Footballers return after a minor muscle injury through an individual, criteria-based assessment—not a fixed number of days or a single test. A player may resume some training before being cleared for unrestricted training or match selection. Clinicians consider symptoms, strength and flexibility, football-specific performance, confidence, and the player’s readiness, with input from relevant medical and performance staff.
What does “return to play” mean?
A football-specific hamstring consensus defines return to play as “the moment a player has received criteria-based medical clearance and is mentally ready for full availability for match selection and/or full training.” The definition comes from a worldwide expert Delphi process reported by van der Horst and colleagues on behalf of the HIPS-Delphi Group in 2017. That milestone is about readiness and clearance, not proof that a player has completed a match or regained every pre-injury measure. Selection can still depend on the coach and team circumstances. Nor does a training photo, partial session, or squad appearance by itself reveal whether the player has restrictions or has received medical clearance.

The clearest football-specific criteria in the available sources concern hamstring injuries. They are assessment domains, not a universal pass/fail checklist for every muscle problem.

- Symptoms: No pain on palpation or during strength, flexibility, and functional testing.
- Flexibility: Hamstring flexibility similar to the player’s pre-injury data and/or the uninjured side, depending on which comparison the medical staff considers available and reliable.
- Strength and function: The player’s capacity is assessed alongside soccer-specific actions. A 2026 systematic review found that strength and pain criteria were among the most commonly cited for hamstring clearance, including between-limb knee-flexor/extensor comparisons and absence of pain during soccer-specific actions. These commonly reported measures do not establish a universally safe numeric threshold.
- Field performance: Football-specific tests help assess whether the player can meet relevant demands. In a Delphi study involving English professional clubs, functional performance received the highest ratings among five domains, and sprint ability was rated particularly highly. The authors said further work was needed to establish whether the agreed criteria were valid. Read the club Delphi study.
- Readiness and confidence: The player’s psychological readiness matters alongside physical findings.
For hamstring return-to-play readiness, the worldwide consensus says MRI findings should not be used as the measure. Imaging may have a role in care, but a scan appearing normal is not the stated clearance threshold.
Who makes the decision?
Return is a shared, multidisciplinary decision informed by the player, sports physician, physiotherapist, fitness trainer, and team coach. Their roles are not identical: clinical clearance involves medical judgment, while performance, athlete, and team input contributes to understanding readiness and availability. The worldwide Delphi consensus drew on 58 experts selected through 28 FIFA Medical Centres of Excellence; response rates exceeded 83% in each of its four rounds. See the worldwide consensus.

How is returning to training different from returning to play?
The Italian consensus distinguishes return to training (RTT), which may include restrictions, from return to play (RTP), meaning training and competition without restriction. A player can therefore take part in modified team work without being cleared for all training demands or match availability. The Italian statement discusses clinical and imaging assessment, laboratory and field tests around return to training, and objective GPS-based criteria for RTP; it does not reduce clearance to imaging alone or prescribe one progression for every player. Why is there no set number of days?
The reviewed sources do not establish a universal timeline for a “minor” muscle problem. The duration depends on the injury and the individual assessment, so consensus criteria cannot be converted into a reliable day-count prediction.

The evidence also varies by muscle group. A 2025 expert survey examined a 20-criterion protocol for adductor longus injury across clinical, functional, and performance categories, with 63 injury-management professionals. That work is expert protocol evaluation, not a settled universal standard; the source describes a lack of objective criteria and consensus. Read the adductor longus consensus study.
A 2026 systematic review included 135 studies from 58,057 records, with database searches through 10 March 2024. Its findings describe criteria reported in the literature, not proof that a particular threshold predicts a safe return for every footballer. Read the systematic review.

What can a public injury update tell you?
A report that a player has “returned to training” does not necessarily mean unrestricted training or match readiness. To interpret an update, look for whether it specifies restrictions, full training, medical clearance, or availability for selection. Without that detail, the player’s exact status is not established.
