To check whether your medical records were exposed, look for a notice from your provider or health plan, search the U.S. Department of Health and Human Services (HHS) Office for Civil Rights (OCR) breach portal, and contact the organization directly if the result is unclear. The portal is not an individual lookup tool, and a missing listing does not prove your information was safe.
Check for a notice from your provider or health plan
Search your mail and email for a breach notice from a doctor’s office, hospital, insurer, pharmacy, or health-related service. If you use an online patient portal, check its messages as well. A notice may explain what happened, which types of information were involved, what the organization is doing, and what steps you can take.
For HIPAA-covered organizations, HHS says notices to affected individuals must be sent without unreasonable delay and no later than 60 days after the breach is discovered. That deadline is not a guarantee that you will hear about an incident immediately. See HHS breach notification requirements.
Verify a notice that seems suspicious
Do not rely on phone numbers or links in an unexpected message until you have checked them. Visit the provider’s or insurer’s official website, use its patient portal, or call the number on your insurance card. Ask whether the notice is genuine and whether your information was included.

Search the HHS OCR breach portal
HHS publishes reports of breaches of unsecured protected health information affecting 500 or more people. Search its Breach Portal using the organization’s legal name or a system or incident name mentioned in a notice. The listing can help establish whether a covered organization reported a large incident, but it does not tell you whether a specific person’s records were involved.
Why a missing result is not an all-clear
The public listing is not a complete, real-time checker for an individual’s exposure. Incidents affecting fewer than 500 people may be reported up to 60 days after the end of the calendar year in which the organization discovered the breach; organizations may also report sooner. As a result, a smaller incident may not appear promptly. HHS explains the reporting rules for large breaches and breaches affecting fewer than 500 people.

Contact the organization to confirm your exposure
If you received a notice, cannot find a listing, or are unsure whether an incident affects you, contact the provider or health plan through a channel listed on its official website, patient portal, or insurance card. Ask:
- Was my account or record included in this incident?
- What categories of information about me were involved?
- What steps do you recommend I take, and is any assistance available?
- How can I report suspicious claims or activity connected to my account?
The organization is the party that can confirm whether your particular records were involved. The public portal cannot verify individual inclusion.

Check consumer health apps separately
HIPAA does not cover every service that stores health information. Some personal health record vendors and connected services outside HIPAA may instead be subject to the Federal Trade Commission’s Health Breach Notification Rule. A search of the HHS portal alone may therefore miss an incident involving an app or service. Check that company’s official notices and support channels, and see the FTC’s Health Breach Notification Rule for information about the separate rule.

If you find care or insurance activity you did not authorize, contact your health plan and the provider shown on the record using verified contact details. Ask them to investigate the claim or record and explain how to dispute it. The FTC directs people whose information was used to obtain care or use insurance benefits to IdentityTheft.gov for a recovery plan.
