A passing test suite, clean static-analysis report, and successful build do not prove that software matches its written requirements. In one developer’s account, line-by-line comparison with a specification exposed a game-board bug that automated checks had not caught. The account describes three bugs, but the available details establish the mechanism of only the first; the other two should not be guessed at.
What the reported bug actually did
The developer’s first example involved a game board that was supposed to show five stocks each round. A filter intended for one board mode also removed industries already owned by the player in another mode. As ownership accumulated, the board shrank instead of continuing to show five stocks.
The subtle failure was not simply that a condition was wrong. The test suite had treated the shrinking board as expected behavior because its expectation followed the existing implementation rather than the written specification. In the author’s account, reading the code against that specification exposed the mismatch.
The reported change selected from every industry with an available stock and visually disabled stocks that were already owned, rather than removing them from the board. The author says they checked 5,000 generated game states across five rule sets and found zero violations. That is the author’s reported result, not an independently verified benchmark.

The surfaced account says three bugs were found this way, but does not provide enough detail to establish what the second and third bugs were or how they were fixed. They cannot responsibly be used as additional examples of particular failure mechanisms.
Why a green check can still mean the wrong thing
Tests demonstrate that the software behaves as the assertions specify for the cases they exercise. They do not independently establish that those assertions represent the product requirement. When tests are derived from buggy behavior, they can make a defect look intentional—as the game-board example illustrates.

Other automated methods inspect different evidence and catch different classes of problems. Google Cloud describes a workflow that layers unit and integration tests, fuzzing, static analysis, dynamic analysis, and human review. That is one company’s example, not a universal ranking or guarantee.

NIST’s 2023 summary of its SATE VI security-focused evaluation says static-analysis results varied by bug class, complexity, and test case, with simpler bugs generally easier for tools to find. NIST recommends evaluating a tool on the intended codebase rather than assuming a general ranking. Those findings concern that evaluation; they should not be generalized to every language, tool, or ordinary application defect. NIST’s SATE VI summary provides the scope.

What “review against the specification” adds
The useful distinction is not “humans versus automation.” It is what each check is asked to verify. Tests can confirm behavior against expected results; analyzers can flag patterns; runtime methods can reveal failures under execution. A reviewer can also ask whether the code’s behavior makes sense for the stated requirement and all relevant modes or states.
- What does the written requirement say, in observable terms?
- Does the change implement it for each relevant mode, state, and boundary?
- Were the tests derived from the requirement independently, or do they reproduce behavior already present in the implementation?
- Which inputs, dependencies, data assumptions, or execution paths have not been exercised?
- Would an apparently reasonable test pass even if the implementation contradicted the requirement?
These are practical prompts inferred from the reported case and documented workflows, not a validated checklist from a controlled study. Their value depends on the reviewer having access to clear requirements and enough system context to interpret the change.

Why line-by-line review is not a safety net
Review can add evidence, but it does not reliably prevent escaped defects. Mozilla’s 2018 overview of research into reviewed Firefox code reports that crash-related defects could elude reviewers and reach users; the examined crash-prone code often had high complexity and many class dependencies. The overview identifies memory and semantic errors among major root causes in the studied patches. This supports extra scrutiny for complex, interconnected code—not a promise that review will catch every issue. Mozilla’s research overview describes that work.
A 2015 paper record from Microsoft Research similarly summarizes a concern: code reviews often miss functionality issues that should block submission, and reviewer skills and social context matter. A line-by-line pass can still miss a flaw if the reviewer misunderstands the requirement, lacks context, or accepts a misleading test. Escaped security flaws are warnings, not examples of caught bugs
Two familiar security incidents are sometimes invoked to argue for inspection, but neither demonstrates a pre-release line-by-line review catching a defect. The Software Engineering Institute says Apple’s 2014 “goto fail” flaw involved a duplicate line that bypassed credential validation and that review should have identified it. Its discussion of Heartbleed describes an unchecked payload length that enabled an over-read of server memory. Both were escaped flaws, not bugs shown to have been caught before release by review. SEI’s discussion covers the incidents and its recommendations.
The same SEI discussion cites an estimate of approximately 500,000 secure web servers believed vulnerable at the time Heartbleed was disclosed, circa 2014. That is a historical estimate, not a current count.
A practical way to layer checks on a change
- Start with the requirement. Identify the intended outcome and the modes, states, and boundaries it covers before treating existing behavior as correct.
- Write or verify tests from that requirement. Check that the expected result is grounded in the specification, not merely copied from what the current code does.
- Run complementary automated checks. Use tests, analysis, and runtime checks for the failure modes and surfaces they actually cover; treat a clean report as evidence within that scope.
- Review the change in context. Trace relevant branches and data flow, then compare actual behavior with the requirement. Pay particular attention to complex code and dependencies.
- Use failures to improve the evidence. When a defect is found, add a requirement-based regression test or other check that would distinguish the correct behavior from the mistaken one.
Google Cloud’s documentation describes a layered workflow in which human review follows automated presubmit checks. It is useful as an implementation example, not independent proof that any fixed sequence is sufficient. Google Cloud’s test-automation guidance explains its approach.

A 2026 arXiv preprint by Domenico Cotroneo, Giuseppe De Rosa, Cristina Improta, and Benedetta Gaia Varriale says the authors mined more than 14,000 defects across C/C++ and Java systems. They report that post-release defects were concentrated in older, frequently modified, high-churn components. Because this is a preprint based on open-source defect histories, it is preliminary evidence: it does not show that churn alone causes escaped defects or that every older component is riskier. The preprint record states its scope.
The defensible lesson is narrower than “line-by-line review catches what automation misses.” In the game-board account, comparing implementation with the written requirement reportedly exposed a mismatch that tests had encoded as correct. Other research shows both that tools vary in what they detect and that review itself can fail. A reliable quality process treats these methods as complementary evidence, with each judged against the behavior and risks it can actually examine.
