Memory or thinking changes deserve a conversation with a clinician, but a complaint—or a blood-test result—does not establish Alzheimer’s disease on its own. Diagnosis usually draws on the pattern of changes, their effect on daily life, cognitive testing, medical history, and, when appropriate, laboratory tests, brain imaging, or biomarkers.
What to do if you notice memory or thinking changes
Arrange an appointment with a health professional if changes are persistent, worsening, or interfering with everyday activities. Bring someone who knows you well if you are comfortable doing so; a family member or friend may be able to describe changes you have not noticed. A memory concern is a reason to evaluate, not a diagnosis.
Before the visit, note when the changes began and a few specific examples—such as trouble following a familiar recipe, managing bills, or finding words. Bring a list of prescription and over-the-counter medicines and supplements, and jot down relevant changes in sleep, mood, health, or daily function. This information helps the clinician understand the situation; it is not a self-test.
What happens during an Alzheimer’s evaluation
The clinician considers several kinds of evidence rather than relying on one symptom or test. The National Institute on Aging (NIA) describes a process that may include:

- Health and symptom history: Questions for you and, when possible, someone close to you about health, medicines, prior medical problems, daily activities, and changes in behavior or personality.
- Cognitive assessment: Tasks that may assess memory, problem solving, attention, counting, and language.
- Medical tests: Standard blood, urine, or other tests to look for health factors that could contribute to symptoms.
- Further evaluation when indicated: A psychiatric evaluation, cerebrospinal fluid (CSF) testing through lumbar puncture, or brain imaging such as CT, MRI, or PET.
These assessments can help support an Alzheimer’s diagnosis and investigate other explanations. The specific workup depends on the person’s symptoms and circumstances; not everyone needs every test. Repeat assessments may also help show how thinking abilities change over time. NIA’s diagnosis guidance describes these evaluation methods.
Can a blood test diagnose Alzheimer’s early?
A blood test may provide evidence about biological changes associated with Alzheimer’s, but a result is not a stand-alone diagnosis. The NIA says, “At present, blood test results alone should not be used to diagnose dementia but may be taken into consideration along with other tests.” A clinician interprets biomarkers alongside symptoms, function, examination, and other relevant evidence. Availability of diagnostic blood tests is limited and may depend on state-specific access and FDA-related guidance. Read the NIA’s current diagnosis guidance and ask your clinician what is available locally.

One study illustrates why performance figures need context. In an August 2024 summary of a study published in JAMA, NIH Research Matters reported that the evaluated blood test predicted an Alzheimer’s diagnosis with 88% to 92% accuracy across study participants. In the same study, clinical evaluations were reported as 73% accurate in specialty memory clinics and 61% in primary care. These figures describe that test and study—not every blood test or an individual’s chance of receiving a correct result. The study population was Swedish, and the summary said replication in more diverse populations was needed. NIH Research Matters explains the study and its limits.
Biomarkers can also be assessed through CSF tests and brain imaging. They are not interchangeable: each measures different evidence, has different practical demands, and must be interpreted in the clinical context. NIA also describes algorithms combining accessible tests such as blood biomarkers and brief cognitive assessments as research efforts, not as established proof that one screening approach can diagnose Alzheimer’s in people without symptoms in routine care. What else can cause memory or thinking problems?
Alzheimer’s is one possible explanation, but cognitive symptoms can also be associated with stroke, a tumor, Parkinson’s disease, sleep disturbances, medication side effects, infection, or other dementias. Some causes may be treatable and possibly reversible. That is why an evaluation looks beyond Alzheimer’s and reviews medicines, health history, and other symptoms rather than assuming the cause from memory changes alone. The NIA discusses these alternatives in its diagnosis guidance.

Knowing the cause of symptoms can give a person and family a clearer basis for discussing what is happening and planning next steps. An early diagnosis can also create an opportunity to ask about clinical trials or research studies testing possible treatments. A diagnosis does not guarantee trial eligibility, access to a particular treatment, or a specific outcome; those depend on the person and the options available.
