Add pomegranate arils to meals as one fruit within an overall heart-healthy eating pattern. Pomegranate is not a required heart-health food, and current evidence does not establish a pomegranate-specific serving target. The strongest practical guidance is to focus on the whole eating pattern—not to treat pomegranate as a way to prevent or manage heart disease.
How to include pomegranate in meals
Use fresh pomegranate arils as a fruit ingredient alongside other nutritious foods. For example, scatter them over oatmeal, plain yogurt, a grain bowl, or a salad. These are simple ways to add fruit; they are not meal plans tested in cardiovascular trials.
Whole fruit is a straightforward choice when your goal is to include fruit with a meal. Pomegranate juice is another option, but treat it as a beverage rather than a treatment. If choosing packaged juice, check the product label for added sugars. The evidence does not show that juice or supplements are better choices than arils.

Build the rest of the diet around heart-healthy foods
The American Heart Association recommends a nutrition plan built around fruits, vegetables, whole grains, legumes, nuts, fish and seafood, and poultry, while advising people to limit sugary drinks and full-fat dairy. The USDA describes the DASH eating pattern similarly: it includes fruits and vegetables, whole grains, poultry, fish, nuts, seeds, and legumes, while reducing saturated fat, sweets, and sugary beverages. The USDA review reports that the DASH trial lowered systolic blood pressure by 5.3 mm Hg compared with a control diet and by 2.8 mm Hg compared with a fruit-and-vegetable diet. Those figures concern overall dietary patterns, not pomegranate.
What research says about pomegranate and blood pressure
Evidence from pomegranate studies is suggestive but inconsistent, and it does not establish that pomegranate prevents cardiovascular disease or reliably lowers an individual’s blood pressure. The National Center for Complementary and Integrative Health (NCCIH) says, “Pomegranate juice or extract may be helpful in reducing blood pressure, but additional research is needed to confirm this effect.” NCCIH also reports that studies have not demonstrated clear effects on cholesterol or other blood lipids. - A 2023 review of 14 clinical trials involving 573 participants reported a pooled systolic blood-pressure change of −5.02 mmHg with pomegranate juice (95% confidence interval −7.55 to −2.48). The review said the apparent benefit was lost after two months of intake. These pooled results are not an individual dose or a guarantee of benefit. See Ghaemi et al. (2023).
- A 2017 review of eight trials involving 619 participants found inconsistent reporting and results, and concluded that the evidence did not convincingly show a blood-pressure benefit. See Gbinigie et al. (2017).
- A 2024 review of 22 randomized trials reported pooled reductions in systolic and diastolic blood pressure, but found high variation between studies (I² 90.0% and 91.8%, respectively) and called for further high-quality studies. See the 2024 systematic review and meta-analysis.
The different review findings, including substantial variation across studies, make it inappropriate to promise a personal blood-pressure effect or long-term cardiovascular benefit from eating pomegranate.
Safety and when to seek advice
NCCIH says pomegranate juice is believed to be safe, though some people may have digestive symptoms and allergic reactions have been reported. It cautions that large amounts of pomegranate root, stem, or peel may not be safe. If you take medicines and are considering herbal products such as pomegranate extracts, discuss them with a health care provider. NCCIH does not identify a specific interaction between ordinary pomegranate food and a named medicine.

Do not use pomegranate in place of prescribed blood-pressure medicine or other medical care. The American Heart Association notes that health professionals may prescribe medication to manage blood pressure and other cardiovascular risk conditions.
