A Claude Code hook can run a check at a specific point in a coding session, making it possible to block a detectable action, report a problem after a tool runs, or ask Claude to verify work before it stops. In my case, adding one hook stopped Claude Code from repeating the same mistake; that is my experience, not a measured result or a guarantee for other projects.

What a Claude Code hook does

A hook is a configured handler tied to an event in Claude Code’s lifecycle. When that event occurs, Claude Code runs the handler, which can inspect available information and return an outcome such as allowing an action, blocking it, or giving feedback. The key is matching the check to the point when it can still help.

Anthropic describes hooks as user-defined commands, HTTP endpoints, or LLM prompts that provide more deterministic control by firing on lifecycle events such as file edits, tool calls, or session start. Read the current Claude Code Hooks reference for the available events, configuration, and behavior, which can change between versions.

Choose the event that matches the mistake

What you need Event or mechanism What it can do Limit to understand
Catch a detectable problem before a tool action PreToolUse Inspect a proposed tool call and block it when the hook signals a block. The condition must be detectable from the event information available to the hook; test what happens when the handler succeeds or fails.
React to a tool’s result PostToolUse Inspect tool input and response, then provide feedback. The tool has already run, so this cannot prevent that action.
Check work before Claude finishes Stop Prevent stopping and continue the conversation if a condition is unmet. Continuation is bounded; the condition should be specific and resolvable.
Provide a rule as guidance CLAUDE.md, rules, or skills Put instructions in Claude Code’s context. Guidance is not an automatically executed check.
Keep project knowledge available across sessions Project or automatic memory Retain information for later use. Recall is different from enforcing a check at a particular lifecycle event.

For a mistake that must be caught before a consequential tool call, start by considering PreToolUse. If the error only becomes visible in the result, PostToolUse can surface feedback, but cannot undo the completed action. For a final verification condition, Stop may be a fit.

Claude Code’s hook reference explains how a handler’s exit code affects whether an action proceeds, is blocked, or is ignored: hook exit-code behavior. Read the event-specific rules before relying on a particular outcome.

Turn a repeated mistake into a precise check

A hook is most useful when the mistake can be expressed as a condition rather than a vague instruction. Identify the attempted action or result that signals the problem, then decide what response will actually help: block the action, report what went wrong, or request a bounded completion check.

  • Trigger: Choose the lifecycle event that occurs at the right time.
  • Input: Determine which event data the handler can inspect.
  • Condition: Define the observable pattern that identifies the mistake.
  • Outcome: Specify whether the action should be blocked or feedback returned.
  • Recovery: Give Claude a clear reason and a plausible next step rather than a generic demand to try again.

The exact implementation depends on the mistake, the event, and the handler you configure. No single hook configuration can be inferred from the claim that one particular hook worked: the specific bug, code, tested Claude Code version, and observed evidence are not established here. Check the current reference and test your own handler against both matching and non-matching cases before depending on it.

Use instructions and memory for their different jobs

Not every repeated error calls for a hook. Anthropic’s guidance distinguishes hooks from instruction files and skills: use hooks for a deterministic action at a lifecycle event, and use CLAUDE.md, rules, or skills to give Claude instructions and guidance. Anthropic’s Claude Code best-practices article discusses how these steering mechanisms fit together.

When the need is to preserve project knowledge across sessions rather than enforce a check at a specific moment, memory is the adjacent mechanism. See Claude Code memory documentation for its behavior and options. A saved note can remind Claude of a convention; it is not the same as a check that runs before a tool action.

Make Stop checks bounded and actionable

A Stop hook can keep the conversation going when a completion condition is unmet. This is useful only if the check identifies something Claude can plausibly resolve—for example, a specific failed validation—rather than repeatedly asking for another attempt without defining success.

The current reference documents a default cap of eight consecutive hook-driven continuations and provides stop_hook_active so a hook can detect that it is already continuing because of a Stop hook. Account for that behavior when designing a Stop check; it is not an unlimited retry mechanism. Consult the current Stop-hook reference for details.

Treat hook commands as executable automation

A hook runs configured code or calls a configured endpoint, so inspect what it does before enabling it. Anthropic’s security guidance recommends validating and sanitizing hook input, quoting shell variables, checking paths for .., and using absolute paths for scripts. Avoid exposing sensitive material such as .env files, .git/, or keys to a handler. The current hooks security guidance covers these precautions.

  • Reject unsafe paths, including traversal with .., and prefer absolute script paths.
  • Limit what the handler can read, especially credentials and other sensitive files.
  • Test expected failures as well as successful checks, so a handler error does not silently defeat the behavior you intended.

What the result does—and does not—show

In this case, I observed that one added hook stopped Claude Code from making the same mistake again. That is an author-reported result; it does not establish a general reduction rate, prove that hooks prevent every repeat, or substitute for testing the specific check. The documented mechanism explains how a lifecycle check can catch or report a problem at a predictable point, while the outcome depends on the condition, event, and handler.
