Extrachromosomal DNA (ecDNA) can give cancer cells extra copies of growth-promoting genes and change how those genes are switched on. Because ecDNA is inherited differently from chromosome-bound DNA, its amount can vary among cells as a tumour grows. That variation can help some cells gain an advantage under treatment, although ecDNA is one contributor to resistance—not a complete explanation for it.

What is ecDNA in cancer?

Extrachromosomal DNA is DNA that sits outside the chromosomes in a cell’s nucleus. In cancer, ecDNA is often circular and can carry oncogenes—genes that promote cell growth—along with enhancers and other regulatory elements. Nature’s 2024 review describes ecDNA elements as typically larger than 500 kilobases.

Chromosomes are copied and distributed to daughter cells through an organized process. ecDNA does not follow the same inheritance pattern. Its copies can be apportioned unevenly when a cancer cell divides, so daughter cells may end up with different amounts or combinations. A 2024 Nature study also examined coordinated inheritance of ecDNAs, underscoring that their inheritance patterns can be complex rather than simply random.

How can ecDNA help cancer cells grow?

More copies of growth-promoting genes

An ecDNA molecule can carry an amplified oncogene. When cancer cells have many copies of such a gene, they may produce more of its growth-promoting signals. This is a gene-dosage effect: the extra genetic copies can increase the amount of gene activity, though the effect depends on which genes the ecDNA carries and how they function in that cell.

Regulatory elements can raise gene activity

ecDNA can carry enhancers and promoters—DNA elements involved in regulating when and how strongly genes are expressed. Its regulatory layout may bring these elements into interactions that increase oncogene transcription. ecDNA molecules can also cluster into hubs, where enhancers and promoters on separate molecules interact. These mechanisms offer a route to high gene activity beyond simply having extra copies of an oncogene, as discussed in Nature Reviews Cancer’s 2024 review.

How can ecDNA contribute to treatment resistance?

Unequal ecDNA inheritance can leave a tumour with a mix of cancer cells: some may carry more ecDNA, different ecDNA molecules, or different levels of ecDNA-borne oncogenes than others. That cell-to-cell variation gives the tumour material on which selection can act. If treatment puts pressure on the tumour, cells with a genetic profile that helps them survive may become more prominent. Changes in ecDNA content can therefore contribute to tumour evolution over time.

This does not mean ecDNA inevitably makes a cancer resistant, or that it is the sole cause when treatment stops working. Resistance can have multiple causes, and the effect of ecDNA depends on the tumour and the genes and regulatory elements it carries.

What clinical studies have found—and what they do not prove

A 2024 Nature study analyzing 14,778 patients and 39 tumour types in the Genomics England 100,000 Genomes Project detected ecDNA in 17.1% of tumour samples. In that cohort, ecDNA detection was associated with tumour stage, metastases, shorter overall survival, and greater prevalence after targeted therapy and cytotoxic treatment. These are observational associations: they show that the features occurred together in the analyzed data, not that ecDNA alone caused an individual patient’s outcome or that treatment created ecDNA.

The same cohort study reported that ecDNA can carry immunomodulatory and inflammatory genes. Tumours with ecDNA carrying these genes were associated with reduced T-cell infiltration. This suggests a possible link with immune evasion, but the reported association does not establish that ecDNA caused the lower infiltration.

Are ecDNA-targeted treatments available?

The cited treatment findings are experimental, not established cancer care. A 2024 Nature study investigated whether increasing transcription–replication conflicts could selectively harm ecDNA-positive cancer cells in experimental models and tested CHK1 inhibitors. The National Cancer Institute’s December 2024 report described laboratory experiments involving BBI-2779. These results point to possible research directions; they do not establish an approved ecDNA-targeted treatment or a standard treatment choice.

Do not start, stop, or change cancer treatment based on ecDNA status or these laboratory findings. Treatment decisions should be made with the oncology team using evidence relevant to the specific diagnosis and patient.
