Hearing turns pressure waves in the air into electrical signals the brain can interpret as sound. The process begins when the outer ear directs sound to the eardrum, continues as tiny middle-ear bones transmit vibration into the fluid-filled cochlea, and ends as signals travel along the auditory nerve toward the brain.
How does hearing work, step by step?
The National Institute on Deafness and Other Communication Disorders (NIDCD) describes hearing as a sequence that changes sound waves in air into electrical signals. Its “How Do We Hear?” overview, last updated March 16, 2022, shows the pathway from the outer ear through the brain.

The outer ear collects sound
The visible outer ear, called the pinna, helps collect sound and direct it into the ear canal. Sound waves travel down the canal and make the eardrum—a thin membrane at the canal’s end—vibrate.

The middle ear passes on the vibration
The eardrum’s movement sets three small bones in the middle ear in motion: the malleus, incus, and stapes. These bones, collectively called the ossicles, transmit and amplify the vibration toward the inner ear.

The cochlea turns vibration into fluid movement
The stapes moves at the entrance to the cochlea, a fluid-filled, snail-shaped structure in the inner ear. That movement creates motion in the cochlear fluid and a traveling wave along the basilar membrane, a strip of tissue inside the cochlea.

The cochlea separates information by pitch
Sensory hair cells sit along the basilar membrane. Different places along it respond best to different sound frequencies, so pitch information is organized by location. NIDCD says hair cells near the wide end respond to higher-pitched sounds, while those closer to the center respond to lower-pitched sounds. The MSD Manual’s inner-ear overview also describes frequency-responsive regions of the cochlea.
Pitch is not the same as loudness: this location-based mapping concerns how high or low a sound is, not how loud it is.

Hair cells convert motion into electrical activity
As the basilar membrane moves, the hair cells move with it. Each cell has tiny projections called stereocilia. When these bend, channels at their tips open, allowing chemicals to enter the cell and generate an electrical signal. This conversion of mechanical motion into electrical activity is called transduction—in plain language, changing one kind of signal into another.

The auditory nerve carries information toward the brain
The electrical activity is relayed through the auditory nerve from the cochlea toward the brain. The brain interprets the incoming information as sound that can be recognized and understood. Sound waves do not travel through the nerve; the ear has converted their mechanical effects into neural signaling.
What happens to sound along the way?
A relay is a useful analogy: the outer and middle ear pass along motion, the cochlea sorts some information by frequency and converts motion into electrical signaling, and the auditory nerve carries information onward. It is only an analogy, not a literal account of every step in the neural code.

How can problems at different points affect hearing?
Hearing depends on several linked stages, so hearing loss does not always begin in the same place. Damage to cochlear hair cells is one possible cause: NIDCD’s noise-induced hearing loss overview explains that noise can damage and eventually kill hair cells, which do not grow back in humans. That is a specific example involving noise; it should not be taken as an explanation for every type of hearing loss.

NIDCD explains that they use a microphone, amplifier, and speaker to make sounds louder. Amplification may help some people with sensorineural hearing loss, but extensive inner-ear damage can limit how much benefit a hearing aid provides. The device cannot restore the cochlea’s normal conversion process.
