If you want a game that starts somewhere other than conventional horror, Doki Doki Literature Club!, Subnautica, Inscryption and Dredge offer four different routes into unease. This is a spoiler-light editorial ranking, not an objective score: it weighs how far each opening sits from horror, how the tension develops, and what kind of fear the game creates. The available evidence supports these four examples; adding three more just to reach seven would make the list less reliable.
How these games are ranked
The order reflects three practical questions: how strongly the opening suggests a non-horror activity, whether the unease arrives gradually or through a sharper tonal change, and how distinctive the resulting fear feels. It is an editorial comparison, not a published rating system. The descriptions below avoid explaining the twists that make these games memorable.

- Starting impression: the genre or activity the game first appears to offer.
- Pace: whether tension builds over time or the tone changes more abruptly.
- Kind of fear: psychological disturbance, environmental dread or a more overt horror blend.
1. Doki Doki Literature Club!
Why it makes the list
It begins with the familiar surface cues of a cute dating or literature-club visual novel, then moves into psychological horror. That gap between its approachable presentation and its later tone makes it the clearest sharp-shift example here. Common Sense Media’s review describes that change and notes suicide and self-harm themes. Treat those themes seriously; the tonal surprise does not make them trivial.

What to know before playing
This is the most spoiler-sensitive pick in the group, so it is better approached with minimal plot details. Nintendo also includes Doki Doki Literature Club Plus in an official feature on games with plot twists. The original game and Plus are separate edition references, so check which one you intend to play rather than assuming their listings or contents are interchangeable.

2. Subnautica
Why it makes the list
Subnautica starts as open-world underwater exploration, not as a game presented as horror. Its tension comes from venturing into an unfamiliar environment and not knowing what lies ahead. That slow, place-driven dread is different from a sudden plot twist, and it is the strongest example here of fear emerging from exploration itself.
What the developers said
The development team described the intention directly in a PlayStation Blog feature: “We didn’t set out to make a scary game when we started developing Subnautica five years ago. We wanted to make an open-world underwater game where you didn’t kill creatures. Yet, some of our biggest fans tell us we ‘accidentally made a horror game.’” That supports calling it an unexpected horror experience, not claiming it was marketed as one.

However, combat isn't your only option, as you have a multitude of tools at your disposal to aid in sneaking past any creatures that wish to do you harm.
3. Inscryption
Why it makes the list
Inscryption is a stranger, more openly hybrid case: its official description already names psychological horror alongside card-based deckbuilding and escape-room-style puzzles. Its place here is less about hiding the horror entirely and more about how an unusual combination of activities can lead into an unsettling experience. That makes it a useful contrast with games whose opening gives little indication of horror.

What to expect
PlayStation’s official listing describes that mix and lists PS4 and PS5 versions. Nintendo also includes the game in its plot-twist feature. Those listings support the game’s identity and broad description, but they do not establish current retailer stock or a physical edition.

Why it makes the list
Dredge opens with the approachable activity of fishing, then layers eerie dangers and an unsettling atmosphere into that premise. Its shift is more of a slow burn than a single sharp turn: the contrast between an ordinary-seeming pursuit and what surrounds it is the point. A TechRadar 2025 roundup describes it as initially resembling an innocent fishing simulator before darker fears emerge. That is secondary editorial coverage, rather than a developer statement.

- For a slow atmospheric build around a familiar activity: pick Dredge.
