To add a custom banner to a Google Form, open the form, select Customize theme, then choose Header → Choose image. After selecting an image source, you can replace the image or reposition its crop from the header controls.
Add a header image

Open the form in Google Forms.

At the top right, select Customize theme.

In the Theme panel, find Header and select Choose image.

Choose an available source and follow the prompts to insert or save the image. Google’s help documents built-in themes and image selection through Upload, Photos, Google Drive, Google Images, or By URL. Which options appear may vary. See Google’s instructions for changing a form’s theme.

Preview the form to check how the banner appears above the questions.
Replace or reposition the image
To change the selected image, click the header and choose Replace Header Image. To adjust its framing, click the header, choose Reposition Header Image, drag the crop box corners, and save. Check the preview after cropping so important text, logos, or faces remain visible. Google documents the crop control but does not specify an official safe area.
What size should the header image be?
Google’s help page does not state a required pixel size or file format for a Forms header image. A Google Docs Editors Community answer from Greg Gambone on August 20, 2019, recommends 1600 × 400 pixels and notes that “the bottom center of the header image is obscured by the top of the question area.” This is unofficial, older advice—not a Google requirement or a guarantee of how the image will render today. Treat it only as a possible starting canvas, then use the crop controls and preview. The community discussion is at Size of header image in Google Forms.

The current Google help page reviewed here also does not establish an exhaustive list of supported upload formats. Avoid relying on older community reports as current format rules.

These are sources documented in Google’s help; their availability may vary. The documentation does not compare their image quality or guarantee that an image selected by URL will remain accessible.

If a theme or image option is missing
If you expected to see an organization-branded theme, check with your Workspace administrator. Google’s Learning Center says these themes appear only when an administrator makes them available. See Create your first form in Google Forms.
If an image is present but framed poorly, select the header and use Reposition Header Image to adjust the crop. Preview the form to confirm the important parts remain visible.
