No single tactic in the available evidence is shown to cut healthcare ransomware risk by 70%. The percentage is easy to misread: one source uses a figure above 70% for ransomware’s share of successful healthcare cyberattacks, while another reports a 70% payment likelihood across its dataset. Neither measures a tactic’s reduction in attack risk. For healthcare organizations, the supported approach is layered risk management—with tested recovery, incident readiness, and attention to vendors—not reliance on a one-number cure.
What does the 70% figure actually measure?
“Risk” could mean the chance of an attack, successful encryption, ransom payment, clinical downtime, or financial loss. A claim that a tactic reduces risk by 70% is meaningful only if it defines the outcome, the population, the time period, and the comparison being made. The sources cited here do not establish that a single healthcare control produces that reduction.
- The American Hospital Association says ransomware accounted for more than 70% of successful cyberattacks on healthcare organizations in each of two years. That is ransomware’s share of successful attacks, not the amount by which a control reduced risk. AHA: Ransomware Attacks on Hospitals Have Changed.
- An Arete and Cyentia Institute report gives an overall ransom-payment likelihood of 70.0%. Its figures describe payment outcomes among organizations in its ransomware dataset; they do not show that a tactic prevented 70% of attacks. In the report’s MFA row, MFA was present in 19.3% of the healthcare dataset, 34.4% of the demand was paid, and payment likelihood was 52.0%. These are report-specific payment measures, not an estimate of attack prevention. Arete and Cyentia Institute: Healthcare Sector Ransomware Spotlight.
Payment comparisons among organizations that already experienced ransomware cannot, by themselves, establish how likely all healthcare organizations are to be attacked. Nor do they prove that MFA or another control caused a difference in payment outcomes. Treat prevention, payment, recovery, and patient-care disruption as distinct measures.

Why healthcare ransomware is an operational risk
Ransomware can affect care delivery as well as the confidentiality of electronic protected health information (ePHI). A 2022 cohort study examined 374 reported attacks on U.S. healthcare delivery organizations from 2016 through 2021, affecting nearly 42 million individuals. It documented disruption to care in 166 attacks, or 44.4%. Reported effects included electronic-system downtime, canceled scheduled care, and ambulance diversion. Annual attack counts in that dataset rose from 43 in 2016 to 91 in 2021. Those figures describe reported incidents in a defined period; they are not a complete census of attacks or a controlled test of a prevention measure. The study notes limits including underreporting and lack of visibility into attempted attacks that did not succeed. Its value is in showing that incident planning must account for clinical operations, not only data restoration.
What healthcare organizations should do instead
HHS guidance does not prescribe one universal tactic or promise a particular percentage reduction. It describes a risk-analysis and risk-management process, safeguards to guard against and detect malicious software, and user training. Covered entities and business associates should identify risks to ePHI and implement measures sufficient to reduce them to a reasonable and appropriate level. Build recovery around tested backups
Backups matter only if the organization can restore usable data and systems. HHS calls frequent backups and the ability to recover from them crucial, recommends periodic restoration tests, and advises considering offline backups because some ransomware variants disrupt online backups. A backup plan should therefore be validated through restoration—not judged solely by whether backup jobs report success.
Plan for downtime and response
HHS contingency-planning guidance includes disaster recovery, emergency operations, criticality analysis, and periodic testing. Incident-response procedures should cover detection, containment, eradication, recovery, and post-incident review. For a healthcare organization, exercises should involve the people responsible for clinical workflows as well as security and IT: the documented attacks show that system outages can lead to canceled care or ambulance diversion.

Include business associates in the risk picture
Third-party exposure is not peripheral. A study published in 2025 analyzed HHS Office for Civil Rights provider filings and identified 831 U.S. healthcare ransomware incidents from 2016 through 2024; 281, or 33.81%, explicitly involved a HIPAA business associate. The study found that business-associate-involved breaches were smaller on average across the distribution but could be larger among very large incidents affecting 100,000 or more individuals. These are findings from provider-reported incidents and the study’s classification, not a universal estimate for every vendor relationship. Health and Technology: Third-party risk in U.S. health care ransomware incidents: business associate involvement and breach size.
The authors’ practical implications include assessing vendors according to operational criticality, setting proportionate assurances and incident-coordination expectations, and exercising with key vendors. These measures are risk-management recommendations, not experimentally proven ways to achieve a fixed reduction in attack risk.

How to judge a ransomware-risk claim
Before acting on a percentage, ask what it counts and how it was established. A useful claim should specify:
- Outcome: attack attempts, successful compromise, encryption, payment, downtime, or losses.
- Population and period: which organizations were studied, where they operate, and when the data were collected.
- Comparison: what happened with the tactic versus without it, and whether the groups were otherwise comparable.
- Evidence type: whether the result is a causal estimate, an observational association, or a descriptive statistic among reported incidents.
- Operational coverage: which accounts, systems, clinical workflows, ePHI, and vendor dependencies the measure addresses.
- Testability: whether the organization can demonstrate recovery and response readiness in periodic tests and exercises.
A percentage about payment after an incident cannot answer how much a control reduces the likelihood of being attacked. Likewise, a percentage describing ransomware’s share of successful attacks cannot establish the effectiveness of a particular safeguard.
