To audit an AI agent across tools, connect every step of a run with a stable identifier, record both the request and its outcome at each tool boundary, and keep the authorization decision with the action. Then monitor operational and security signals against a baseline. Crucially, a trace records what an instrumented system observed; it does not by itself prevent an agent from taking an unauthorized action. Enforce permissions and approvals separately, where actions execute.
What should an AI agent audit trail capture?
Build the trail around a run or conversation ID that connects the initiating user or trigger to the agent, model, retrieval, tool, and subagent steps. For each tool interaction, record a request before execution and an outcome afterward. The request event shows what the agent attempted; the outcome event shows whether it was allowed, denied, failed, or completed.
| Event or field | What to record |
|---|---|
| Run context | A stable run or conversation ID; initiating user or trigger; agent identity; model or system version where available; timestamp. |
| Tool request | Tool name and identity; action; target; arguments or a privacy-safe representation; permission context; risk classification; authorization decision; approval ID and policy version where applicable. |
| Tool outcome | Execution identifier linking it to the request; timestamp; success, denial, or failure status; result or a minimized representation; error details where relevant. |
| Other context boundaries | Retrieval source and provenance, memory reads or writes that materially affect the task, and agent-to-agent or MCP interactions when those boundaries exist. |
Keep failed and denied attempts as well as successful actions. Those records can reveal policy problems or unusual behavior that a success-only log would miss. Microsoft’s guidance calls for execution details such as tool names, arguments, permissions, and outputs. OWASP’s event model distinguishes a pre-execution toolCallRequest from a post-execution toolCallResult, linked by an execution identifier. See Microsoft’s observability guidance and the OWASP supported-events specification.
Do not treat the table as a mandate to retain every prompt, argument, or result verbatim. Choose the detail needed for debugging, security investigations, or audit, and make that choice explicit in your data contract.

How do you trace tool calls across a run?
- Create a shared correlation ID. Assign a stable run or conversation ID when work starts, then propagate it through model, retrieval, tool, and subagent instrumentation. Give each individual operation its own span or execution ID so events within a run can be ordered and linked.
- Emit a request event at each execution boundary. Record the actor, tool, target, action, relevant arguments or their safe representation, and the permission or approval context before the tool runs.
- Emit the outcome against the same operation. Capture completion, denial, or failure, along with the timestamp and an appropriately minimized result or error. Preserve the connection to the original request even when execution fails.
- Instrument context and handoffs. Add retrieval provenance when a knowledge source contributes context. Record material memory reads and writes, and capture agent-to-agent or MCP protocol events if your system uses them. OWASP’s event specification describes these as observable boundaries.
- Check coverage end to end. Verify that the relevant tools and downstream services emit events and that the event pipeline is operational. A trace is only as complete as the paths instrumented.
OpenTelemetry-compatible tracing gives teams a common telemetry layer. OWASP’s Agent Observability Standard extends OpenTelemetry and OCSF concepts with agent-specific observability. Its event definitions can help teams name and connect agent actions consistently; they do not require indiscriminate collection of sensitive payloads. See the OWASP Agent Observability Standard overview.
How should consequential tool actions be authorized?
Separate the agent’s proposal from permission to execute it. A tool call produced by an agent is not evidence that the actor is authorized. Put an authorization check at the execution component or policy service, and require approval according to the action’s risk. The check should apply to the actual operation, not merely to a broad session-level permission.
For destructive, financial, administrative, or externally visible actions, bind approval to the exact actor, tool, target, normalized parameters, time, and expiry. Use short-lived authorization artifacts and replay protection for irreversible operations; make actions idempotent where possible. Unknown or unmapped actions should not silently be treated as low risk. Record the risk classification, authorization result, approval identifier, execution result, and policy version alongside the action.

- HARDWARE PLUS SECURITY SERVICES: FortiGate-60F Firewall Appliance bundled with 1 year of FortiCare Premium and FortiGuard Unified Threat Protection.
- UNIFIED THREAT PROTECTION (UTP): Secures against advanced online threats with comprehensive web filtering and anti-botnet technologies.

Fail closed if risk classification, approval validation, policy lookup, or audit logging fails. OWASP’s AI Agent Security Cheat Sheet specifically calls for these controls and says the execution component must still check authorization and required approval even when an action has been classified.
What should you monitor and alert on?
Combine service health with signals about policy and agent behavior. Track a baseline for your own system, then alert on meaningful deviations rather than treating sample thresholds from documentation as universal defaults.
- Service health: latency, errors, request volume, tool-call volume, token use, and tool failures or outcomes.
- Authorization and security: privilege elevation, unusual invocation frequency, repeated approval bypass attempts, approval drift, and changes in the frequency of high-risk actions.
- Agent quality: evaluation results for safety, groundedness, quality, and tool-use correctness where your system supports those evaluations.
- Telemetry health: missing events, broken links between request and outcome, or interruptions in the audit pipeline.
Use alerts to prompt investigation or control action, not as a substitute for a policy gate. OWASP includes threshold examples in sample code, but those are illustrative implementation material, not validated cross-system benchmarks. Microsoft’s observability guidance covers operational metrics such as latency, errors, token usage, and volume; OWASP’s event model provides examples of action outcomes.

How do you protect sensitive telemetry?
Tool arguments, results, retrieved content, memory, prompts, and messages between agents can contain sensitive information. Define a data contract that states what is collected, why it is needed, who can access it, and how long it is retained. Apply minimization, access control, encryption, and applicable residency and retention requirements.
Where full payloads are unnecessary, consider recording identifiers, hashes, or redacted representations while retaining enough evidence for the intended investigation. Balance forensic usefulness against privacy and exposure risk; do not assume that logging more content automatically makes the system safer. Microsoft’s guidance discusses privacy, data residency, minimization, retention, and legal obligations in observability design.
A trace can show what the instrumented path recorded, but it cannot prove that every possible action was constrained. Pair audit records with independent execution-time authorization, protect the event pipeline, and test that relevant tools and downstream services are covered. The cited guidance supports failing closed when audit logging fails, but does not establish a universal tamper-evidence standard or retention duration; define those requirements for your environment.

How do you choose an agent observability approach?
Evaluate whether an approach captures model, tool, retrieval, and subagent spans; records both requests and outcomes; exports or queries standard telemetry; integrates with identity, policy, SIEM, and compliance workflows; and provides appropriate privacy, location, and retention controls. Also account for instrumentation effort and operating cost.
| Documented example | What its documentation describes | What to verify for your deployment |
|---|---|---|
| OpenAI Agents API tracing | Agent, generation, and tool spans, with optional OTLP JSON trace export. | Configuration, permissions, and whether the trace data and span coverage meet your retention, privacy, and downstream workflow requirements. See OpenAI tracing documentation. |
| Amazon OpenSearch Service AI observability | Hierarchical agent traces, GenAI semantic conventions, and OpenTelemetry integration. | Current service availability and configuration, along with data handling, integration, and operating requirements for your environment. See Amazon OpenSearch documentation. |
These are examples of documented capabilities, not a performance comparison or endorsement. A vendor-described deployment can also illustrate what an organization chooses to collect: OpenAI’s May 8, 2026 article about its Codex deployment says telemetry can include prompts, tool approval decisions, execution results, MCP server usage, and network proxy allow-or-deny events, with logs centralized in SIEM and compliance systems. That is one implementation example, not a requirement for every agent system; see Running Codex safely at OpenAI.
