The insertion itself is usually very brief: Planned Parenthood says the placement process generally takes less than five minutes, while the NHS allows about 5–10 minutes for fitting and at least 30 minutes for the entire appointment. Recovery has no universal deadline. Many people return to ordinary activities when they feel comfortable; others need some rest because of cramps, backache, spotting or temporary dizziness.

Insertion time and appointment time are different

When someone asks how long an IUD takes, they may mean the moment the device is placed or the complete clinic visit. Those are not the same clock.

What is being timed? Estimate Source and qualification
Placement process Usually less than five minutes Planned Parenthood patient guidance; this describes the insertion process, not necessarily check-in, counseling or recovery time.
IUD fitting About 5–10 minutes NHS guidance, page last reviewed February 15, 2024.
Whole appointment Allow at least 30 minutes NHS guidance; the visit can include questions, examination, testing, fitting and time to feel steady afterward.

These figures are compatible because they cover different parts of the visit. A clinic may take longer if it reviews your medical history, performs a pregnancy or sexually transmitted infection test, discusses pain-control choices, or needs additional time for an examination. Scheduling, consent and post-procedure observation can also affect the total.

What happens during the visit

Before insertion

The clinician usually asks about your health, menstrual and pregnancy history, medicines, allergies and contraception goals. Planned Parenthood describes questions, an examination and insertion as parts of the process. The NHS says a pregnancy test or STI test may be offered before fitting, depending on your circumstances and local practice.

Use this time to say if you have had difficult pelvic examinations, cervical procedures or severe menstrual pain. Tell the clinician about medicines and conditions that could affect which pain-relief options are safe. You can ask what type of IUD is planned, when it becomes effective and what symptoms should prompt a call.

During fitting

The NHS describes insertion with a speculum to view the cervix, followed by passing the IUD through the cervix into the womb. Cramping or pressure can occur. You can describe pain or discomfort and ask the clinician to pause or stop at any point; consent is ongoing throughout the procedure.

After placement

The clinician trims the device strings and may explain how and when to check them. You may sit or lie down briefly if you feel light-headed. A short observation period is especially sensible if you feel dizzy, because Planned Parenthood notes that dizziness can occur during or immediately after insertion and that fainting is possible, although uncommon.

When can you resume normal activities?

There is no evidence-backed universal number of hours that applies to everyone. Planned Parenthood says many people feel fine right away, while others want to take it easy for a while. The practical rule is to resume ordinary activities as your symptoms allow, while following any instructions from your own clinician.

Period-like cramps may last a few days, according to the NHS, and light bleeding or spotting can occur. These symptoms can make work, exercise or travel less comfortable even when no complication is present. If you feel well, there is generally no need to remain in bed solely because an IUD was inserted; if you feel unwell, scale back and rest.

Work or school

Some people return the same day, including to a desk-based job or classes. Others prefer a lighter schedule or the rest of the day off because of cramps, backache or dizziness. If your work involves lifting, prolonged standing, operating machinery or driving, judge those tasks by how steady and comfortable you feel rather than by a fixed deadline.

Driving and getting home

Ask the clinic whether you are fit to drive before leaving. If you feel dizzy, faint or significantly uncomfortable, do not drive until you have recovered and can control the vehicle safely. Planned Parenthood suggests considering a companion so you do not have to travel home alone if dizziness is a concern. A companion is an option, not a requirement for every patient.

Exercise and lifting

Return to walking, routine exercise and heavier activity gradually according to comfort. Cramping or bleeding that increases when you move is a reason to stop, rest and contact your clinician for advice if it does not settle. Your insertion clinician’s specific restrictions take precedence over general patient guidance, particularly if the fitting was difficult or you have another medical condition.

Sex and vaginal activity

Physical comfort and contraceptive protection are separate questions. You may choose to wait for comfort, even when the IUD is already effective. Follow the backup-contraception timing for your device and insertion date, described below, rather than assuming that feeling well means pregnancy protection is immediate.

Expected short-term effects

Cramping and backache

Cramping can feel similar to period pain. The NHS says period-like pain may continue for a few days. Planned Parenthood also mentions backache. Rest, gentle movement or a heating pad may make symptoms easier to manage; a heating pad is an optional comfort measure and does not speed the IUD’s medical recovery.

Spotting or light bleeding

Light bleeding or spotting can occur after fitting. Track whether bleeding is easing or becoming heavier, and use the instructions supplied by your clinic for pads, tampons or menstrual cups. Very heavy bleeding is not something to dismiss as routine; seek prompt advice using the warning-sign guidance below.

Dizziness or fainting

Some people feel dizzy during or shortly after insertion. Sit or lie down until the feeling passes, rise slowly and ask staff for help. Fainting is possible but uncommon. If dizziness persists, returns repeatedly or is accompanied by other concerning symptoms, contact your clinician or local urgent-care service.

Pain relief: discuss options rather than expecting one guaranteed solution

ACOG’s May 15, 2025 guidance says IUD insertion and other in-office uterine or cervical procedures can be painful or uncomfortable. It recommends counseling patients about available options and using shared decision-making. Kimberly Hoover, MD, FACOG, a coauthor of that guidance, said: “Comprehensive counseling is key because patients must be able to decide for themselves what interventions, if any, they would like to try.”

Tell the clinician what you are worried about and ask which options are appropriate for you. The right choice depends on your health, allergies, medicines, prior experiences and the clinic’s resources.

Local anesthetic and other techniques

CDC guidance on intrauterine contraception reports that a paracervical lidocaine block might reduce pain, but trial results vary. Topical lidocaine may reduce pain in some settings, while other studies did not find a reduction. These findings do not establish a single best technique or promise pain elimination.

Misoprostol

The CDC says evidence does not support routine misoprostol for improving pain, placement success or satisfaction in patients without a recent failed placement attempt. Do not take it or any other prescription medicine for insertion unless your clinician specifically recommends it.

Over-the-counter medicine

Planned Parenthood mentions over-the-counter pain medicine and a heating pad for cramps. The NHS lists paracetamol or ibuprofen as options. Check with a clinician or pharmacist if you have allergies, stomach or kidney problems, take blood thinners, are pregnant or could be pregnant, or have another reason a medicine may be unsuitable. Follow the product label and the advice given at your appointment.

When does the IUD start preventing pregnancy?

Contraceptive effectiveness is not the same as physical recovery. A person can feel comfortable before the device’s backup-contraception period ends, or feel crampy even though protection has already begun.

Device and timing Protection guidance
Copper IUD ACOG says protection is immediate.
Hormonal IUD (also called an IUS by the NHS) inserted more than seven days after the period began ACOG recommends avoiding vaginal sex or using a backup method for the next seven days. CDC guidance gives the same seven-day threshold for levonorgestrel IUDs.

The exact instruction can depend on the device, cycle timing and clinical circumstances. Confirm it with the clinician who inserted your IUD. Do not use your comfort level as a substitute for that contraceptive advice.

Warning signs that need prompt medical advice

Most post-insertion discomfort is temporary, but contact your own clinician or local urgent-care service promptly if you have symptoms that concern you. NHS guidance for hormonal IUS users identifies these warning signs:

  • Severe or worsening lower-abdominal pain, especially when painkillers do not help.
  • Fever.
  • Unusual or bad-smelling vaginal discharge.
  • Very heavy vaginal bleeding.
  • Possible pregnancy.
  • Strings that cannot be felt or that feel different from before.

These symptoms do not prove a particular complication, and the absence of symptoms cannot confirm that everything is positioned correctly. Follow the written instructions from your clinic, including what to do if you cannot feel the strings. Seek emergency help for severe illness, collapse or symptoms that feel immediately life-threatening.

A practical plan for the day of insertion

  1. Allow enough time. Reserve at least 30 minutes for the appointment even though fitting itself may take only 5–10 minutes.
  2. Arrange flexibility afterward. Keep the rest of the day lighter if possible, especially if you have previously felt faint with pelvic procedures.
  3. Ask about pain options before the procedure starts. Discuss local anesthetic, what the clinic can offer and which medicines are safe for you.
  4. Plan transport based on how you feel. You may drive only if you are alert and steady; consider a companion if dizziness worries you.
  5. Keep comfort supplies available. A heating pad and an appropriate over-the-counter medicine may help cramps, but neither is mandatory.
  6. Write down the protection date. If a hormonal IUD was placed more than seven days after your period began, note the seven-day backup window unless your clinician gives different instructions.
  7. Save the clinic’s contact details. You should know whom to call about pain, bleeding, strings or other symptoms before you leave.

How to decide whether an activity is reasonable

Use a symptom-based check rather than an internet countdown. Before driving, exercising, working a physical shift or having sex, ask whether you can move normally, remain upright without dizziness and manage the discomfort without pushing through worsening symptoms. If the answer is no, postpone the activity and rest. If symptoms are severe, worsening or accompanied by fever, unusual discharge, very heavy bleeding, possible pregnancy or a change in strings, contact a clinician instead of trying to work around them.

Terminology and individual instructions

“IUD” is the common term in the United States and many other places. The NHS commonly calls a hormonal IUD an “IUS” or hormonal coil and uses “IUD” for a copper coil. The devices and timing rules discussed above therefore need to be matched to the product you received. A clinician’s instructions for your specific device, examination and medical history override general patient-education guidance.

The NHS fitting information was last reviewed February 15, 2024. ACOG’s pain-management release is dated May 15, 2025, and CDC guidance is clinical information rather than a promise that every person will have the same experience. Together, these sources support a short procedure, a potentially longer appointment and a flexible return to activities guided by symptoms.
