There is no single standard price tag for healthcare ransomware. A data-breach average, the cost of a broad cyberattack, a ransom payment, and the expense of restoring an organization measure different things. Recovery also has a clinical objective: keep essential care moving while teams contain the threat, verify systems, and restore them safely.

What does ransomware recovery cost in healthcare?

The available figures show the financial stakes, but none is a universal estimate of what it costs a hospital or clinic to recover from ransomware. The unit measured, study population, and included expenses differ.

Reported figure What it measures Scope and limitation
$10.93 million Average healthcare data-breach cost in IBM’s account of the 2024 Cost of Data Breach Study, conducted with the Ponemon Institute. Healthcare-specific, but not ransomware-only; it is not a ransomware system-restoration estimate.
$3.9 million Respondents’ estimated average total cost of their single most expensive cyberattack in the prior 12 months, in the Ponemon Institute’s 2025 Study on Cyber Insecurity in Healthcare. The study includes cloud or account compromise, supply-chain attacks, ransomware, and business email compromise. The estimate includes direct cash outlays, labor, overhead, and lost business opportunities, so it is not ransomware-specific.
$1.2 million The Ponemon Institute’s 2025 study extrapolation of the costliest ransom paid. This is a ransom-payment amount, not an average, total incident cost, or recovery bill. The study reported $1.1 million for the corresponding figure in 2024.

The 2025 study also found that 72% of surveyed organizations affected by its four studied attack types reported disruption to patient care. Among surveyed organizations that reported ransomware attacks, 67% said ransomware had a negative impact on patient care. These are respondent-reported survey results: the 72% figure covers multiple attack types, and neither percentage is a causal estimate for every incident.

For leaders planning budgets, the practical takeaway is to avoid treating any one of these numbers as a forecast. A local recovery estimate depends on the systems affected, length of disruption, recovery objectives, clinical dependencies, response needs, and applicable notification obligations.

How do hospitals recover from ransomware?

Recovery is a coordinated process, not simply copying files back from a backup. Teams need to contain and investigate the incident, remove the threat and its entry paths, sustain care through contingency procedures, and restore verified systems in a deliberate order. The nine tactics below are a practical synthesis of HHS and HHS 405(d) guidance, not an official nine-step government list.

1. Know what must be protected

Maintain an accurate, thorough risk analysis of electronic protected health information (ePHI) and the risks to its confidentiality, integrity, and availability. Map the systems, applications, vendors, and workflows that handle or depend on ePHI. Revisit the analysis when material changes occur, such as a new vendor, system, or clinical workflow.

This inventory supports both prevention and recovery: it helps identify which services are exposed and which applications and data must be available to deliver care.

2. Prepare and rehearse the response

Keep an incident-response plan with assigned decision roles and clear escalation paths. Exercise it with clinical operations, security, IT, privacy, legal, and communications stakeholders so people know who can authorize isolation, downtime procedures, and restoration decisions.

HHS 405(d) encourages organizations to create and exercise incident plans and regularly test a ransomware recovery playbook. A rehearsal should expose practical dependencies, such as how staff will access patient information if a system is unavailable and how restored data will be checked before clinical use.

3. Contain the spread quickly

When ransomware is suspected, activate the response plan and isolate affected systems as appropriate to limit propagation. The response team should coordinate containment with clinical leaders so actions reduce risk without creating avoidable danger to patients.

4. Scope the incident before restoring

Identify affected networks, systems, and applications; determine whether the attack is still active or has spread; and investigate the entry point, methods, and vulnerabilities involved. Restoration before responders understand the scope can return compromised systems to production or leave connected systems exposed.

Include relevant vendors and business associates in the investigation when their systems or access could be involved. Scope should guide containment, remediation, and the order in which services can safely return.

5. Eradicate the threat and remediate access paths

Remove ransomware and address the vulnerabilities, credentials, or other access paths that enabled the attack and its spread. Do not simply return compromised systems to production unchanged. Verify that remediation has addressed the conditions that allowed the incident before reconnecting systems or restoring normal access.

6. Protect and test backup copies

Maintain frequent backups and periodically test restorations for integrity. HHS 405(d) recommends offline, encrypted backup copies and a 3-2-1 backup strategy. Offline copies matter because ransomware may disrupt backups that remain reachable from production systems.

HHS OCR’s Fact Sheet: Ransomware and HIPAA says: “Test restorations should be periodically conducted to verify the integrity of backed up data and provide confidence in an organization’s data restoration capabilities.” A backup that has never been restored is not proven to meet the organization’s needs.

WD 12TB My Book Desktop External Hard Drive, USB 3.0, External HDD with Password Protection and Auto Backup Software - WDBBGB0120HBK-NESN
  • Massive capacity, up to 18TB capacity (1 1TB = one trillion bytes. Actual user capacity may be less depending on operating environment.).Specific uses: Business, personal
  • Includes software for device management and backup with password protection (Download and installation required. Terms and conditions apply. User account registration may be required.)
  • 256-bit AES hardware encryption
  • SuperSpeed USB (5 Gbps); USB 2.0 compatible

Evaluate backup approaches by whether copies are isolated from production, encrypted, recoverable within the required time, and verified for data integrity. Match the design to the organization’s scale and recovery objectives; a single external drive may hold an offline copy for a suitably small organization, but it is not a whole-enterprise recovery design.

7. Prioritize and stage restoration

Use the organization’s criticality analysis to establish which systems and data need to return first. Restore verified systems in a planned order, checking data integrity and appropriate access controls before returning them to clinical use.

HHS guidance calls for recovering by restoring lost data and returning to normal operations. Prioritizing essential applications and staging their return applies that contingency-planning goal to the organization’s actual clinical dependencies. Document readiness checks and who approved each service for use.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

8. Keep care moving through contingency operations

Activate business-continuity or emergency-operations procedures while technical responders work. Depending on the service and setting, downtime workflows may include paper processes or other approved ways to keep care moving. Make responsibilities clear, protect information handled through fallback processes, and establish how staff will reconcile records after systems return.

Clinical operations should remain part of recovery decisions throughout the incident. A technically restored application is not ready for use if staff cannot rely on its data or follow a safe workflow.

9. Assess, notify, and learn

Determine whether PHI was accessed, acquired, used, disclosed, or exfiltrated. Document the breach-risk analysis, complete applicable notifications, and incorporate lessons into security management and future exercises. Coordinate incident-specific decisions with the organization’s privacy officer and counsel.

OCR’s July 29, 2026 announcement about OSF Healthcare illustrates why these duties matter: OCR said a ransomware actor exfiltrated PHI for 53,907 individuals and identified potential failures involving risk analysis and timely breach notification. That is a particular enforcement matter, not a prediction of penalties for another organization. In the announcement, OCR Director Paula M. Stannard said: “An accurate and thorough HIPAA risk analysis is not only required by law, but it is also necessary to protect health information and prevent or mitigate ransomware attacks.”

Is it a HIPAA breach if ransomware infects a covered entity’s or business associate’s computer system?

Ransomware presence is a security incident under the HIPAA Security Rule. Whether the incident is also a breach requiring notification depends on the facts and must be assessed and documented; an infection does not mean every organization has the same notification outcome.

HHS says that when ePHI is encrypted by ransomware, a breach has occurred because unauthorized persons have taken possession or control of the information. A breach is presumed unless the organization demonstrates a low probability that the PHI was compromised under the Breach Notification Rule. The organization should assess the facts, document its analysis, and determine applicable notification duties with its privacy officer and counsel.

What should leaders measure during recovery?

Track measures that connect technical restoration to safe service delivery. The right targets depend on the organization’s approved recovery objectives and clinical priorities.

  • Clinical continuity: whether essential services can continue through restored systems or approved downtime procedures.
  • Restoration readiness: whether backups have been tested and restored data verified before production use.
  • Incident scope: which systems, applications, networks, and ePHI may be affected, and whether the threat remains active.
  • Safe return to service: whether vulnerabilities and access paths have been addressed and access is appropriate.
  • Accountability: whether response decisions, breach-risk analysis, and applicable notifications are documented.

These measures help avoid a narrow definition of success in which systems are online but care workflows, data integrity, or incident obligations remain unresolved.
