Most routine vaccine reactions are brief: fever or feeling unwell commonly improves within one to two days, while soreness, redness, or swelling where the needle went in often lasts two to three days. COVID-19 vaccine symptoms typically begin one to three days after vaccination and resolve within one to three days. Some vaccines have a delayed pattern—for example, fever after MMRV can begin six to ten days later in some children. Severe, worsening, or unusual symptoms need medical advice rather than watchful waiting.
Typical duration by symptom
A short-lived reaction is part of the normal immune response for many people. The U.S. Centers for Disease Control and Prevention (CDC) describes most vaccine reactions as minor and resolving within a few days. General NHS guidance gives more specific expectations, although individual responses and vaccines differ.
| Reaction | Common timing or duration | How to interpret it |
|---|---|---|
| Fever or feeling unwell | Often one to two days | A general NHS estimate; a different vaccine, age, or unrelated infection can change the pattern. |
| Sore, red, or swollen injection site | Often two to three days | Local discomfort can outlast a fever. England guidance for children up to age five gives the same two-to-three-day expectation. |
| COVID-19 vaccine symptoms | Usually start one to three days after vaccination and resolve in one to three days | CDC says frequency and severity vary by age, manufacturer, and dose. |
| Fever after MMRV in some children | May start six to ten days after immunisation | The measles component can produce a delayed fever; England guidance says about one in 10 children may be affected. |
These figures describe common patterns, not a personal diagnosis. A symptom that happens after a shot may be coincidental—for example, an infection that began around the same time. Timing alone does not prove that a vaccine caused a health problem, as the CDC explains in its guidance on preventing and managing adverse reactions.
What fever after vaccination usually means
Fever can occur as the immune system responds to vaccination. A temperature may be accompanied by tiredness, headache, chills, reduced appetite, or a generally unsettled feeling. For most routine vaccines, these systemic symptoms ease over the next day or two.

A fever is not a test of whether the vaccine worked. England’s MenB guidance states that the level of fever depends on the individual child and “does not indicate how well the vaccine has worked.” Conversely, having no fever does not mean the dose failed. Protection is assessed from the vaccine schedule and immune response, not from whether a person felt ill afterward.
Why the timing differs between vaccines
Routine injected vaccines
Many inactivated or protein-based vaccines cause soreness soon after the injection and, when they cause systemic symptoms, those symptoms generally appear during the first couple of days. The local reaction can remain after the fever or fatigue has gone.
COVID-19 vaccines
According to the CDC’s COVID-19 vaccine safety guidance, most reported symptoms begin one to three days after a dose and resolve one to three days after they start. The page notes that frequency and severity can vary according to age, manufacturer, and dose, so another person’s experience is not a reliable prediction of yours.

Measles, mumps, rubella, and varicella (MMRV) vaccination has a notable delayed pattern in some children. The England Department of Health and Social Care guide says the measles component may cause fever six to 10 days after immunisation, with about one in 10 children developing it. A fever at that point can still fit an expected vaccine reaction, but the child’s overall condition matters and a later illness is also possible.

MenB given with infant vaccines
England’s guidance highlights fever as common when meningococcal B (MenB) vaccine is given alongside routine vaccines at two and three months. Follow the post-vaccination instructions supplied by the child’s clinician, because advice can differ by schedule and country.
How to monitor a fever safely
Use a thermometer rather than judging temperature by touch. The England guide for babies and children up to age five specifically advises caregivers to check a child’s temperature with one. Record the reading, the time, and any medicine given; that information helps a clinician decide whether the course is typical.
- Check the person’s general state. Alertness, breathing, hydration, and ability to feed or drink are as important as the number on the thermometer.
- Offer fluids. Babies may need more frequent feeds; older children and adults can take small, regular drinks.
- Keep clothing comfortable. Avoid heavy bundling during a fever, while also preventing chills.
- Follow local medicine advice. If you are considering a fever reducer or pain reliever, use the product directions and advice from a pharmacist or clinician for the person’s age, weight, health conditions, and vaccine. Do not give a child a medicine that is unsuitable for their age.
- Recheck if symptoms persist or worsen. A reaction that is not improving within the usual window deserves a call to a healthcare professional, even if the initial temperature was modest.
When a post-vaccination reaction needs urgent help
Most reactions are mild, but do not wait for a stated duration to expire if someone looks seriously ill. Seek urgent medical help for severe or rapidly worsening symptoms, breathing difficulty, collapse, a widespread allergic reaction, confusion, dehydration, or a seizure.
Allergic reactions
The NHS notes that serious allergic reactions usually happen within minutes of vaccination and that vaccinators are trained to treat them. Warning signs can include trouble breathing, swelling of the face or throat, widespread hives, faintness, or sudden weakness. Call emergency services immediately if these occur.

Child-specific fever advice in England
The UK Department of Health and Social Care’s “What to expect after vaccinations” guidance applies to England and to babies and young children up to age five. It says to seek urgent medical advice if a child has a fit and to call a doctor immediately for a temperature of 39–40°C or above. Those thresholds are guidance-specific, not universal rules for every country, age group, vaccine, or clinical situation; elsewhere, use local medical advice.
Symptoms that are persistent or atypical
Contact a clinician when fever lasts beyond the expected period, returns after settling, or is accompanied by a new focal symptom such as a severe headache, stiff neck, persistent vomiting, or a concerning rash. A sore arm that becomes increasingly painful, hot, or draining also warrants assessment rather than assuming it is a routine injection-site reaction.
How to tell a vaccine reaction from another illness
Vaccination does not prevent every infection that might occur in the following days. A cough, runny nose, stomach symptoms, or exposure to someone who is ill may point to another cause. The CDC cautions that reports made after vaccination do not establish causation simply because they followed a dose. Clinicians consider the vaccine, timing, symptoms, exposures, medical history, and examination together.

Delayed onset deserves the same balanced approach. MMRV-related fever can occur six to 10 days later, but not every fever in that window is caused by the vaccine. If the person is unusually drowsy, difficult to wake, breathing abnormally, not drinking, or deteriorating, seek care promptly regardless of the presumed cause.

What to expect from local reactions
Pain, redness, warmth, or swelling at the injection site commonly peaks soon after the dose and fades over two to three days. Gentle movement of the limb can help prevent stiffness. Avoid pressing, massaging, or applying unapproved products to the site. If swelling is spreading quickly, pain is severe, or the area remains markedly inflamed after the usual period, ask a healthcare professional to check it.
Local and systemic reactions do not necessarily occur together. Someone can have a sore arm without fever, or a fever without much local discomfort. Neither combination predicts how much protection the vaccine provides.
Differences between guidance sources
The CDC’s broad statements cover vaccines licensed in the United States and are based on Vaccine Information Statements derived from Advisory Committee on Immunization Practices recommendations. NHS estimates are general UK guidance. The child fever thresholds and MMRV figures cited above come from England-specific guidance updated 24 December 2025. Vaccine schedules, age cutoffs, and escalation advice can differ in Scotland, Wales, Northern Ireland, the United States, and other countries.
Use the information sheet given at the appointment as the first reference for the exact product and dose. If it conflicts with a general online estimate, contact the vaccinating clinic, pharmacist, or local health service for advice tailored to that vaccine and person.

A practical decision checklist
- First 48 hours: Mild fever, fatigue, headache, or a sore arm commonly fits the expected pattern for many vaccines.
- Days two to three: Local soreness or swelling may still be present even as systemic symptoms improve.
- Days six to 10 after MMRV: A fever can be a recognized delayed reaction in some children, but assess the child’s overall health and seek advice for concerning symptoms.
- Any time: Emergency signs, a seizure, severe deterioration, or an allergic reaction require immediate help.
- Outside the expected course: Persistent, recurrent, or worsening symptoms should be discussed with a clinician rather than attributed automatically to the vaccine.
Questions people commonly ask
Is a fever after a vaccine normal?
A mild, short-lived fever is a recognized reaction to several vaccines. The usual duration is about one to two days, but the vaccine, age, and individual response matter. A high or worsening fever needs medical advice.
Can side effects start a week later?
Usually symptoms begin sooner, but MMRV is an important exception: England guidance describes fever beginning six to 10 days after immunisation in about one in 10 children. A later fever should still be assessed in context because unrelated illness is possible.
Does feeling sick prove the vaccine worked?
No. Fever and other reactions neither confirm nor measure protection, and no reaction does not mean the vaccine failed.
