanelled hospital, Government of India information lists CGRMS and 14555 as routes, with the helpline described as 24×7. This scheme channel does not apply to all patients or all billing concerns. If the problem is an insurer’s decision, IRDAI advises policyholders to complain to the insurer first and describes a separate policyholder grievance route. Keep the insurer complaint and decision distinct from any complaint about the hospital’s own charges.
What to do if the state rules or complaint route are unclear
The NHRC Charter is a useful statement of patient complaint and redressal principles, but it is not implemented identically in every State or UT. The Ministry of Health and Family Welfare has said that adoption, implementation, display and grievance mechanisms sit with the relevant State or UT. Check the local health department’s current process and whether the Clinical Establishments Act applies where the care was provided. A local framework may affect which authority handles a complaint; do not assume the same escalation route applies throughout India.
