Short answer: you cannot confirm an AI highlight from the colored mark alone. It is a model-generated cue showing where an algorithm predicted or attended to something. A qualified clinician must compare that cue with the original image series, the radiology report, the patient and study details, and the clinical question. Ask the clinician interpreting the scan to identify the relevant slice and explain whether the highlighted area is confirmed on the source images.
A screenshot can be useful for discussion, but it may be only a frozen annotated image. An interactive result that links the annotation to the original DICOM series, location, measurements, or a structured report is more inspectable; it still does not prove that the model is right.
What an AI highlight actually represents
Most medical-imaging AI systems produce an output for a defined task: for example, flagging a possible nodule, outlining a region, estimating a measurement, or assigning a probability to a finding. A colored overlay, box, contour, or saliency map is the visual presentation of that output. It is not an independent observation made by a radiologist and it is not, by itself, a diagnosis.
The mark can therefore be useful as a pointer without being proof. The system may highlight normal anatomy, an imaging artifact, a postoperative change, or a real abnormality. Whether it corresponds to a clinically meaningful finding depends on the exact model, the image series it analyzed, image quality, and the clinician’s interpretation.

Color and shape are not universal codes
Red, yellow, green, blue, a thick outline, a translucent wash, and a confidence number do not have a universal meaning. One product may use red for a suspected lesion; another may use it for the area receiving the most algorithmic attention. Unless the documentation for that particular system defines the display, do not infer that a color means confirmed cancer, severity, urgency, or certainty.
Why the original scan and report matter
The meaningful comparison is between the AI output and the corresponding original images, not between the overlay and your expectations about what a scan should look like. A clinician needs to know which patient, examination, body part, acquisition series, and slice the result belongs to. The report then supplies the interpretation, measurements, comparison with prior studies, and clinical context that an isolated mark lacks.

Radiology viewers can display annotations beside the DICOM images from which they were generated. The quality of that display and the ability to inspect the full series affect how useful the result is. Static secondary capture versus an interactive result
A common reason for confusion is that the same AI output can be delivered in very different forms. A static image may show exactly where the software drew an outline, but it can omit the source series and the machine-readable details behind it.

The distinction comes from the RSNA standards primer. A secondary capture is effectively a static annotated picture, while structured reports and segmentations preserve information that can be displayed and integrated more usefully. If all you received is a screenshot, ask whether the underlying study and structured result are available to the interpreting clinician.
How a clinician checks whether the highlight matches
Clinical review is not a single visual glance. It is a sequence of identity, image, and context checks. The exact workflow differs by modality and institution, but the logic is consistent.

The clinician verifies the patient, examination date, body region, modality, accession or study identifier, and the series used by the AI. A correct-looking overlay on the wrong series is still a wrong result.
- Open the original image series. The reviewer locates the slice or three-dimensional region corresponding to the annotation and scrolls through neighboring images. This helps distinguish a persistent structure from a one-slice artifact.
- Review image quality and acquisition details. Motion, metal, low dose, reconstruction method, contrast timing, field of view, and incomplete coverage can change what the algorithm and human reader can see. The reviewer checks whether the study is technically adequate for the task.
- Compare the marked anatomy with the suspected finding. The contour, box, or point is checked against the actual structure. The reader decides whether it represents a lesion, an expected anatomical feature, an artifact, or an unrelated change.
- Check measurements and prior examinations. If the system supplies size, volume, density, or another measurement, the clinician verifies that it is attached to the correct object and considers prior images when available. A measurement is not meaningful if the boundary or series is wrong.
- Read the AI output in the context of the report and clinical question. The final interpretation incorporates symptoms, history, examination indication, prior findings, and other sequences. The report records what is confirmed, uncertain, or not clinically significant.
- Resolve disagreement explicitly. If the overlay and the source images do not agree, the radiologist may disregard the mark, document an artifact or false positive, or recommend another review or examination when clinically appropriate. The AI display does not overrule the professional interpretation.
Questions to ask at your appointment
You do not need to interpret the pixels yourself. These questions help connect the visual cue to the clinical record:

- Which image or slice shows the area the AI marked?
- Does the highlighted area correspond to a finding in the radiology report?
- Is the area confirmed on the original images, or is it an AI suggestion that was not confirmed?
- Which patient, study, and image series did the software analyze?
- Did image quality, motion, metal, contrast timing, or another technical issue affect the result?
- If a size or other measurement is shown, was it checked against the source images?
- Does the finding need comparison with an earlier scan or any follow-up?
These questions are appropriate whether the highlight appears on a patient portal, a printed page, or a screen shared during a consultation. The clinician can access the complete study and report; a patient-facing image often cannot.
What evidence supports an AI system’s accuracy?
There is no single correctness percentage that applies to every AI scan highlight. Performance depends on the task, model, imaging protocol, patient population, and deployment setting. A number reported for one system cannot be transferred to a different scanner, body part, or clinical workflow.
The CLAIM 2024 reporting checklist encourages authors to describe the data source, study goal, image-acquisition protocol, and reference standard. It distinguishes internal testing on held-out data from external testing on an entirely separate dataset and discourages using the word validation without explaining what was actually tested. Those details describe the evidence behind a model’s published performance; they do not validate an individual patient’s overlay.
How to read common performance claims

Why an apparently wrong highlight can occur
A mismatch does not necessarily mean that the software is defective. Several failure points can occur before a clinician even evaluates the mark.

Wrong patient, study, or series
Radiology examinations contain multiple series and reconstructions. A model may be intended for one series but receive another, or a displayed overlay may be associated with a different study than the one the patient is viewing. Identity and series checks are therefore the first safety step.
Different scanners and protocols
Changes in equipment, reconstruction kernels, slice thickness, contrast timing, dose, positioning, or field of view can alter image appearance. A multi-society statement from the ACR, CAR, ESR, RANZCR, and RSNA emphasizes matching the patient, study, and series to the AI tool and monitoring how performance changes as data, equipment, protocols, or populations shift: Developing, Purchasing, Implementing and Monitoring AI Tools in Radiology.
Artifacts and normal structures
Motion, metal, noise, partial-volume effects, overlapping anatomy, and normal variants can resemble the patterns a model was trained to detect. The algorithm may flag them even when a radiologist recognizes that no actionable abnormality is present.
False negatives
An unmarked abnormality can still be present. AI highlighting is not a guarantee that unhighlighted areas are normal, which is why the clinician reviews the complete examination rather than only the colored regions.

Display and image-quality problems
Annotations can be misregistered, clipped, hard to see, or shown at an unsuitable scale. Product documentation for the FDA-cleared OmegaAI Image Viewer describes overlaying annotations and measurements on DICOM and AI/CAD images and states that professionals must ensure viewing conditions and image quality are appropriate for the clinical purpose. Treat the screenshot as a conversation aid, not as a complete diagnostic record. Do not enlarge it, compare it with internet images, or use a consumer anatomy tool to decide what the mark means. A frozen image may omit the neighboring slices, window and level settings, measurements, patient identifiers, and report linkage needed for a safe interpretation.
Contact the clinician or imaging department that issued the result and ask for an explanation using the original study. If the screenshot came from a portal, ask whether the portal can provide the formal radiology report and whether the interpreting clinician can review the annotated series. Keep the image with the correct patient and date information, but avoid posting identifiable scans publicly.

What patients can and cannot verify themselves

How professional governance reduces risk
AI performance is monitored as part of a clinical service, not just at the moment a model is installed. Selection, implementation, updating, and continuous quality improvement all affect whether an output remains useful. On May 5, 2026, the American College of Radiology announced approval of its first practice parameter for imaging AI, covering those areas of professional governance: ACR practice-parameter announcement.
For a patient, the practical implication is simple: the safest check is a governed clinical workflow in which the right study is sent to the intended system, the result is linked back to the source images, and a qualified professional interprets it. A screenshot outside that workflow cannot reproduce those safeguards.

- Write down the examination date, body region, and the name of the clinician or department that supplied the image.
- Obtain the formal radiology report if you do not already have it.
- Ask whether the image is a static secondary capture or an interactive result linked to the original series.
- Request the corresponding slice or series and ask whether the marked area is confirmed on the source images.
- Ask what the report says about the finding, its measurement, comparison with prior studies, and recommended follow-up.
- Do not delay care or change treatment based on the overlay. Follow the interpreting clinician’s instructions, and seek urgent medical help for symptoms that your healthcare team has identified as emergencies.
