For most people, the best choice is an automatic upper-arm blood pressure monitor with a cuff sized for their arm and a model validated for clinical accuracy. Wrist monitors are a fallback when an upper-arm cuff is medically unsuitable or no correctly sized one is available; they require careful positioning. The American Heart Association (AHA) generally does not recommend wrist or finger monitors because they are less reliable than upper-arm models. AHA home-monitoring guidance and the AMA’s validated-device listing explain what to check before buying.

What kind of home blood pressure monitor should you use?

Start with an automatic, cuff-style upper-arm monitor. The AHA recommends this type for home use and says wrist and finger devices are generally less reliable. That is a general preference, not a claim that every wrist model is unusable: the AMA says a wrist cuff may be appropriate when an upper-arm cuff is clinically inappropriate or a properly sized upper-arm cuff is unavailable.

There is no verified head-to-head figure here for how many millimeters of mercury or what percentage one design differs from the other. Choose based on validation, fit, and whether you can use the device correctly—not an assumed numerical accuracy gap.

How to compare upper-arm and wrist monitors

What to check Upper-arm monitor Wrist monitor
Usual role Preferred option for most people by AHA guidance. Conditional alternative when an upper-arm cuff cannot appropriately be used or obtained, according to AMA guidance.
Validation Check the exact model in a recognized clinical-accuracy listing. Check the exact model in a recognized clinical-accuracy listing; wrist design alone does not establish accuracy.
Fit and positioning Measure upper-arm circumference and match it to the monitor’s cuff range. Follow the device instructions closely and keep the wrist at heart level during measurement.
Practical suitability Use if the cuff fits and you can position it as directed. Consider with a healthcare professional if an upper-arm cuff is medically unsuitable or a correct-size cuff is unavailable.

Memory or transmission features can make recordkeeping easier, but they do not replace clinical validation or correct technique. If choosing between otherwise suitable devices, consider whether you can comfortably use the controls and keep a consistent record of readings.

How to choose a monitor step by step

  1. Measure your upper arm. Wrap a tape around the upper arm as directed by the monitor manufacturer, then compare that measurement with the cuff-size range printed for the device. The AHA warns that an incorrectly sized cuff can distort readings; a standard cuff that does not fit is not a suitable choice. See AHA guidance on choosing a monitor and cuff and the AMA’s home-measurement guidance.
  2. Check the exact model’s clinical validation. Search the model—not just the brand—in a recognized listing such as the US Blood Pressure Validated Device Listing. The AMA describes its listing as a resource for identifying devices validated for clinical accuracy. Do not treat a general marketing statement, FDA clearance claim, or smartwatch blood-pressure feature as proof that the exact device has independent clinical validation.
  3. Ask about special suitability if needed. If you cannot use an upper-arm cuff or cannot find one that fits, ask a healthcare professional whether a wrist model is suitable for you. This is especially prudent where the user’s needs differ from the ordinary adult home-monitoring case, including pregnancy, childhood, or other specific clinical circumstances.
  4. Bring the monitor to an appointment. Ask a healthcare professional to check your cuff fit and technique and compare your device with office equipment. The AHA advises bringing it about once a year or as directed by the manufacturer.

How to take a reliable home reading

Even a validated monitor can give misleading results if preparation or positioning is inconsistent. The AHA home-monitoring instructions and the CDC measurement guidance recommend this routine:

  • For 30 minutes beforehand, avoid smoking, caffeinated drinks, and exercise. Empty your bladder before measuring.
  • Sit quietly for at least five minutes. Support your back, keep both feet flat on the floor, and do not talk or use your phone during the rest or measurement.
  • Place the cuff on bare skin, not over clothing. For an upper-arm cuff, support the arm so the cuff is at heart level. For a wrist model, follow its specific positioning instructions and keep the wrist at heart level.
  • Take two readings one minute apart, ideally at consistent times each day. Record the readings and ask your healthcare professional how often you should measure.

For a wrist monitor, positioning is especially important: a wrist held above or below heart level can affect the reading. Use the posture and placement specified in that model’s instructions rather than improvising.

How to use readings in care decisions

Keep a log—on paper, in a device’s memory, or in another reliable record—and discuss the results with a healthcare professional. A single reading does not replace review of the pattern or clinical advice. Do not stop or change prescribed medication based on home readings without consulting a healthcare professional.

In a 2024 AHA report, Karen Margolis, a senior research investigator at HealthPartners Institute, said that about a dozen properly taken readings over one to two weeks on a validated device with the correct cuff may help determine whether someone has hypertension or needs a medication adjustment. That is an expert’s contextual estimate, not a universal diagnostic schedule; follow the plan given by your own clinician. AHA report, September 6, 2024.
