NFL players recovering from a surgically repaired Achilles tendon generally have to rebuild from protected movement to calf strength, impact tolerance, running and football-specific work—but public evidence does not establish one standard NFL team protocol. Progression depends on symptoms, strength and movement quality, and returning to a game is not the same as restoring preinjury performance. In one 2017 NFL cohort, the average time to return to NFL play was about 340 days; that is a group average, not an individual timetable.

What Achilles rehab is trying to restore

The Achilles tendon connects the calf muscles to the heel and helps produce the force used to push off, accelerate and jump. After a rupture and repair, rehabilitation has to restore that force while protecting the healing tendon. The practical challenge is not simply being able to walk: a player must build calf capacity and tolerate progressively faster, more forceful movements.

Published clinical guidance describes staged rehabilitation, but it should not be mistaken for a published NFL-wide plan. The 2024 review describes approaches to rehabilitation after Achilles repair, while the American Orthopaedic Society for Sports Medicine (AOSSM) gives general patient guidance on protection and recovery. Neither establishes the specific care plan used by every NFL team or player. Marrone et al., 2024 review; AOSSM Achilles tendon guidance, reviewed November 2025.

How recovery progresses from protection to running

The phases below describe broad clinical patterns, not a schedule to follow without a treating clinician. The calendar ranges are reference points from the cited guidance; they do not guarantee readiness at a particular date.

Phase What the phase is for Timing and evidence
Protection and controlled movement Protect the repair while beginning appropriate movement and gradually restoring function. AOSSM says a boot or brace may be used for about 6 to 8 weeks after repair or during non-surgical treatment, followed by gradual recovery of motion, strength and function. The surgeon’s protocol determines the details. AOSSM
Progressive calf and leg strength Rebuild push-off capacity through progressively challenging calf work, alongside balance, movement control and strength elsewhere in the leg. The 2024 review describes progressive loading and functional milestones rather than one universal NFL timetable. In its described approach, passive calf stretching is generally avoided for up to 12 weeks after surgery; this is not a rule for every protocol. 2024 review
Impact preparation and running Build tolerance for impact and running once strength, symptoms and movement quality support progression. The 2024 review says its authors typically begin running no sooner than 12–16 weeks after surgery, with timing varying by patient and protocol. Its late rehabilitation phase is described as 12–24 weeks; those phase labels are clinical recommendations, not an NFL schedule. 2024 review
Return-to-sport progression Increase training demands toward the speed, power and repeated effort required by the athlete’s sport. The 2024 review places its return-to-sport phase at 24 weeks and beyond and emphasizes progressive training volume and strength and power criteria. AOSSM says return to athletic activity typically occurs after six months in its general guidance. Neither timeframe is equivalent to clearance for an NFL game. 2024 review; AOSSM

Protection is a phase, not a complete plan

A boot or brace helps protect the tendon during early recovery, but does not determine when a person should move, load the calf or stop using it. Those decisions belong to the treating surgeon and rehabilitation team. The 2024 review discusses early protected mobilization and progressive loading while cautioning against premature stress on the tendon. Individual instructions can differ.

Strength work has to lead to controlled function

Rebuilding plantar-flexor capacity—the muscles’ ability to point the foot down and push off—is central to progressing toward running. The review discusses double- and single-leg heel raises, balance, proximal leg strength and movement mechanics. A single-leg heel raise is a useful milestone, but it is not by itself proof that someone is ready to run or play.

For field and court athletes, the review authors describe potential running prerequisites that include seated isometric plantar-flexion strength of 1.5–2.0 times bodyweight, ten full-range single-leg heel rises and good single-leg squat control. These are clinical recommendations in that review, not validated universal clearance thresholds or NFL rules. A clinician interprets tests in context.

Running readiness is more than a date

A modified Delphi consensus study published online in July 2026 identified seven criteria experts agreed to include when considering return to running after surgical repair: no pain in daily life, no pain during or after rehabilitation sessions, walking without a limp, walking on tiptoes, ten single-leg heel rises, good single-leg balance and psychological readiness. Experts excluded calf-circumference symmetry as a criterion. The authors say the proposed criteria require prospective validation before they can be used as decision rules; they are not a validated checklist that independently clears an athlete. • Premium Quality: Enjoy unmatched convenience and versatility with this medical-grade walking boot; The durable, lightweight German-designed walking boot for Achilles tendon injury ensures comfort and mobility for every step of your recovery journey

After running begins, the review describes progression toward low-amplitude impact, plyometrics and sport-specific work as strength and mechanics allow. It also notes that ideal return-to-sport testing remains poorly established; clinicians may consider plantar-flexor strength, heel-rise work, jumping and movement quality together rather than relying on a single test. 2024 review.

What NFL return-to-play studies actually show

Two NFL studies report different return rates because they examined different groups and used different definitions. Neither percentage is a universal forecast for a player recovering today.

Sylong® Achilles Tendon Rupture Night Splint, Post-Op Achilles Tendon Brace for Surgery Recovery, with 3 Plantar Flexion Angles 40°/30°/20° Metal Brackets, Sleeping Only, Not for Walking, Right, Large
  • 【Specifically for night sleep use after Achilles tendon rupture surgery recovery】 This Achilles tendon rupture night splint is designed to help patients maintain the foot in a plantar flexed position during rest and sleep following surgery, thereby promoting healing of the Achilles tendon. Compared to bulky walking boots, it offers a lighter, more comfortable alternative. The built-in metal brackets also provide superior support and stability compared to standard plastic splints.
  • 【3 Metal brackets for Flexible adaptation】The biggest difference of this splint is that it is equipped with 3 metal brackets, offering plantarflexion angles of about 40°, 30°, and 20°. Unlike splints with a single fixed angle, this splint allows you to select and interchange the brackets based on the specific nature and stage of the Achilles tendon rupture (consult your physician is recommended). This enables more flexible adaptation.
  • 【Adjustable Fixation Strap】There are 3 Velcro straps for the calf, ankle, and foot, all of which are adjustable. In addition, we have additionally provided 2 lengths of calf straps as spares, so whether your leg is too thick or too thin, you can choose a suitable length.
  • Long-Term Wear】Achilles tendon rupture requires long-term wearing of a splint after surgery (especially during nighttime sleep), so wearing comfort is very important. This splint lining is made of a specially selected soft and skin-friendly material, which can reduce the need to frequently remove the splint due to skin discomfort. It can be worn with peace of mind while sitting at rest and during nighttime sleep.
  • 【2 Sizes, Left/Right Specific】This splint is suitable for use within 3 months after Achilles tendon rupture surgery. 2 sizes are available: S/M and L/XL, with left and right foot differentiation. A illustrated user manual is included with the product, containing a reference for metal bracket angle selection.
Study Reported return outcome What the result means—and does not mean
Jack et al., 2017; retrospective comparative series 71 of 95 players returned to NFL sport (72.4%); the 95 players had 98 surgeries. Mean time to return was 339.8 ± 84.8 days after surgery. This is the cohort’s return-to-sport result and average, not an individual prediction. Matched controls had a longer NFL career than repair patients (3.6 ± 2.1 versus 2.7 ± 2.1 years). Performance scores were significantly worse after surgery for running backs and linebackers compared with their own preoperative scores. These are associations in a retrospective study, not proof that surgery caused career or performance differences. Study record
2021 study of NFL skill-position players 57% returned to play, defined as playing at least one game after repair. The reported position-specific rates ranged from 38% for wide receivers to 71% for tight ends. This cohort covered the 2009–10 through 2015–16 seasons and was limited to skill positions. The authors also found performance decreases in some positions among players who returned. Its population and return definition differ from the broader 2017 study, so the rates should not be averaged or treated as competing estimates of one identical outcome. Study record

Why “returned” does not necessarily mean “back to normal”

In an outcome study, returning may mean appearing in a game; that alone does not show that a player regained preinjury speed, workload, roster role, career length or movement capacity. The 2017 study’s reported career and performance findings illustrate why those are separate outcomes, while the 2021 study reported position-specific performance decreases among some returning players. A successful return and full restoration of prior performance are not interchangeable claims.

General guidance that athletic activity typically resumes after six months also uses a broader endpoint than return to NFL play. The 2017 cohort’s mean of about 340 days refers specifically to return to NFL sport in that study. Those figures describe different activities and populations, not a contradiction or a timetable for a particular athlete. AOSSM guidance; Jack et al., 2017.

Treatment and rehabilitation decisions are individual

Rehabilitation protocols vary, and a public source does not establish one standardized NFL Achilles pathway. Whether a complete rupture is treated surgically or non-surgically is a separate decision from how recovery is progressed. AOSSM says the patient and treating physician should consider age, activity level and other medical issues together. It does not say surgery is mandatory for every elite athlete. AOSSM Achilles tendon guidance.

For an NFL player, the treating clinicians can adapt loading and progression to the individual injury, recovery and football demands. Published cohort outcomes can describe what happened to groups of players, but they cannot determine an individual’s treatment, running start date or return-to-play decision. This overview is general information, not a personal rehabilitation prescription.
