Smart bands are best used to follow broad trends, not as precise measuring instruments or medical tests. Wrist heart-rate estimates tend to agree more closely with reference measurements at rest, during sleep, and during steady treadmill activity than during cycling or resistance training. Sleep estimates can differ from a sleep-lab study, and pooled blood-oxygen results do not establish that a particular band is accurate.
How accurate is a smart band’s heart-rate reading?
Accuracy depends on what you are doing. A 2020 systematic review and meta-analysis compared wrist photoplethysmography (PPG) devices with ECG or chest-strap measurements. Its pooled mean differences were close to zero during sleep, rest, and treadmill exercise, but larger during cycling and resistance training. These are averages across studies, not a guarantee about any individual reading or current band.
| Condition | Pooled mean difference reported in the 2020 review |
|---|---|
| Sleep | -0.40 beats per minute (bpm), pooled across studies |
| Rest | -0.01 bpm, pooled across studies |
| Treadmill activity | -0.51 bpm, pooled across studies |
| Resistance training | -7.26 bpm, pooled across studies |
| Cycling | -4.55 bpm, pooled across studies |
These figures come from the 2020 wrist-PPG systematic review and meta-analysis. A pooled mean difference near zero does not mean every reading was close to the reference: positive and negative errors can offset one another, and the figures do not describe the spread of individual readings. They also should not be transferred to a specific model without a study of that model.
In practical terms, a band may be more useful for seeing whether your resting heart rate generally rises or falls than for judging an exact heart rate during a set of weights or a cycling interval. The review did not establish performance for every band, wearer, or real-world condition.

Can a smart band accurately track sleep?
A consumer band’s sleep estimate is not equivalent to polysomnography (PSG), the clinical sleep-study reference used in validation research. A 2024 meta-analysis of 24 studies involving 798 participants found significant pooled differences between consumer wrist trackers and PSG across four measures:
| Sleep measure | Pooled mean difference versus PSG reported in the 2024 meta-analysis |
|---|---|
| Total sleep time | -16.854 minutes, pooled across the included studies |
| Sleep efficiency | -4.691 percentage points, pooled across the included studies |
| Sleep latency | +2.574 minutes, pooled across the included studies |
| Wake after sleep onset | +13.255 minutes, pooled across the included studies |
The values are the meta-analysis’s reported pooled mean differences; they summarize group-level comparisons and do not predict how far a particular person’s band will differ on a given night. The analysis does not make a band’s sleep stages interchangeable with clinical assessment. The 2024 consumer wrist-tracker meta-analysis supports using trackers to notice broad patterns, while treating nightly totals and stage estimates as estimates rather than PSG results.

Are wrist blood-oxygen (SpO₂) readings reliable?
There is no basis in the pooled evidence here for calling a particular smart band’s oxygen reading accurate. A 2024 living umbrella review reported wearable blood-oxygen mean absolute differences of up to 2.0%, but found substantial heterogeneity among the underlying evidence. That pooled result covers wearables overall and cannot validate an individual band or its readings.

The 2024 living umbrella review also found that validation varied across devices and outcomes. An average error across a group of products is not a model-specific accuracy rating; it does not tell you how a specific device performs for your circumstances or whether a reading is suitable for a health decision.

Why do readings vary, and what does validation tell you?
Heart rate, sleep, and SpO₂ are separate measurements. Evidence for one metric does not validate the others, and results for one device generation should not automatically be applied to another. Activity matters for heart rate, while the reference method and study design matter when judging any metric.
The 2024 umbrella review covered 24 systematic reviews, 249 non-duplicate validation studies, and 430,465 participants. Its authors estimated that approximately 11% of commercially available wearables had been validated for at least one biometric outcome; the available validation studies represented about 3.5% of the studies they said would be needed for comprehensive evaluation across outcomes. These review-level estimates describe the breadth of validation evidence, not the accuracy of a specific band.

When assessing a claim about a band, look for an independent study of the exact model and metric, with an appropriate reference: ECG or a chest strap for heart rate, PSG for sleep, or an appropriate clinical reference for oxygen. A study’s participants, setting, and limits of agreement matter; correlation alone does not show how close readings are to the reference.
When should you trust a band reading?
- For personal trends: Use readings to notice broad changes over time, rather than treating one number as exact.
- For exercise: Be especially cautious about exact heart-rate values during cycling or resistance training, where pooled differences were larger in the reviewed studies.
- For sleep: Treat duration and stage breakdowns as estimates, not as a diagnosis or a substitute for a clinical sleep study.
- For oxygen: Do not use a wellness band reading alone to diagnose a condition or make a medical decision. A separate fingertip pulse oximeter is a different product, not a way to validate or calibrate the band.
The FDA says general-wellness pulse oximeters are not reviewed or evaluated by the agency before they are marketed. It also notes that readings can be affected by factors including poor circulation, skin pigmentation, skin thickness, skin temperature, current tobacco use, and fingernail polish. Medical-purpose devices are a separate category; a device’s status should be checked for its intended use, and clearance is not a blanket guarantee for every person or situation. See the FDA’s Pulse Oximeter Basics.
