Mismatch-repair (MMR) and microsatellite-instability (MSI) tests help oncologists find a subset of colorectal cancers that may respond to immune-checkpoint drugs. A tumor described as dMMR (deficient mismatch repair) or MSI-H (microsatellite instability-high) has a DNA-repair problem or the molecular pattern associated with that problem. In advanced colorectal cancer, that information can move immunotherapy into the treatment discussion, but it does not guarantee a response, prescribe one drug by itself, or prove that the cancer is inherited.
The evidence and U.S. approvals summarized here concern colorectal cancer. “Bowel cancer” is used for the reader-facing question, while the treatment data refer specifically to advanced or metastatic colorectal cancer and to the dates and settings described by the National Cancer Institute (NCI).
What MMR and MSI mean
Mismatch repair is the cell’s proofreading system
When a cell copies its DNA, occasional copying errors occur. Mismatch-repair proteins identify and correct certain errors so that the cell’s genetic instructions remain stable. A tumor is called dMMR when this repair function is deficient. A tumor with functioning repair is often described as pMMR, for proficient mismatch repair.

Without effective repair, errors can accumulate in short, repeated stretches of DNA called microsatellites. A tumor with a high level of variation in those regions is reported as MSI-H. “MSI” describes a measurable DNA pattern; “MMR” describes the repair machinery. They are closely related findings, not two names for one identical test.

What MSI-high means
MSI-H means that testing found high microsatellite instability in the tumor sample. It is a tumor biomarker, not a stage, grade, or statement about how long someone has had cancer. Because the accumulated DNA changes can make tumor cells more visible to the immune system, MSI-H/dMMR status can identify a group for whom immune-checkpoint treatment may be considered, especially in advanced disease.
How the tests are performed
Both approaches generally use tissue from the tumor, but they look for different evidence. NCI describes molecular genetic testing that examines the lengths or patterns of microsatellite regions and immunohistochemistry (IHC) that examines whether particular MMR proteins are present in tumor cells.

A pathology report may contain one test, both tests, or a comment recommending additional work. The exact assay, specimen quality, and laboratory interpretation matter. A result that is technically difficult, incomplete, or inconsistent with the clinical picture should be reviewed rather than treated as an automatic treatment order.

For advanced colorectal cancer, the biomarker can identify people who may be candidates for immune-checkpoint inhibitors. Checkpoint drugs release some of the immune system’s brakes; they do not work for every tumor, and the decision also depends on disease extent, prior therapy, overall health, possible toxicities, patient preferences, and the regulatory and guideline status where treatment is being delivered.

NCI’s current Colon Cancer Treatment PDQ reports that pembrolizumab received U.S. Food and Drug Administration approval in 2020 for treatment-naive metastatic colorectal cancer with dMMR/MSI-H tumors. The same PDQ summarizes the phase III KEYNOTE-177 trial, in which the pembrolizumab group had a median progression-free survival of 16.5 months, compared with 8.2 months with chemotherapy. Objective response rates were 43.8% and 33.3%, respectively.
Those are outcomes for the trial population, not a forecast for an individual. “Median” means half the people in that study had a longer time without documented progression and half had a shorter time; it does not promise that a person receiving pembrolizumab will remain progression-free for 16.5 months.
U.S. immunotherapy options described by the evidence
The sources describe two initial-treatment immunotherapy approaches for some people with advanced MSI-H/dMMR colorectal cancer. The appropriate choice is individualized; neither option is automatically best for every patient.

The CheckMate-8HW figures compare study groups and should not be converted into an individual probability. The NCI report described the combination as a preferred first-line option for some patients, not as a universal recommendation. Approval details can change, and coverage outside the United States follows local regulators and health systems.

How clinicians use the result in practice
The result is one part of a sequence rather than a stand-alone prescription.
- Confirm the cancer and clinical setting. The team establishes that the diagnosis is colorectal cancer and records whether it is localized, recurrent, advanced, or metastatic. The treatment evidence cited here concerns advanced or metastatic disease, particularly initial treatment.
- Review the tumor biomarker report. The oncologist and pathologist check whether the report says MSI-H, another MSI category, dMMR, retained MMR proteins, or an uninterpretable or equivocal result. They also check which method was used and whether the sample was adequate.
- Check for agreement and clinical fit. MSI and MMR findings usually point in the same direction, but they can be discordant. The team considers the full pathology, imaging, previous treatments, other molecular findings, health conditions, and the person’s goals.
- Discuss eligible immunotherapy choices. If the tumor is confirmed MSI-H/dMMR and the disease setting matches an approval or guideline, pembrolizumab or, where appropriate, nivolumab plus ipilimumab may enter the discussion. This is where the team explains expected benefits from relevant trials, possible immune toxicities, monitoring, alternatives, and local access.
- Separate tumor testing from inherited-risk evaluation. The same result may prompt a discussion of Lynch syndrome and genetic counseling, but a tumor result alone does not diagnose an inherited condition.
What happens when MSI and MMR results do not match?
A mismatch can occur because the two tests measure different features, because a specimen contains limited or mixed tumor tissue, or because one result is difficult to interpret. The existence of a discordant report does not establish which test is “wrong” without reviewing the laboratory details.
NCI’s 2025 report on CheckMate-8HW said that local hospital test results were sometimes incorrect when compared with central expert confirmation. The experts cited in that report encouraged clinicians to follow guidance calling for two testing methodologies and referral to an expert diagnostic center in cases of uncertainty.

An oncology team may request a second method, pathology review, or expert-center consultation based on the actual report and specimen. The evidence does not establish one mandatory confirmatory workflow or a single false-result rate.

Questions to ask about an unclear report
- Was the result obtained by MSI molecular testing, MMR IHC, or both?
- Which MMR proteins were lost or retained, and was the staining interpretable?
- Does the laboratory recommend repeating or confirming the result with another method?
- Should the specimen be reviewed by a specialist diagnostic center?
- Would a confirmed MSI-H/dMMR result change the treatment options for this stage of disease?
Does MSI-high mean Lynch syndrome?
No. MSI-H/dMMR is a finding in the tumor. Lynch syndrome is an inherited cancer-predisposition condition caused by a germline change affecting mismatch-repair genes, including MLH1, MSH2, MSH6, or PMS2. A tumor can acquire the MSI-H pattern through a sporadic mechanism, including gene silencing caused by DNA methylation, without a germline mutation.
NCI and SEER describe MSI-H tumors in people with Lynch-associated cancers and in a smaller portion of colon cancers not associated with Lynch syndrome. Therefore, the correct interpretation is that an MSI-H/dMMR result may trigger hereditary-risk assessment; it does not prove that the patient or relatives carry an inherited mutation.
What follow-up may involve
The oncology or genetics team may review the family history, the patient’s age and tumor features, the specific MMR protein pattern, and any additional tumor or germline testing that is appropriate. Germline testing examines DNA that is inherited and present throughout the body; it is a separate consideration from testing the tumor. Genetic counseling can help explain what a result could mean for relatives, screening, and future cancer-risk decisions.

- Prevalence: The same NCI report estimates that about 5% of people with advanced colorectal cancer have MSI-H/dMMR tumors. That is an NCI-reported estimate for that setting, not a universal percentage for every stage, population, or bowel-cancer definition.
Study results compare groups under trial protocols. They do not account for every person’s tumor burden, other illnesses, prior treatments, access to care, or tolerance of therapy. Ask the oncology team how closely the trial population and treatment setting match the situation being treated.
Balancing single-agent and combination immunotherapy
The most useful comparison is not simply the drug names. It is the fit between the confirmed biomarker, the disease setting, the evidence, and the person’s priorities.
- Expected benefit in the relevant setting: The team uses the trial that matches the patient’s line of treatment and tumor features rather than applying a number from a different setting.
- Toxicity and monitoring: NCI specifically notes increased side-effect concerns with nivolumab plus ipilimumab. The clinician explains what symptoms require urgent contact and how monitoring will be organized.
- Comorbidities and other medicines: Existing health conditions and concurrent treatments can affect whether an immune-based approach is suitable and how risks are managed.
- Patient preferences: Some people may prioritize a simpler single-agent approach; others may consider a combination when the expected benefit and risks are acceptable. The decision belongs with the patient and oncology team.
- Regulatory and local access: The approvals described here are U.S.-specific. Local licensing, guidelines, payer rules, and availability may differ elsewhere.
Limitations of MSI and MMR testing
A biomarker result is only as reliable as the specimen and interpretation behind it. Tumor tissue may be limited, staining may be difficult to read, and local testing can disagree with expert review. Because MSI and MMR assess related but different evidence, a borderline or conflicting report deserves context rather than a reflexive conclusion.

It can indicate whether a tumor belongs to a group in which checkpoint inhibitors may work; it cannot by itself determine the cancer’s stage, predict an individual response, select a dose, identify every safe treatment, or establish Lynch syndrome. Those decisions require the rest of the pathology, imaging, medical history, and—when appropriate—germline evaluation.
Practical appointment checklist
Bring the pathology and molecular reports, and ask the team to explain the result in plain language. The following checklist keeps the discussion focused:
- What exactly does my report say: MSI-H, another MSI category, dMMR, or a result that cannot be interpreted?
- Which test produced the finding, and was a second method used?
- Is my disease stage and treatment line one in which an approved or guideline-supported checkpoint option is considered?
- What are the potential benefits and immune-related risks of each option for my situation?
- How would side effects be monitored, and whom should I contact urgently?
- Does the tumor pattern warrant genetic counseling or germline testing for possible Lynch syndrome?
- If the tests disagree, should the tissue be reviewed or tested at an expert diagnostic center?
The practical takeaway
MMR and MSI tests guide immunotherapy by identifying a biologically distinct subgroup of colorectal tumors. Confirmed MSI-H/dMMR status can support a discussion of checkpoint inhibitors in advanced disease, including pembrolizumab and, for some patients under the U.S. approval reported in May 2025, nivolumab plus ipilimumab. The result is neither a guarantee of benefit nor proof of inherited Lynch syndrome. When results are unclear, using two testing methodologies and seeking expert diagnostic review can help the oncology team make a safer, better-supported decision.
