Paired ion electrospray ionization mass spectrometry (PIESI-MS) is a laboratory method designed to make selected drug metabolites easier to detect. It pairs negatively charged molecules with a positively charged reagent so they can be measured in positive-ion mode. Studies published in 2014 and 2015 reported substantial sensitivity gains for the compounds they tested, but those findings do not establish that PIESI-MS is a routine or regulator-approved anti-doping test.
What is the “super sensitive test”?
The test is PIESI-MS, short for paired ion electrospray ionization mass spectrometry. Mass spectrometry identifies and measures molecules by analyzing their ions. In this method, an ion-pairing reagent combines with a negatively charged analyte, allowing the resulting complex to be measured in positive-ion mode.
The approach targets a technical challenge: some drug metabolites are anions, and the research sought to improve their detection by using a mode that may provide a stronger signal for the studied compounds. The larger analyte–reagent complex can also shift the signal to a region with less background noise, according to Chemistry World’s 2014 report. The method’s performance depends in part on the reagent’s choice and structure, as described in the 2014 Analytical Chemistry paper.
What did the studies test?
A 2015 study applied high-performance liquid chromatography with PIESI-MS (HPLC-PIESI-MS) to selected glucuronide and sulfate conjugates of performance-enhancing drugs and drugs of abuse. These conjugates are forms of drug metabolites. The researchers reported chromatographic separation of structurally similar steroid conjugates and used off-line solid-phase extraction to prepare human urine samples, with the aim of addressing matrix effects—the influence of other substances in a sample on the measurement. The work is described in the 2015 International Journal of Mass Spectrometry study.

This was an analytical method applied to selected compounds and samples, not a demonstration of one test that detects every banned drug. Results for a particular analyte do not automatically apply to other substances, reagents, instruments, or urine samples.
How much more sensitive was it?
The reported gains vary because the sources describe different studies, compounds, and comparisons. They should not be combined into a single claim about all doping tests.

A low analytical detection limit describes performance under specified laboratory conditions; it does not by itself establish how long a drug can be detected after use. Daniel W. Armstrong of the University of Texas at Arlington told Chemistry World in 2014, “If the method is more sensitive, you have a longer time window to detect these substances.” That is a general implication of greater sensitivity, not a measured detection window for every drug.

Does this mean anti-doping labs use it?
Not on the evidence in the cited sources. The 2014 news report and research papers from 2014 and 2015 establish the method’s mechanism, its application to selected metabolites, and historical laboratory results. They do not establish current routine use by anti-doping laboratories, accreditation, or regulator approval for anti-doping decisions. A promising laboratory method and a low detection limit are not, on their own, proof of operational or regulatory adoption.

The 2014 report quoted Chengdong Xu, then a student in Armstrong’s lab and part of the team developing PIESI: “It’s a simple technique, but it has the potential to be extremely useful.” That comment describes the method’s potential at the time, not evidence of later deployment.

A sensitivity figure is meaningful only alongside the conditions that produced it. For PIESI-MS and other analytical approaches, the useful comparison points are:

- Comparator: the exact alternative method against which sensitivity or detection limits were measured.
For the 2015 work, the key comparison was with negative-ion-mode methods for the tested metabolites, using a urine workflow that included sample preparation. That scope is narrower—and more informative—than treating a headline sensitivity figure as a guarantee for any drug test.
