Change impact analysis is not a single decision about whether a device modification is acceptable. It has to connect the change to the quality-system records and risk controls it may affect, then separately assess whether the device change calls for a new 510(k). Software and machine-learning changes can bring additional FDA guidance and planning considerations. The work can involve substantial coordination, but FDA sources do not quantify the hours involved or establish that it is a universal time-sink.
What the QMSR changes about the review context
FDA’s Quality Management System Regulation (QMSR) became effective on February 2, 2026. It amends the device current good manufacturing practice requirements in 21 CFR Part 820 and incorporates ISO 13485:2016 by reference. It applies to finished-device manufacturers intending commercial distribution, including manufacturers of certain accessories treated as finished devices. If an ISO clause conflicts with applicable U.S. law or regulation, the FD&C Act and implementing regulations control.
The framework makes risk-based decision-making and the ISO 13485 quality-management approach central to U.S. device QMS requirements. FDA also began using its updated inspection process on the effective date, replacing QSIT with Inspection of Medical Device Manufacturers Compliance Program 7382.850. This is the current regulatory context for change reviews, not a future transition date.

Older records remain part of the evidence trail
FDA may review QMS records created before February 2, 2026. The agency says a manufacturer may find a comparative analysis useful to show how earlier documents and records meet QMSR requirements. That is not a direction to recreate, rename, or migrate every legacy record. In a change review, the practical concern is whether the relevant older evidence can still be understood and connected to the current assessment.

Why one proposed change creates several decisions
A change can affect the quality system without automatically requiring a new marketing submission, and the submission question does not replace the QMS review. A useful way to understand the coordination is to separate three decision layers:
- QMS and records: identify which controlled processes, records, and risk controls the change may affect, including relevant legacy records.
- Device-specific regulatory assessment: assess whether the modification could require a new 510(k), based on the device and the consequences of the particular change.
- Specialized software or ML considerations: when applicable, use FDA’s software-change guidance or consider whether a planned change control plan is relevant to an ML-enabled device.
These are connected questions, but they are not interchangeable. The need to document an assessment does not by itself establish that a 510(k) is required; conversely, a conclusion that a new 510(k) is not needed does not mean the change has no QMS impact.
Trace the proposed change through its evidence
FDA identifies supply-chain changes, continuous process improvement, and technological innovation as reasons devices may be modified. The following categories are a practical organizing framework, not an FDA-prescribed taxonomy or checklist.

Design or material changes
Start with what is different in the device and which risk controls or supporting records could be affected. A material or design revision may have implications beyond the changed component, so the review should follow the evidence trail to the device-specific consequences rather than assume that a small physical change is automatically minor for regulatory purposes.
Manufacturing, process, or supply-chain changes
Identify the changed process, supplier, or input and consider how the change may affect the device and the records used to support its quality and risk controls. FDA specifically recognizes supply-chain and process-improvement changes as possible device modifications. Their presence does not create a universal submission rule; the regulatory assessment depends on the proposed change and its implications.
Risk-control implications
FDA points manufacturers to risk-based evaluation, such as ISO 14971, when assessing whether a device modification likely requires a new 510(k). Risk assessment helps connect the modification to potential consequences and relevant controls. It is part of the reasoning, not a shortcut that makes the submission decision automatic.

Make the 510(k) decision separately from the QMS record review
For a device modification, FDA describes a risk-based assessment of whether a new 510(k) is likely required. Do not treat a category label—such as a process improvement or supplier change—as a yes-or-no answer. Assess the actual device change, its potential consequences, and the evidence supporting the decision. The applicable regulatory requirements and the facts of the device remain decisive.

This distinction helps explain why change review can require cross-functional coordination: the records and risk rationale must support the quality-system assessment, while the regulatory submission analysis addresses a different question. FDA’s device-modification guidance informs that assessment but should not be converted into an automatic rule for every change.
Apply the dedicated analysis to software and firmware
Software and firmware changes to an existing device have a dedicated FDA guidance, Deciding When to Submit a 510(k) for a Software Change to an Existing Device. It addresses whether a software change may require a new premarket notification and is intended to improve predictability, consistency, and transparency while providing a least-burdensome approach. It predates QMSR; it is not a new QMSR requirement. Use it for the software submission question alongside current applicable regulations, while separately addressing the change’s QMS records and risk controls.

Know when an ML change-control plan may be relevant
For machine-learning-enabled medical devices, FDA, Health Canada, and the UK’s MHRA have described guiding principles for predetermined change control plans (PCCPs). FDA describes PCCPs as a pathway for certain specified changes for which marketing authorization before implementation is typically required. The principles can inform how a plan manages those changes; they do not create a general exemption from change control or regulatory review.
FDA also identifies an August 2025 final guidance on marketing submission recommendations for a PCCP for AI-enabled device software functions. This is a specialized consideration for applicable AI-enabled device software functions, not a default mechanism for every software update or device modification.

- Describe the change: state what is changing and identify the affected device, process, software or firmware, supplier, and relevant QMS records.
- Map the evidence and risk: trace the change to pertinent records and risk controls, including older records where relevant. Use risk-based reasoning to evaluate potential consequences.
- Assess the regulatory submission question: determine whether the specific device modification may require a new 510(k). For software or firmware changes, apply the dedicated FDA guidance to that question.
- Address legacy records where applicable: consider whether a comparative analysis would help demonstrate how pre-February 2, 2026 records meet QMSR requirements; FDA describes this as something manufacturers may find useful.
- Check for specialized planned-change considerations: for an ML-enabled device, assess whether an applicable PCCP is relevant to specified changes and their authorization pathway.
The point of this sequence is to keep the evidence trail and the distinct regulatory decisions visible. The cited FDA materials do not prescribe one universal impact-assessment form or decision tree.
What can—and cannot—be concluded about the time burden
The coordination burden is understandable: a proposed change may need to be traced through QMS records and risk controls, assessed for device-specific regulatory consequences, and routed through dedicated software or ML considerations when those apply. Legacy records can add another evidence task because FDA may inspect records created before QMSR took effect.
Those drivers support describing impact analysis as a potential time-sink, but not as a measured industry-wide fact. The available sources do not establish average hours, cost, frequency, project delays, or that the process is the largest burden in every medical-device company. The scale of work depends on the change and its evidence trail.
