In the United States, a transplant hospital evaluates a person and decides whether and when to list them for a heart transplant. After listing, the OPTN matching system orders candidates for each donated heart using urgency, compatibility and distance rules. The transplant team—not the patient or a simple first-come queue—reviews the offer and decides whether to accept it.

How does a heart transplant evaluation work?

A heart specialist can refer someone to a transplant hospital, but referral does not automatically put that person on the waiting list. The hospital’s transplant team gathers medical information, runs tests and discusses the person’s condition and current treatment. A coordinator and often a social worker help guide the process. The hospital makes the listing decision; there is no single test or identical public checklist that determines eligibility at every center. See HRSA/OPTN’s Transplant Process and Adult Heart Allocation System FAQs.

Evaluation does not end at listing. Candidates should tell their transplant team about major changes in health or treatment. The team can submit updated information, and a candidate’s status may change as a result.

What do adult heart transplant statuses mean?

Adults in the United States are assigned one of six medical urgency statuses. Status 1 indicates the highest urgency; Status 6, the lowest. The status reflects detailed medical criteria and current support or treatment, not a guarantee of an offer or a personal waiting-time estimate.

Advanced support, severe or difficult-to-control conditions, complications of treatment and the need to remain hospitalized are among the factors HRSA describes. Status may rise or fall as a person’s health and treatment change. HRSA’s patient FAQ summarizes the sequence: “The most medically urgent candidates (Status 1) are considered first, followed by less urgent candidates who are a good match to the donor.”

Status 1 and 2 candidates can be considered across a larger geographic area before candidates in lower statuses. Even so, priority depends on the particular donor, compatibility and the applicable allocation rules. Status alone does not determine who receives a specific heart.

How is a donor heart matched to a recipient?

For each donated heart, the OPTN matching system identifies candidates who may match and orders offers under organ-specific policy. Heart allocation gives particular weight to medical urgency, blood-type match and distance between the donor and transplant hospitals. Other general matching considerations can include organ size, donor and candidate height or weight, age category and waiting time; their role varies by organ and policy. HRSA explains the broader process in Donor Matching System and How Organ Allocation Works.

Distance matters in part because a heart has a limited preservation window. HRSA says hearts and lungs are usually transplanted within six hours after removal. That is general guidance, not a guaranteed deadline for every offer; transport and clinical circumstances differ.

The process is not a simple chronological queue. A candidate’s urgency and compatibility with the donor matter, as does the candidate’s location relative to the donor hospital. If nearer candidates are not a match or their transplant programs decline, offers can move to candidates at more distant hospitals under the applicable rules. Detailed classifications and ordering rules are policy-specific; consult the current HRSA/OPTN policy index for operational policy.

Factors the matching system does not use

HRSA’s broad donor-matching explanation says the matching system does not consider ethnicity, sex, religion, financial status, celebrity status or citizenship. Separately, HRSA’s adult heart FAQ says insurance type and the cause of heart disease do not determine an adult candidate’s urgency status. The latter concerns status assessment, not a claim that every aspect of medical evaluation is identical.

What happens when a heart becomes available?

Once the system orders candidates, the organ procurement organization (OPO) contacts a transplant team for a candidate near the top of the match run. The team receives detailed information about the donor and organ, considers it alongside the candidate’s current condition and decides whether to accept or decline. A team may decline if the candidate is too sick at that moment or if medical details indicate that the heart is not suitable. An offer therefore does not guarantee that transplantation will proceed.

How long is the wait for a heart transplant?

There is no universal waiting-time estimate in HRSA’s patient guidance. Waiting varies with factors that include blood type, tissue type, candidate height or weight, donor-organ size, medical urgency and time on the waiting list. A suitable donor must become available for that candidate under the matching rules, so time listed by itself does not establish how soon an offer will come. Ask the transplant team for guidance based on the candidate’s current circumstances; a general online estimate cannot predict an individual wait.

How does allocation differ for children?

Children younger than 18 do not use the adult Status 1–6 categories. HRSA describes pediatric statuses as 1A, 1B and 2, with different medical criteria. A child’s team can explain the criteria and allocation process applicable to that patient.

Where can candidates confirm current rules?

Allocation policy can change, and individual status and offer decisions depend on clinical details. Candidates should discuss their tests, status, health changes and any offer with their transplant team. For current U.S. operational rules, use the HRSA/OPTN policy index rather than relying on a general description as a personal eligibility decision.
