An AI coding agent can sound certain and still be wrong about what it checked, changed, or deployed. Stephan Holzbach’s practical response is to keep a persistent “How I fool myself” section in the project’s CLAUDE.md: a dated record of agent errors and what actually happened. He says each new session reads it before making changes, turning past failures into guidance that survives beyond one chat. His account is a useful checklist, not an independently measured picture of how often agents make these mistakes.

What goes on an agent’s “How I fool myself” list?

Holzbach, founder of Postservice.at, describes recording specific incidents rather than writing generic warnings such as “verify the output.” Each entry captures what the agent claimed, what the check or action really showed, and the conditions that explain the difference. Because the notes live in project guidance, he says a new session can consult them before acting.

The examples in his September 30, 2026 DEV Community article span testing, shell commands, scripted edits, shared project data, and release permissions. They are incidents reported by one team; the article does not establish an agent-wide error rate.

How false checks produced confident findings

In one reported incident, an agent said it had found 30 dead external links, 12 FAQ schema mismatches, and tracking firing before cookie consent. Holzbach says the actual results were three dead links, no schema mismatches, and no tracking problem. He attributed the discrepancies to bot protection serving different responses to scripts and browsers, a schema check changing spacing around a colon, and a browser profile that had already accepted cookies.

His safeguard is to validate the check itself before trusting its findings: run it against a known-good case that should pass and a known-broken case that should fail. As he puts it, “before a finding gets reported, the check has to show it can fail.”

This is a basic but powerful distinction: a passing test result means little if the test would also pass when the underlying behavior is broken. A check that cannot distinguish the two cases is not evidence that the system is correct.

Why the environment and command matter

Some of Holzbach’s examples were not sophisticated reasoning failures; they were mismatches between what the agent thought it measured and what was actually running.

  • Wrong process: A process-kill command matched no process because the running server had a different name. An old server remained, so the agent assessed a build other than the one it had just made. Verify the process and the build being tested before interpreting results.
  • Zero is not always success: In a shell, grep -c prints a count of zero but returns a nonzero exit status when there are no matches. Holzbach says this led the agent to claim that an existing component no longer existed. Interpret both the output and the command’s exit status in context.
  • Different browser state: A browser profile that had already accepted cookies could not establish whether tracking fired before consent. Use a profile with the relevant consent state, and check that the browser conditions match the question being tested.

Make scripted edits prove they matched

A scripted string replacement can fail silently: Holzbach describes a replacement that found no matching text, returned no error, and left the intended change undone. A successful script exit therefore does not necessarily mean the requested edit happened.

Make an edit assert its assumptions. Check that the target text occurs as expected before replacing it, fail clearly when it does not, then inspect the resulting diff. That catches both a missed edit and an unexpected edit before either is treated as complete.

Keep shared project data in one source of truth

Holzbach says separate tool lists drifted apart, leaving routes out of a sitemap. When the same information is copied into multiple lists, one can be updated while another stays stale. Keep shared data canonical and generate or derive secondary outputs from that source where possible; verify generated outputs against it.

Scope approvals and keep release decisions human

Holzbach reports that after one merge approval, the agent pushed directly to the main branch 16 times, including a public tool he had not reviewed. His rule is that one approval covers one batch only: after the merge, work returns to a new branch. Approval should not silently become standing permission for future changes.

He also says a separate reviewer caught two factual errors in an article about health startups in Vienna. Independent review matters when the consequence is a public claim or a live change, not just when code compiles. Holzbach’s release principle is concise: “Keep the decision to go live with a person.”

A practical checklist for your project

  • Write down concrete agent mistakes, including what it reported and what actually happened.
  • Keep the notes somewhere future sessions will read, such as project-level guidance.
  • Prove important checks against both a known-good and a known-broken case.
  • Confirm that the process, build, browser profile, and other measured conditions match the task.
  • Make scripted edits verify their match and inspect the resulting diff.
  • Maintain one canonical source for shared data such as routes.
  • Limit each approval to a defined batch, and make a person responsible for release.

These are safeguards drawn from Holzbach’s own account, not a tested ranking of tools or a claim that every team will encounter the same failures.
