TMAO (trimethylamine N-oxide) is measured with a laboratory blood test, usually using a serum sample and liquid chromatography–tandem mass spectrometry (LC-MS/MS). Follow the performing lab’s preparation instructions—commonly an overnight fast and no fish or other seafood the day before—and review the result with a clinician. A TMAO number alone does not diagnose a disease.
How is a TMAO test performed?
A clinician orders the test and a blood specimen is collected for laboratory analysis. Cleveland Heart Lab describes a serum test using a method it calls LCTMS; the University of Michigan MLabs directory describes LC/MS/MS and lists Cleveland Heart Lab as the performing reference laboratory. Those are laboratory-specific service details, so confirm the collection arrangements with the clinician or lab handling your test.
Turnaround depends on the laboratory and whether the sample is sent elsewhere. Cleveland Heart Lab lists five days; University of Michigan MLabs lists send-out testing with an analytic time of 7–10 days. Ask the ordering office when to expect your result.

How should you prepare for the blood draw?
Instructions differ by laboratory. The cited testing information advises fasting and avoiding fish or other seafood the day before collection. Labcorp specifies a 10–12-hour fast; Cleveland Heart Lab and University of Michigan MLabs describe overnight fasting. Use the instructions for the laboratory that will perform your test rather than combining or substituting directions.

- Avoid fish and other marine foods the preceding day if instructed by the performing lab.
- Do not stop prescribed medicines or supplements unless a clinician tells you to.
How do you interpret a TMAO result?
Use the reference or decision limits printed by the laboratory that performed the assay. Published limits differ: University of Michigan MLabs lists 6.2 µM as its reference value; Labcorp categorizes results below 6.2 µM as low, 6.2–9.9 µM as moderate, and above 9.9 µM as high. Cleveland Clinic patient education describes less than 6.2 µM as ideal. These are source-specific ranges, not a single universal diagnostic cutoff.
TMAO is studied as a possible health risk marker, but an association in a research population does not establish what an individual’s result means. Cleveland Clinic reported a 2026 study of more than 5,000 people undergoing heart procedures examining TMAO and atrial fibrillation, and a 2024 analysis with Tufts University involving nearly 12,000 apparently healthy participants followed for heart-failure risk. Neither finding means a TMAO result diagnoses atrial fibrillation, heart failure, or another condition, or determines an individual’s prognosis.

When should you talk to a doctor?
Contact the clinician who ordered the test if the result is outside the lab’s stated range, the report is unclear, or you are considering testing because of cardiovascular concerns. A clinician can interpret it alongside your medical and family history and established risk factors; TMAO is one of several less-common tests that may be considered in some risk evaluations.

If your concern is kidney health, ask about established screening if you have diabetes, high blood pressure, heart disease, or a family history of kidney failure. NIDDK describes standard assessment using blood and urine tests, including measures of kidney filtration and urine albumin; TMAO is not a substitute for those tests.

Severe or sudden symptoms—such as chest pain, difficulty breathing, fainting, or stroke-like symptoms—need urgent medical care. A TMAO test is not an emergency diagnostic test.
