Hospital wastewater is managed through a combination of controlling hazardous inputs, collecting and containing wastewater, treatment, and disposal under local requirements. There is no single treatment process that fits every hospital: wastewater varies by department, and the appropriate approach depends on whether the facility connects to a central sewer, what is in its wastewater, and what it can reliably operate and maintain.
What makes hospital wastewater different?
Hospital wastewater is not one uniform stream. Waste from general medical areas may be similar to domestic wastewater, while particular activities can add pharmaceuticals, chemicals, disinfectants, or other specialized contaminants. The World Health Organization (WHO) describes higher concentrations of antibiotics, cytotoxic substances, metabolites, X-ray contrast media, and disinfectants in some ward urine. Kitchen wastewater can contain food residues, detergents, disinfectants, grease, and oils. These substances are possible concerns, not a claim that every facility or discharge contains them at the same levels. Potential contaminants
- Pathogens and drug-resistant microorganisms.
- Pharmaceuticals, including antibiotics and cytotoxic substances, and their metabolites.
- Disinfectants and chemicals from laboratory or other clinical activities.
- X-ray contrast media and kitchen residues such as grease and oils.

How is hospital wastewater treated?
Treatment belongs within a broader management system: identify and manage sources, contain and collect wastewater, select treatment appropriate to the facility, and dispose of treated flows and residuals according to applicable requirements. - Plan for reliable operation. A treatment option must be supportable with the facility’s available staff and resources, including maintenance and appropriate handling of treatment residuals. How to choose an approach for a facility
The choice is a site-specific planning decision, not a technology ranking. Start with the facility’s actual wastewater sources and the conditions governing its discharge.

The available WHO sources support these decision factors, but do not establish a universally preferred process, provide comparative performance rankings, or give engineering design specifications. A facility should not assume that a particular process guarantees removal of every pathogen or chemical, or that municipal treatment always removes every pharmaceutical.
Wastewater is not the same as healthcare waste overall
Healthcare waste includes solid and liquid streams; the term is broader than wastewater. Infectious, chemical, pharmaceutical or cytotoxic, and radioactive wastes involve related but distinct management tasks. The WHO’s 2024 fact sheet estimates that about 85% of waste generated by healthcare activities is general, non-hazardous waste, with the remaining 15% considered hazardous.
