A mild hamstring strain can often start with home care, but severe pain, trouble standing or walking, substantial or worsening bruising or swelling, numbness or tingling, or symptoms that are not improving should prompt medical advice. A clinician can assess the injury, decide whether imaging is useful, and guide a gradual return to activity. Recovery varies; symptom-duration estimates are not the same as a clearance date for work, exercise, or sport.

When should I see a doctor for a hamstring injury?

Seek urgent medical advice if the pain is very severe or getting worse, swelling or bruising is large or increasing, the leg is very stiff or difficult to move, or standing or walking is painful. NHS guidance also advises seeking help if the injury is not improving with self-care or if you have a fever and feel hot, cold, or shivery. NHS: Hamstring injury

Do not wait to be assessed if you cannot bear weight or cannot walk more than four steps without a lot of pain, a threshold identified by Mayo Clinic. Contact a health professional for sudden numbness or tingling, a sudden increase in pain or swelling, or healing that is not progressing as expected. When home care may be reasonable

If symptoms are mild, you can walk and bear weight, and pain and swelling are not worsening, initial self-care may be appropriate. For the first two to three days, NHS guidance recommends protecting the leg, resting from activities that hurt, applying ice wrapped in cloth, using snug compression, and elevating the leg. What should I expect from a doctor’s appointment?

The clinician will usually ask how and when the injury happened, what activity you were doing, whether you felt or heard a pop or tearing sensation, and which movements make the pain better or worse. The physical exam may include checking tenderness and swelling and moving the leg to locate the injury and assess function. Tests are chosen based on the suspected injury

Imaging is not automatically needed for every strain. If the exam suggests a more serious injury, a clinician may use an X-ray to look for an avulsion fracture, or ultrasound or MRI to assess muscle or tendon damage. How to prepare

  • Note when symptoms began and how they have changed.
  • Be ready to describe the activity and movements involved, including whether you felt a pop.

How long does hamstring injury recovery take?

Recovery depends on the injury’s severity and location, and a short-term symptom estimate does not tell you when the hamstring is ready for full activity. MedlinePlus, in aftercare instructions reviewed June 27, 2026, gives these broad symptom-duration estimates:

What treatment and rehabilitation might involve

Early care generally focuses on reducing pain and swelling. As symptoms allow, rehabilitation progresses toward movement, strength, and everyday function. A clinician or physical therapist can tailor progression to the injury rather than relying on a fixed calendar. For athletes, the 2022 clinical practice guideline recommends comprehensive, progressive rehabilitation. Clinicians may consider hamstring strength, pain at the time of injury, days until pain-free walking, and the area of tenderness when estimating return-to-play timing. Most strains are treated without surgery. Severe tears, selected complete tendon tears, or injuries with substantial loss of function may warrant specialist evaluation and, in some cases, surgery.
