There is no single price that tells you what CAR-T therapy will cost you. Your bill depends on the CAR-T product and indication, insurance benefits, treatment center and network status, prior authorization, separately billed services, and your plan’s cost-sharing rules. The best comparison is a written estimate for the full treatment episode, checked against written coverage and authorization details from your insurer.
This guide focuses on the United States. It explains how to compare coverage and estimates, what Medicare’s national policy does—and does not—establish, and where to ask about financial help. It cannot determine an individual patient’s benefits or final bill.
How much will CAR-T cost me?
The available official sources do not establish a comparable typical patient bill or national out-of-pocket range. A product’s list price or a hospital payment amount is not the same as what a particular patient owes. Your insurer’s benefit design and the services billed for your care determine your cost sharing.
Ask for an itemized estimate covering the treatment episode, then confirm the assumptions with both the insurer and the treatment center. Check whether the estimate is for the same product, diagnosis or indication, location, services, and benefit period you are comparing. Ask which amounts are estimates, excluded, or subject to later claims review.

What does CAR-T insurance cover?
Coverage can involve several benefits rather than one CAR-T line item. Ask the insurer and center to verify coverage for the exact product, your diagnosis and indication, and the specific treatment facility. Request written confirmation of network status, referral requirements, prior authorization, and the services covered.
Check each part of the treatment episode
- Product and administration: Ask how the CAR-T product and its administration are covered, and whether they are handled under different benefits.
- Hospital and professional care: Check facility charges, inpatient or outpatient services, physician charges, and whether the hospital and physician groups are both in network. Separate providers may send separate bills; for example, Dana-Farber notes that patients receiving care at both Dana-Farber and Brigham and Women’s receive bills from both hospitals (Dana-Farber’s financial and insurance planning guidance).
- Collection, testing, and follow-up: Ask whether collection, testing, pathology, radiology, supportive care, and follow-up visits are included or billed separately.
- Prescriptions and related care: Check pharmacy benefits, medications, transfusions, IV fluids, nursing care, and post-infusion monitoring.
- Your cost sharing: Confirm the deductible, copayments, coinsurance, out-of-pocket maximum, and which charges count toward that maximum. Ask whether any services are excluded or out of network.
The care sequence can extend beyond the infusion. Mayo Clinic describes collection, processing, chemotherapy, infusion, close monitoring, possible hospitalization, and ongoing visits. Use those steps as prompts when asking which costs appear in the estimate and which may be billed separately (Mayo Clinic’s CAR-T program overview).

How do I compare treatment centers and estimates?
Compare estimates only when their assumptions match. A lower figure may leave out services, use a different location or product, or assume different insurance benefits. Do not compare a manufacturer’s acquisition or list price with your expected out-of-pocket bill as though they measure the same thing.

- Identify the exact care being estimated. Record the product, diagnosis or indication, treatment center, and services included.
- Verify the plan details. Ask whether the plan is an HMO, POS, or PPO; whether the center, hospital, and physician groups are in network; and whether referrals or prior authorization are required.
- Request an itemized treatment-episode estimate. Ask the center to identify excluded items, estimates that could change, and services that may generate bills from other providers.
- Ask the insurer to confirm benefits in writing. Check the coverage of the product and each major service, as well as your cost sharing and which amounts count toward your out-of-pocket maximum.
- Compare like with like. Use the same product, location, services, benefit period, and coverage assumptions for each estimate.
Does Medicare cover CAR-T therapy?
CMS has a national coverage determination for qualifying autologous CAR-T. The policy applies to services on or after August 7, 2019, when treatment is administered at a health care facility enrolled in the FDA risk evaluation and mitigation strategy (REMS) and used for an FDA-approved indication or another medically accepted indication supported by a CMS-approved compendium. See CMS National Coverage Determination 110.24 for the policy’s conditions.
This national rule does not calculate an individual’s total cost sharing or promise that all related services will be paid without patient responsibility. Confirm that the specific facility, indication, and services meet applicable requirements, then ask the treating center and plan how Medicare benefits and any other coverage apply to your care.
How do I get prior authorization for CAR-T?
Ask whether authorization is required for the exact product, indication, facility, and services—not just for CAR-T generally. Mayo Clinic says most insurers require prior authorization and that its additional treatment steps cannot begin until approval is received (Mayo Clinic’s CAR-T program overview).

- Ask the treatment center who submits the authorization and what information the insurer needs.
- Request written confirmation of the decision, including what product, facility, and services it covers and any conditions or limits.
- Keep the authorization with your estimates and benefit records. If the insurer denies or limits coverage, ask the center’s financial counselor or navigator what review or appeal process applies.
- Tell the center promptly if your insurance changes during the process. Dana-Farber warns that a coverage change may cause delays and advises contacting its financial counselor.
What costs may fall outside the main treatment bill?
Ask about expenses that may not appear in a hospital or physician estimate. Dana-Farber recommends checking travel and lodging benefits and calls out meals, caregiver needs, supportive care, and medications after treatment. Depending on the care plan, ask whether transfusions, IV fluids, nursing, monitoring, and follow-up are included in the estimate or could be separately billed.

Can I get financial help for CAR-T travel or copays?
Start with the treatment center’s financial counselor, nurse navigator, or social worker. Ask for help understanding benefits, estimates, coverage reviews or appeals, and current assistance options. Dana-Farber specifically recommends asking a CAR-T nurse navigator or clinical social worker about manufacturer assistance.
- Ask the center what support is available. Find out whether it can help with benefit checks, estimates, paperwork, and referrals to assistance programs.
- Contact the manufacturer of the prescribed product. Ask about current patient access, reimbursement, copay, travel, lodging, or meal support, and request the eligibility rules and covered-expense details in writing.
- Ask about charitable or disease-specific funds. Have the center or social worker check whether a relevant fund is currently accepting applications and whether it covers your specific expense.
- Keep records and report changes. Save benefit checks, authorizations, estimates, bills, program decisions, and notices of insurance changes.
Example: BMS Cell Therapy 360
Cell Therapy 360 describes support for eligible patients prescribed BMS CAR-T. Its copay program is limited to the BMS product and excludes people enrolled in Medicare, Medicaid, TRICARE, VA, or other government health programs. The program does not cover other provider charges or treatment costs. Check the current financial support terms directly; eligibility and program details may change.
Example: Kite Konnect
Kite Konnect is a support resource for patients prescribed a Kite product. Ask the manufacturer and treatment center what help is currently available and whether your insurance type, product, and expenses qualify.
